Lesbian womens' access to healthcare, experiences with and expectations towards GPs in German primary care.

BACKGROUND: Lesbian women have higher rates of physical and psychiatric disorders associated with experiences of discrimination, homophobia and difficulties with coming out. Therefore, easy access to specialized healthcare in an open atmosphere is needed. We aimed to describe women's access to and experiences with healthcare in Germany, and to assess the responsibility of the general practitioner (GP) compared to other specialities providing primary health care. METHODS: A questionnaire study was conducted via internet and paper-based sampling. Using current literature, we designed a question

For partner services, do we need "face-time," or FaceTime?: Trends in relative proportion of in-person notifications and HIV testing after introduction of a telephone option for HIV exposure notification by public health.

Public health usually notifies partners of STD exposure in-person despite availability of other options. We examined trends in in-person versus telephone notification for HIV 3 years after the introduction of a telephone option. Most notifications were made by telephone. Partners notified doubled; however, the proportion HIV testing declined slightly.

A tale worth telling: the impact of the diagnosis experience on disclosure of genetic disorders.

BACKGROUND: Research suggests children with genetic disorders exhibit greater coping skills when they are aware of their condition and its heritability. While the experiences parents have at diagnosis may influence their decision to disclose the diagnosis to their children, there is little research into this communication. The aim of the current study was to examine the relationship between the diagnosis experience and the disclosure experience for parents of children with developmental disorders of a known genetic aetiology: parents of children with 22q11.2 deletion syndrome (22q11DS) were co

Testing for sexually transmitted infections in a population-based sexual health survey: development of an acceptable ethical approach.

Population-based research is enhanced by biological measures, but biological sampling raises complex ethical issues. The third British National Survey of Sexual Attitudes and Lifestyles (Natsal-3) will estimate the population prevalence of five sexually transmitted infections (STIs) (Chlamydia trachomatis, Neisseria gonorrhoeae, human papillomavirus (HPV), HIV and Mycoplasma genitalium) in a probability sample aged 16-44 years. The present work describes the development of an ethical approach to urine testing for STIs, including the process of reaching consensus on whether to return results. T

Casual sexual encounters among gay men: familiarity, trust and unprotected anal intercourse.

Familiarity with and a history of prior sex with casual partners is associated with unprotected anal intercourse and may increase the risk of HIV transmission among gay men. Using data from the Sydney Gay Community Periodic Survey 2007, we explored the relationship between familiarity and unprotected anal intercourse with the last casual partner (UAI-LC). 51% of the men knew their last casual partner and 49% had previously had sex with him. Men were more inclined to engage in UAI-LC if they had previously had sex with this partner. HIV-negative men were more likely to have UAI-LC with a more f

Task and socioemotional behaviors of physicians: a test of reciprocity and social interaction theories in analogue physician-patient encounters.

The purpose of the present study is to assess social interaction and reciprocity theories as explanations for patient responses to a physician in a medical consultation. Social interaction theory predicts that patients mostly recognize and react to socioemotional behavior of their physicians due to a lack of understanding of physician task behaviors or a preoccupation with anxiety. Reciprocity theory predicts that patients recognize socioemotional and task behaviors of their physicians, and they respond to these behaviors in thematically similar ways. We examined these hypotheses by having sub

The disclosure of information to cancer patients and its relationship to their mental state in a consultation-liaison psychiatry setting in Japan.

In Japan, as in some other countries, not all cancer patients are informed of their diagnosis and expected prognosis. However, we do not know enough about the relationship between the kind of information given to cancer patients and their mental state. The following is a study that examines this relationship. The subjects were 81 adult cancer inpatients referred to psychiatrists. Those with mental disorders before admission or with delirium were excluded. The subjects were classified into three groups according to the kind of information given to them: 28 were not informed of a diagnosis of ca

Privacy Concerns and Configurational Patterns of Health Information Sharing Intention Among Patients with Chronic Diseases in Online Health Communities: An SEM and fsQCA Analysis.

BACKGROUND/OBJECTIVES: Privacy concerns are negatively associated with health information sharing intention, but their role within combinations of facilitating conditions remains unclear. This study examined variable-level associations and configurational patterns among patients with chronic diseases. METHODS: This single-center, cross-sectional study used a convenience sample of 428 patients who had used online health communities. The outcome was self-reported sharing intention rather than observed disclosure. Covariance-based structural equation modeling (SEM) assessed adjusted associations

Enhancing Estimation Precision in Sensitive Public Health Data: Implications for Evidence-Based Decision-Making.

INTRODUCTION: Reliable estimation of sensitive public health outcomes, such as underreported COVID-19 cases and vaccine hesitancy, is frequently hindered by social stigma and nonresponse bias, particularly in culturally sensitive contexts like Pakistan. Traditional surveys often underestimate prevalence, limiting the effectiveness of public health interventions. METHODS: Multi-stage stratified paired response frameworks (MSPRFs) incorporating two and three-stage randomization with dual scrambling mechanisms under stratified simple random sampling were applied. A cross-sectional survey of 1,20

Concussion Symptom Underreporting Among Incoming National Collegiate Athletic Association Division I College Athletes.

OBJECTIVE: Accurate diagnosis of sport-related concussions relies heavily on truthful self-reporting of symptom severity. Previous studies have emphasized lack of knowledge as a factor in symptom nondisclosure. This study sought to examine concussion knowledge and the relationship of knowledge to reasons for symptom nondisclosure. DESIGN: Cross-sectional study. SETTING: Data were collected during preparticipation athletic evaluations via electronic survey. PARTICIPANTS: One hundred fifty-six incoming National Collegiate Athletic Association Division I student-athletes. MAIN OUTCOME MEASURE

Examining the Effects of a Service-Trained Facility Dog on Stress in Children Undergoing Forensic Interview for Allegations of Child Sexual Abuse.

Disclosure of child sexual abuse can be a stressful experience for the child. Gaining a better understanding of how best to serve the child, while preserving the quality of their disclosure, is an ever-evolving process. The data to answer this question come from 51 children aged 4-16 (M = 9.1, SD = 3.5), who were referred to a child advocacy center in Virginia for a forensic interview (FI) following allegations of sexual abuse. A repeated measures design was conducted to examine how the presence of a service-trained facility dog (e.g. animal-assisted intervention (AAI) may serve as a mode of l

The Preference to Discuss Expected Survival Is Associated with Loss of Meaning and Purpose in Terminally Ill Cancer Patients.

BACKGROUND: Demoralization is a syndrome of existential distress characterized by loss of meaning and purpose in life, hopelessness, and helplessness. Empirical data on its occurrence and associated factors in terminally ill patients is limited. OBJECTIVE: The study objective was to determine the frequency of demoralization and its association with individual and disease-related characteristics and to analyze the association between demoralization and the preference to discuss expected survival. METHODS: We recruited N = 55 terminally ill cancer patients (54% women, mean age 67 years) within

Posttraumatic growth among men with histories of child sexual abuse.

Despite an increased risk of long-term mental health problems, many survivors of child sexual abuse (CSA) experience positive changes in areas such as appreciation for life, personal strength, and interpersonal relationships. Drawing on life course theory, this study examined factors related to posttraumatic growth among a sample of men with CSA histories (N = 487). Using multiple linear regression (i.e., ordinary least squares), we found that men who had a better understanding of the sexual abuse experience, who ascribed to less traditional masculine norms, and who experienced a turning point

Ethical considerations in paediatric neurology: neuromuscular disease and epilepsy.

The pace of developing technology with respect to many diagnostic tests, as well as available treatments including artificial ventilation, may have progressed at a faster rate than our ethical, humane ability to decide on the optimal choices for our patients. In fact, who should make these choices; physicians or patients and families? Certain ethical aspects of neuromuscular disorders and epilepsy are reviewed. For neuromuscular disease, the example of Duchenne muscular dystrophy (DMD) with regards to genetic testing, relatively early wheelchair placement and individualised invasive ventilatio

Hospice and palliative medicine: new subspecialty, new opportunities.

Palliative care is the physical, psychological, social, and spiritual care provided to patients from diagnosis to death or resolution of a life-threatening illness. Hospice care is a comprehensive program of care that is appropriate when patients with chronic, progressive, and eventually fatal illness are determined to have a prognosis of 6 months or fewer. Hospice and palliative medicine has now been recognized by the American Board of Medical Subspecialties as a field with a unique body of knowledge and practice. With 9 other specialty boards, the American Board of Emergency Medicine has cos

Biographical truths and their clinical consequences: understanding 'embodied memories' in a third psychoanalysis with a traumatized patient recovered from severe poliomyelitis.

The relationship between 'narrative' and 'historical-biographical truth' in psychoanalytic treatment has become the subject of many controversial debates in recent years. Findings of contemporary memory research have lead to great scepticism as to whether therapists are able objectively and reliably to reconstruct biographical events on the basis of their observations in the therapeutic situation. Some authors even claim that psychoanalysts should concentrate exclusively on observing the here and now of the patient's behaviour within the transference relationship to the analyst. In this paper

Uptake of offer to receive genetic information about BRCA1 and BRCA2 mutations in an Australian population-based study.

Research on the utilization of genetic testing services for mutations in BRCA1 and BRCA2 has focused on women with a strong family history of breast and ovarian cancer. We conducted a population-based case-control-family study of Australian women diagnosed with invasive breast cancer before age 40 years, unselected for family history, and tested for germ line mutations in BRCA1 and BRCA2. Case subjects found to carry a deleterious mutation and their relatives who had given a research blood sample were informed by mail that the study had identified "genetic information" and were offered the opp
