Toward more intuitive prosthetic control: structured residual muscle training in transtibial amputees.

BACKGROUND: Myoelectric control of lower-limb prostheses is challenging due to residual muscle co-contraction, inconsistent activation, and signal variability. Evidence regarding the efficacy of training interventions targeting residual muscle activation in transtibial amputees (TTAs) to enhance prosthetic control remains limited. This study investigated whether a 4-week structured training could improve residual muscle control in TTAs. METHODS: Nine male unilateral TTAs were assigned to either (A) biofeedback training with daily home exercises or (B) home exercises alone. Biofeedback sessions

Gait Acquisition with a Quadrilateral Socket after Ultra-short Transfemoral Amputation because of Staphylococcal Toxic Shock Syndrome.

BACKGROUND: Streptococcal toxic shock syndrome (STSS) is a notifiable disease under Japan's Infectious Disease Control Law and has become a pandemic following COVID-19. STSS often leads to necrotizing fasciitis, with a mortality rate exceeding 30%. Even in surviving patients, limb amputations are common. CASE: A 39-year-old woman developed STSS. She underwent 18 surgeries and vacuum-assisted closure therapy, which resulted in ultra-short right transfemoral amputation. With a strong desire to walk again, she began fitting for a provisional prosthesis 3 months post-amputation. Given the preserv

A Pilot Investigation into Targeted Muscle Reinnervation for Complex Regional Pain Syndrome, Type II.

UNLABELLED: Complex regional pain syndrome (CRPS) is a debilitating condition, characterized by severe pain with vascular, motor, or trophic changes. Varied presentations make this a diagnostic and therapeutic challenge. There is a lack of high-quality evidence demonstrating efficacy for most existing therapies, particularly with surgical intervention for type II CRPS (CRPS-II). Targeted muscle reinnervation (TMR) is a surgical technique to transfer the terminal end of a divided nerve to a recipient motor nerve, shown to limit phantom limb pain, residual limb pain, and postamputation neuroma p

Life after conflict-related amputation trauma: a clinical study from the Gaza Strip.

BACKGROUND: More than 17.000 Palestinians were injured during different Israeli military incursions on the Gaza Strip from 2006 to 2014. Many suffered traumatic extremity amputations. We describe the injuries, complications, living conditions and health among a selection of traumatic amputees in the Gaza Strip. METHODS: We included 254 civilian Palestinians who had survived, but lost one or more limb(s) during military incursions from 2006 to 2016. All patients were receiving follow-up treatment at a physical rehabilitation center in Gaza at the time of inclusion. We measured and photographed

Developing Non-Somatotopic Phantom Finger Sensation to Comparable Levels of Somatotopic Sensation through User Training With Electrotactile Stimulation.

Cutaneous electrical stimulation can provide tactile feedback for upper-limb amputees through somatotopic feedback (SF) or non-somatotopic feedback (NF). The SF delivers electrotactile stimulus to projection finger maps (PFMs) on the stumps of amputees, which outperforms NF that transfers stimulus to other human intact skin areas in general. However, the SF areas on stumps are very limited and often occupied by electromyography (EMG) sensors in application of myoelectric prosthesis. This work aims at improving NF performance on human upper arms through user training with electrotactile stimula

Spinal cord stimulation for intractable pain following limb amputation.

OBJECTIVE: €‚ Spinal cord stimulation (SCS) by high-frequency electrical pulses has been used since the early 1970s for relief of chronic intractable pain following limb amputation. The long-€term effectiveness of SCS for amputation-€related pain with ongoing after-care is reviewed by assessment of all such cases managed over 20 years in the Neurostimulator Clinic at the Royal London Hospital. MATERIALS AND METHODS: €‚ Twelve amputation-€related pain patients had quadripolar paddle electrodes (Resume; Medtronic Inc., Minneapolis, MN, USA) inserted epidurally by laminectomy over the thoraci

Dorsal column stimulator applications.

BACKGROUND: Spinal cord stimulation (SCS) has been used to treat neuropathic pain since 1967. Following that, technological progress, among other advances, helped SCS become an effective tool to reduce pain. METHODS: This article is a non-systematic review of the mechanism of action, indications, results, programming parameters, complications, and cost-effectiveness of SCS. RESULTS: In spite of the existence of several studies that try to prove the mechanism of action of SCS, it still remains unknown. The mechanism of action of SCS would be based on the antidromic activation of the dorsal co

A randomised double blind trial of the effect of pre-emptive epidural ketamine on persistent pain after lower limb amputation.

Persistent pain has been reported in up to 80% of patients after limb amputation. The mechanisms are not fully understood, but nerve injury during amputation is important, with evidence for the crucial involvement of the spinal N-methyl d-aspartate (NMDA) receptor in central changes. The study objective was to assess the effect of pre-emptively modulating sensory input with epidural ketamine (an NMDA antagonist) on post-amputation pain and sensory processing. The study recruited 53 patients undergoing lower limb amputation who received a combined intrathecal/epidural anaesthetic for surgery fo

Virtual Integration Environment as an Advanced Prosthetic Limb Training Platform.

Background: Despite advances in prosthetic development and neurorehabilitation, individuals with upper extremity (UE) loss continue to face functional and psychosocial challenges following amputation. Recent advanced myoelectric prostheses offer intuitive control over multiple, simultaneous degrees of motion and promise sensory feedback integration, but require complex training to effectively manipulate. We explored whether a virtual reality simulator could be used to teach dexterous prosthetic control paradigms to individuals with UE loss. Methods: Thirteen active-duty military personnel with

Characteristics of Affected Limb Pain and Associated Factors in Young Adult Survivors of Lower Limb Osteosarcoma in Japan.

PURPOSE: This study aimed to characterize affected-limb pain and its associated factors among adolescent and young adult (AYA) survivors of lower extremity osteosarcoma in Japan. We hypothesized that both physical and psychosocial factors would be associated with pain, with an interaction between pain intensity and pain interference. METHODS: A cross-sectional study was conducted among osteosarcoma survivors aged 18-39 years who had completed treatment at least 2 years earlier. Self-administered questionnaires assessed pain intensity and interference, physical function, fatigue, sleep disturb

Prosthesis use among individuals with limb loss after targeted muscle reinnervation/regenerative peripheral nerve interface for symptomatic neuromas: A scoping review.

Neuromas are aberrant axonal growths that may develop after amputations and contribute to residual-limb and/or phantom-limb pain. Burgeoning surgical approaches for treating neuroma pain include targeted muscle reinnervation (TMR) and regenerative peripheral nerve interface (RPNI) surgery. However, little is known regarding functional outcomes postoperatively. The scoping review aimed to examine prosthesis outcomes among individuals with acquired amputations who have undergone TMR and/or RPNI surgery for neuromas. The study protocol was published on Open Science Framework. A search was conduct

Biomechanical differences between overground and treadmill running in professional runnersa pilot study.

Purpose: The aim of this study was to compare the biomechanical parameters of runners during overground and treadmill running, to assess the significance of these differences for treadmill training, and to evaluate their relevance for physiotherapy. Methods: Ten professional runners (mean age of 31.2 ± 6.8 years) were evaluated using a 10-camera Vicon motion capture system and a Phantom V12 high-speed camera. After completing a 200-meter overground run at a self-selected pace, each athlete entered the calibrated capture volume, where their running velocity and kinetic data were recorded. The i

Neuromusculoskeletal Arm Prostheses: Personal and Social Implications of Living With an Intimately Integrated Bionic Arm.

People with limb loss are for the first time living chronically and uninterruptedly with intimately integrated neuromusculoskeletal prostheses. This new generation of artificial limbs are fixated to the skeleton and operated by bidirectionally transferred neural information. This unprecedented level of human-machine integration is bound to have profound psychosocial effects on the individuals living with these prostheses. Here, we examined the psychosociological impact on people as they integrate neuromusculoskeletal prostheses into their bodies and lives. Three people with transhumeral amputa

The effects of neuron morphology and spatial distribution on the selectivity of dorsal root ganglion stimulation.

Objective. For prosthesis users, sensory feedback that appears to come from the missing limb can improve function, confidence, and phantom limb pain. Numerous pre-clinical studies have considered stimulation via penetrating microelectrodes at the dorsal root ganglion (DRG) as a potential approach for somatosensory neuroprostheses. However, to develop clinically translatable neuroprosthetic devices, a less invasive approach, such as stimulation via epineural macroelectrodes, would be preferable. This work explores the feasibility of using such electrodes to deliver focal sensory feedback by exam

Natural Sensations Evoked in Distal Extremities Using Surface Electrical Stimulation.

BACKGROUND: Electrical stimulation is increasingly relevant in a variety of medical treatments. In this study, surface electrical stimulation was evaluated as a method to non-invasively target a neural function, specifically natural sensation in the distal limbs. METHOD: Electrodes were placed over the median and ulnar nerves at the elbow and the common peroneal and lateral sural cutaneous nerves at the knee. Strength-duration curves for sensation were compared between nerves. The location, modality, and intensity of each sensation were also analyzed. In an effort to evoke natural sensations,

Using mirror visual feedback and virtual reality to treat fibromyalgia.

Fibromyalgia is a condition characterized by long term body-wide pain and tender points in joints, muscles and soft tissues. Other symptoms include chronic fatigue, morning stiffness, and depression. It is well known that these symptoms are exacerbated under periods of high stress. When pain becomes severe enough, the mind can enter what is known as a dissociative state, characterized by depersonalization - the feeling of detachment from one's physical body and the illusion of watching one's physical body from outside. In evolutionary terms, dissociative states are thought to be an adaptive me

Mechanisms of tinnitus.

The generation of tinnitus is a topic of much scientific enquiry. This chapter reviews possible mechanisms of tinnitus, whilst noting that the heterogeneity observed within the human population with distressing tinnitus means that there may be many different mechanisms by which tinnitus can occur. Indeed, multiple mechanisms may be at work within one individual. The role of the cochlea in tinnitus is considered, and in particular the concept of discordant damage between inner and outer hair cells is described. Biochemical models of tinnitus pertaining to the cochlea and the central auditory pa
