The cases of eighty-six patients in whom eighty-nine open fractures of the femoral shaft had been treated by intramedullary nailing with reaming were retrospectively reviewed. Twenty-seven fractures were classified as grade-I open fractures; sixteen, as grade-II open fractures; and forty-six, as grade-III open fractures. Immediate intramedullary nailing was done for fifty-six fractures, and delayed stabilization (five to seven days after delayed closure of the wound) was done for thirty-three fractures. A prerequisite for immediate intramedullary nailing was that irrigation and debridement of

PURPOSE: Pediatric negative pressure wound therapy (NPWT) is widely used, but etiology- and age-informed real-world benchmarks are limited. We compared treatment burden and closure trajectories across pediatric wound categories.
METHODS: Retrospective single-center cohort of children (< 18 years) treated with NPWT for open wounds (2020-2025). Wounds were categorized as surgical (G1), acute (G2), chronic (G3), and burns (G4). NPWT was performed in accordance with an age-adapted institutional protocol in continuous mode; dressing changes were usually performed every 72 h.
RESULTS: 106 patients

OBJECTIVE: Managing chronic wounds is associated with a burden to patients, caregivers, health services and society and there is a lack of clarity regarding the role of dressings in improving outcomes. This study aimed to provide understanding on a range of topics, including: the definition of chronicity in wounds, the burden of illness, clinical outcomes of reducing healing time and the impact of early interventions on clinical and economic outcomes and the role of matrix metalloproteinases (MMPs) in wound healing.
METHOD: A systematic review of the literature was carried out on the role of

Amputation is most closely associated with blunt, lower limb injuries associated with vascular trauma. These vascular injuries require a special attention to prevent life and limb loss. Patient outcomes can also be improved by organizing vascular trauma data into appropriate systems to facilitate future studies.

During the 4-year military conflict in Croatia, we treated operatively 7,928 casualties. Of those casualties, 172 (2.2%) had penetrating liver injury, mostly sustained by explosive devices. Of these injuries, 90.7% were associated with the trauma of other abdominal and extra-abdominal organs. Seventy-five percent of injuries belonged to grades III and IV on the Liver Injury Scale. The main method of treatment was debridement with ligation of severed vessels and bile ducts. In 8.1% of cases with detrimental bleeding, we used liver packing. Fifty percent of these patients have survived but with

Trauma victims are a significant part of the donor pool, though injuries to donor organs normally exclude them from their procurement. We present the case of a 37-year-old male with a stab wound to the chest, resulting in penetrating sharp force injury to the right ventricle of the heart. Despite the ultimately fatal injury, the heart was successfully repaired, and later donated, after brain death. Resuscitation and surgical intervention, along with the collaboration of medical and legal parties, enabled heart donation to occur without compromising the quality of forensic postmortem examinatio

This case report describes an occurrence of a traumatic injury to the inferior vena cava (IVC) secondary to penetrating trauma. A 37-year-old male patient presented to the emergency department (ED) after sustaining a stab wound to the mid back. The patient was transferred directly to the operating theatre (OT) and underwent an emergency exploratory laparotomy. A through-and-through IVC injury at the level of the entry to the liver bed was identified. Surgical repair was attempted but failed, followed by eventual IVC ligation. The patient was shifted to the intensive care unit postoperatively f

CASE: A 31-year-old woman suffered a close-range, high-energy .30-rifle gunshot wound to her right ilium and sacrum resulting in an unstable pelvic ring injury with significant internal soft-tissue damage and bone loss. Given the limited amount of literature for managing this rare clinical scenario in a civilian setting, we described our protocol used on this patient to achieve a safe and effective result.
CONCLUSIONS: We present a civilian-inflicted high-velocity pelvic gunshot injury at close range. Although the patient had extensive pelvic bone loss and soft-tissue damage, she had excellen

Fatal injuries caused by circular saws are rare and mainly self-inflicted. To the best of our knowledge this is the first recorded case of accidental death of a woman caused by an injury to the head region inflicted by a saw blade. The autopsy showed a 35 cm long wound, stretching from the right half of the jaw along the right side of the face and the right temporal region up to the parietal region. The cut went through the right hemisphere of the brain in the temporal and parietal lobe regions, damaging the sagittal sinus. Air embolism was considered to be the cause of death. Fatalities invol

The authors report on a case of a 43-year-old male pedestrian struck and run over by a truck. The accident was accompanied by a mechanism of injury resulting in a unique autopsy picture of trunk and head injuries. The internal organs of the neck (larynx, trachea, esophagus), chest (lungs, thoracic aorta), and part of the abdominal cavity (bottom of the stomach and fragments of the liver) were dislocated in the direction of the head. The result was a traumatically deformed, crushed skull characterized by an extensive gaping wound with excerebration. The organs were dislocated through a channel

Corneal injuries are among the major causes of ocular morbidity and vision impairment. Optimal epithelial wound healing is critical for the integrity and transparency of the cornea after injury. Hepatocyte growth factor (HGF) is a mitogen and motility factor that primarily regulates epithelial cell function. Herein, we investigate the effect of HGF on proliferation of corneal epithelial cells (CECs) in inflamed conditions both in vitro and in vivo. We demonstrate that HGF not only promotes CEC proliferation in homeostatic conditions but also reverses the anti-proliferative effect of the inflam

BACKGROUND: The 25% of traumatic deaths are due to chest trauma with a mortality of 28%, and closed trauma is the most common mechanism of injury. The use of firearms increases the relative risk of death. The five more frequent specific chest injuries are lung contusion (31.8%), hemothorax/pneumothorax (19.4%), rib fractures (13.2%), and diaphragmatic injury (7.5%).
CLINICAL CASE: Patient of 25 years with a single firearm projectile wound in the chest who assisted to the emergency room (ER) with significant respiratory distress and meriting surgical emergency airway, documenting double pneumo

PURPOSE: To describe a case of remarkable visual recovery after severe open globe injury.
OBSERVATIONS: We present a case of a 70-year-old man with an open globe injury with no light perception vision before and after primary repair of his ruptured globe and before secondary vitreoretinal surgery to repair a total retinal detachment with a 360° giant retinal tear and retinal incarceration in a posterior scleral wound who proceeded to recover vision to the 20/60 pinhole to 20/50 level.
CONCLUSIONS AND IMPORTANCE: Poor presenting acuity is a known risk factor for poor visual outcome after open

An unusual case of facial gunshot injury with the missile lodged in the cervical spinal canal, but without any neurological impairment is reported. The extent of tissue damage and missile track termination in a male patient who sustained gunshot trauma to the face was assessed by plain radiography and by CT scans. The patient was treated conservatively and observed for clinical manifestations of neurological deficit for 3 weeks. CT of the head and neck performed 13 years after injury with the three-dimensional (3D) reconstruction of skeletal elements revealed healed fractures of the right nasa

BACKGROUND: The Rapid Deployment Hemostat (RDH) Bandage was developed for the rapid control of bleeding caused by trauma.
METHODS: An extremity wound involving skin, muscle, bone, and femoral arterial injury and a 1-cm vertical incision in the abdominal aorta in swine were studied to compare the RDH Bandage, a fibrin sealant dressing and gauze to restore hemostasis. The total blood loss was determined and the survival of animals was measured.
RESULTS: In the extremity injury model, the RDH Bandage reduced blood loss by 63% compared with the gauze control. In the aorta arterial incision model

Early anatomic (or near anatomic; lingers do not impinge or overlap during flexion or extension) stable fracture fixation provides the foundation for successful wound management and for the repair, reconstruction, and healing of all damaged tissues in a mutilating hand injury. It also plays an instrumental role in pain control and affords an optimal opportunity for timely and favorable rehabilitation of and recovery from mutilating injuries of the hand. Kirschner or other wiring systems or mini external fixators may be used for simple fractures, in children, when rapid fracture fixation is nec
