Contribution of immunofluorescence to the identification and characterization of anti-neutrophil cytoplasmic autoantibodies. The role of different fixatives.

OBJECTIVE: To study the sera from selected groups of antineutrophil cytoplasmic antibody (ANCA) positive patients by means of the indirect immunofluorescence test (ANCA-IIF) with different fixatives, in order to better discriminate among the various ANCAs (Ag-specificity and disease associations), especially those for which the antigen targets have not yet been identified. METHODS: Eighty pathological serum samples and 15 normal sera were evaluated. Pathological samples included sera from 30 ulcerative colitis (UC) ANCA positive patients, 30 P-ANCA/myeloperoxidase (MPO-ANCA) positive microsco

Systemic necrotizing vasculitides in severe alpha1-antitrypsin deficiency.

We describe the clinical presentation and outcome in a series of eight patients with systemic necrotizing vasculitis and severe alpha1-antitrypsin (AAT) deficiency followed up at three Swedish hospitals during 1968-92. We also review six other cases reported in the literature during the same period. Diagnosis of severe AAT deficiency was based on the presence of the PiZZ phenotype, or low plasma total trypsin inhibitory capacity, or a low plasma AAT concentration (10-40% of the normal mean value) and presence of the PiSZ or PiFZ phenotype. The diagnosis of systemic vasculitis was biopsy-verifi

[Angiodysplasias or vascular ectasias of the intestine. Report on 8 personal cases].

Angiodysplasia or vascular ectasia of the intestine is a new pathological entity which was initially revealed by arteriography. More recently it has been found by coloscopy and is considered to be responsible for many cases of intestinal bleeding, especially in older patients The present study is based on eight patients including 2 cases of microscopic bleeding, 4 of massive hemorrhage, and 2 asymptomatic cases. There is a clear difference between intestinal angiodysplasia and other forms of vascular malformations, even though its pathogenesis is still in question. Whereas in older patients it

Disease Outcomes in Immune-Mediated Colitis: A Study Describing Initial Endoscopic and Histologic Presentations and Treatment Options.

BACKGROUND: Immune-mediated colitis (IMC) is a toxicity associated with immune checkpoint inhibitors (ICIs) that is becoming increasingly common. Studies exploring the clinical course and outcomes of IMC have been limited to relatively small sample sizes (<200 patients). We therefore aimed to provide a comprehensive account of the clinical, endoscopic, and histologic features of IMC as well as the efficacy of IMC treatment in a representative sample. METHODS: This was a single-center retrospective study of all patients who received ICIs at our institution between January 2010 and February 202

The emerging role of histologic disease activity assessment in ulcerative colitis.

BACKGROUND AND AIMS: Assessment of disease activity is essential for developing and determining appropriate therapy in patients with ulcerative colitis (UC). Validated clinical and endoscopic scoring systems have been established to accurately define disease activity. Clinical and endoscopic treatment targets have also been proposed, with gastroenterologists encouraged to optimize medical therapy to achieve these targets. Recently, histology has been recognized as an important prognostic factor and potential treatment target in patients with UC. METHODS: This review summarizes the recent lite

Pediatric patients with untreated ulcerative colitis may present initially with unusual morphologic findings.

BACKGROUND: Anecdotal observations by the authors of this study, together with the results of a few previous smaller studies, suggest that children with new-onset (previously untreated) ulcerative colitis (UC) may occasionally present with discontinuous disease, relative or absolute rectal sparing, and may lack histologic features ofchronicity. Therefore, the objectives of this study were to determine the clinical and pathologic features of new-onset UC in children and to compare the initial presentation of UC in this group with a control group of adults. DESIGN: Routinely processed rectal an

Antineutrophil cytoplasmic autoantibody in the absence of Wegener's granulomatosis or microscopic polyangiitis: implications for the surgical pathologist.

Antineutrophil cytoplasmic antibodies (ANCA) are useful serologic markers for the diagnosis and management of patients with Wegener's granulomatosis (WG) and microscopic polyangiitis (MPA). However, problems in diagnosis and classification may occur when patients with other disorders develop ANCA. A 7-year review (1993-1999) disclosed 247 patients whose sera tested positively for ANCA by an indirect immunofluorescence method: 166 patients for cytoplasmic-ANCA (C-ANCA) and 81 patients for perinuclear-ANCA (P-ANCA) Twenty-seven patients had active pulmonary disease and underwent open-lung biopsy

Study of the Mechanism of Xianglian Anchang Decoction Ameliorating Ulcerative Colitis Based on Transcriptomics and Network Pharmacology Analysis.

INTRODUCTION: The therapeutic efficacy of Xianglian Anchang Decoction (XLAC) in Ulcerative Colitis (UC) has been established, but the underlying pharmacological mechanisms have remained poorly elucidated. In this study, we employed an integrated strategy combining network pharmacology, transcriptomic profiling, and in vivo experimentation to reveal these mechanisms, with a particular focus on how XLAC modulates the IL-17/AP-1 signaling axis to attenuate intestinal inflammation and promote mucosal repair. METHODS: UC was induced in mice by 2% Dextran Sulfate Sodium (DSS) administration in drin

Molecular hydrogen suppresses gut inflammation and pyroptosis in ulcerative colitis through promoting PKM2 lactylation to block NLRP3 inflammasome activation.

BACKGROUND: Molecular hydrogen is considered to be able to alleviate the progression of ulcerative colitis (UC), whose underlying molecular mechanism remains largely unclear. METHODS: Lipopolysaccharide (LPS)/adenosine triphosphate (ATP)-stimulated human colonic epithelial cells (HCoEpiC) were used to mimic UC model in vitro and were treated with hydrogen-rich medium (HRM). Dextran sulfate sodium (DSS)-induced mice were used to construct UC model in vivo and were administrated by hydrogen-rich water (HRW). Colon tissues from UC patients and healthy volunteers were collected. Release of inflam

EruA, a Regulator of Adherent-Invasive E. coli, Enhances Bacterial Pathogenicity by Promoting Adhesion to Epithelial Cells and Survival Within Macrophages.

Adherent-invasive E. coli (AIEC) is closely related to inflammatory bowel disease (IBD). However, its pathogenic mechanism has not yet been fully elucidated. Using a BLASTP search, we discovered that the amino acid sequence of a putative protein (UFP37798.1) in the AIEC LF82 strain is highly homologous to some regulators in the SlyA family. We named it EruA. We displayed the secondary structures of EruA using bioinformatics, overexpressed the His6-tagged EruA protein using SDS-PAGE, and dissected the genetic organization of the eruA chromosomal region using 5'RACE. We constructed an eruA delet

[Protective effect of ethyl syringate against ulcerative colitis based on JAK2/STAT3 pathway].

To study the therapeutic effect and mechanisms of ethyl syringate(MD) on ulcerative colitis(UC), the MTT assay was used to detect the proliferation inhibition of RAW264.7 cells and HT-29 cells by different concentrations of MD(50, 100, 200, 400 μmol·L~(-1)). UC cell models were constructed by inducing RAW264.7 cells and HT-29 cells with lipopolysaccharide(LPS) and tumor necrosis factor-α(TNF-α). An animal model was established by inducing mice with 2.5% dextran sulfate sodium(DSS) to verify the therapeutic effect of MD on UC. A control group, a model group(LPS or TNF-α), and groups treated wit

Number of intraepithelial lymphocytes and presence of a subepithelial band in normal colonic mucosa differs according to stainings and evaluation method.

Chronic watery diarrhea is a frequent symptom. In approximately 10% of the patients, a diagnosis of microscopic colitis (MC) is established. The diagnosis relies on specific, but sometimes subtle, histopathological findings. As the histology of normal intestinal mucosa vary, discriminating subtle features of MC from normal tissue can be challenging and therefore auxiliary stainings are increasingly used. The aim of this study was to determine the variance in number of intraepithelial lymphocytes (IELs) and presence of a subepithelial band in normal ileum and colonic mucosa, according to differ

Preparation, Characterization, and In Vitro Evaluation of Chlorogenic Acid Loaded Hydrogel Beads for the Management of Ulcerative Colitis.

Ulcerative colitis (UC) is a chronic inflammatory colon disorder. Several modern medicines have been used for UC treatment but are associated with side effects. Hence, herbal medicine-inspired lead molecules are promising for managing UC. Chlorogenic acid (CGA), an herbal bioactive, has been reported for anti-inflammatory, anticancer, antioxidant, and immunomodulatory activity. The current study aimed to develop enteric-coated mucoadhesive beads of CGA for colon targeting. CGA-loaded beads were prepared using chitosan and carrageenan as polymers through an ionic gelation technique. Furthermore

The Rectal Stump During and After Subtotal Colectomy for Ulcerative Colitis: A Narrative Review of Surgical Strategies, Medical Management Options, and Cancer Surveillance Recommendations.

Background/Objectives: There are multiple decision nodes, during and after subtotal colectomy for ulcerative colitis (UC), regarding the management of the rectal stump. Intra-operatively, the surgeon must decide on the closure technique and positioning of the retained stump, while post-operatively, clinicians often face the challenge of managing diversion proctitis, as well as determining an appropriate endoscopic surveillance strategy, given the increased risk of cancer. This narrative review aims to summarise the evidence relating to these key decision points in rectal stump management. Meth

Interobserver Reliability of the Nancy Index for Ulcerative Colitis: An Assessment of the Practicability and Ease of Use in a Single-Centre Real-World Setting.

BACKGROUND: Histological disease severity assessment in ulcerative colitis [UC] has become a mainstay in the definition of clinical endpoints ['histological remission'] in clinical trials of UC. Several scores have been established in the microscopic assessment of disease activity, but the Nancy index [NI] stands out as being the histological index with the fewest scoring items. To what extent histological assessment using the NI is affected by interobserver reliability in a real-word setting is poorly understood. We therefore performed a single-centre retrospective analysis of NI assessment i

Case Report: Proteinase 3 Antineutrophil Cytoplasmic Antibody-Associated Ulcerative Colitis Presenting as Recurrent Intestinal Pseudo-Obstruction in a Teenage Patient With in situ Proteinase 3 Immunohistochemical Staining.

Ulcerative colitis (UC) is a chronic relapsing inflammatory bowel disease with the colorectum as its major target organ. Involvement of the upper gastrointestinal tract in UC is rare and presents with nonspecific endoscopic and microscopic characteristics. Recent studies have demonstrated proteinase 3 antineutrophil cytoplasmic antibody (PR3-ANCA) to be a serological marker for differentiating UC from Crohn's disease in children and for detecting disease activity and nonresponse to steroid therapy and antitumor necrotizing factor-α agents. Herein, we report a 13-year-old female patient mainly

Systemic Immune Signatures of Endoscopic-Histologic Discordance in Inflammatory Bowel Disease: A Pilot Study.

Background: Endoscopic remission is a central therapeutic target in inflammatory bowel disease (IBD); however, histologic inflammation may persist despite apparent mucosal healing. The biological mechanisms underlying endoscopic-histologic discordance remain incompletely defined. We aimed to characterize systemic cytokine patterns associated with discordant disease. Methods: In this prospective cross-sectional study, 59 patients with IBD undergoing clinically indicated colonoscopy underwent concurrent endoscopic and histologic assessment. Endoscopic remission was defined as a Mayo endoscopic s
