Revision operations after previous stapes surgery for persisting hearing loss.

OBJECTIVES: The aim of the study was to find out the reasons of the recurrent or persisting hearing loss after previous stapes surgery indicated for otosclerosis. BACKGROUND: Revision stapes surgery is a relatively safe surgical method. Recurrent or persisting conductive hearing loss is commonly caused by prosthesis dislocation and adhesions in the oval window. Hearing loss is directly proportional to the number of previous operations. METHOD: Retrospective analysis of 48 patients after revision stapes surgery was done over a period of 4 years (2005-2008). Improvement of the hearing and the

Endoscopic management of limited attic cholesteatoma.

OBJECTIVES: Microscopic postauricular tympanomastoidectomy provides a limited exposure to the attic, especially anteriorly. In contrast, the endoscope offers wide transcanal access to the attic, allowing for complete removal of limited attic disease, possibly without interrupting the ossicular chain. This report evaluates 8 years of experience with transcanal endoscopic management of limited attic cholesteatoma. STUDY DESIGN: Case series. METHODS: Seventy-three ears with limited attic cholesteatoma underwent endoscopic transcanal tympanotomy and extended atticotomy to access and completely r

[Congenital fistula of the stapedial footplate as an avenue for recurrent purulent meningitis].

INTRODUCTION: Congenital fistula of the stapedial footplate is a rare cause of recurrent purulent meningitis and a diagnostic and therapeutic problem as well. CASE REPORT: We report a case of a stapedial fistula diagnosed intraoperatively in a 4-year-old boy, after recurrent meningitis. The boy was hospitalized at the Pediatric Clinic three times in a 6-month period because of recurrent meningitis. The applied anti-meningitis therapy presented with good general state of the child, but the main target was to discover the cause of recurrent meningitis. Immunologic and genetic investigations wer

[THe Laumonier wax model of the ear (Collections of the Museums of the Anatomy Laboratory of Saints Pères, Paris)].

Authentified by the name of Laumonier (fecit), written on the zygomatic process of the temporal bone, a model in wax of the medial, middle and lateral ears, belongs to the collection of the "Musées Delmas, Orfila et Rouvière" in Paris. This model presents large dimensions with a great anatomical rightness. The entire temporal bone is represented with the upper part of the endocranial petrous bone removed in order to demonstrate the main elements of the internal ear (cochlea and semi-circular canals) and of the middle ear (malleus and incus). Also were represented the temporal muscle and tempor

New growth of axons in the cochlear nucleus of adult chinchillas after acoustic trauma.

This study determined the effect of acoustic overstimulation of the adult cochlea on axons in the cochlear nucleus. Chinchillas were exposed to an octave-band noise centered at 4 kHz at 108 dB sound pressure level for 1.75 h. One chinchilla was never exposed to the noise, and several others had one ear protected by an ear plug or prior removal of the malleus and incus. Exposure of unprotected ears caused loss of inner and outer hair cells and myelinated nerve fibers, mostly in the basal half of the cochlea. Cochlear nerve fiber degeneration, ipsilateral to the exposed ears, was traced to regio

[A short history of hearing research. II. Renaissance].

The history of basic research on the function of the hearing organ is revisited. The present, second part of the review covers the period between the renaissance of anatomical research in the 16th century and the beginning of modern hearing research at the end of the 19th century. Andreas Vesalius gave the two ossicles malleus and incus their names. His scholar Philippus Ingrassia found 1546 the third ossicle, the stapes. The cochlea was discovered 1552 by Bartholomeus Eustachius and denoted as cochlea 1561 by Gabriel Falloppio. Thomas Willis speculated 1672 that different "tones" (species aud

Ossicular abnormalities in Down's syndrome.

Down's syndrome has been associated with hearing loss and otitis media. Because of difficulty in examination, however, there have been few detailed otologic studies on this population. An understanding of the nature and frequency of ear disease in Down's syndrome is important, since it is common and occurs in more than one in every 600 live births. To define the aural manifestation of Down's syndrome, complete otologic and audiometric examination was performed on 107 consecutive patients. This included micropneumatic otoscopy, pure-tone and speech audiometry, impedance tympanometry, and, in so

Development of the human primary and secondary jaw joints.

This study investigates the development of the primary and secondary jaw joints in humans, focusing on their concomitance and subsequent disconnection. Development begins with the primary temporomandibular joint as a connection between Meckel's cartilage and the incus, while the secondary temporomandibular joint develops anteriorly as an articulation between the mandibular condyle and the mandibular fossa. Previous research in mice has provided insights into the morphogenesis of these joints, but their specific development of the 3D morphogenesis in humans remains unclear. To address this gap,

Audiological outcome after stapes surgery in relation to prosthesis type.

PURPOSE: Different techniques are used to fix crimp and CliP® Piston stapes prostheses to the long process of the incus (LPI). The CliP® Piston provides a stiff connection in contrast to the static bended loop of the crimp prosthesis, which imitates the physiological incudostapedial joint (ISJ) and thereby potentially leads to different hearing outcome. METHODS: In a retrospective single-center study of German-speaking one hundred and ninety patients who underwent stapes surgery CliP® Piston or crimp prostheses between the years of 2016 and 2019 by the same surgeon and in the same setting. Impact of Otosclerosis on Auditory Ossicle Remodeling: A Scanning Electron Microscopy Analysis of Stapes Head Overloads.

BACKGROUND Otosclerosis is a pathology that interferes with the conduction of vibrations to the inner ear, triggering changes in the auditory ossicles and their associated joints due to mechanical overload. This study primarily aims to evaluate these overload-induced modifications in the stapes head resulting from the immobilization of the base of the third auditory ossicle in otosclerosis patients. MATERIAL AND METHODS We conducted a comparative analysis of patients undergoing their first surgery for otosclerosis. The test group consisted of 31 patients who underwent stapedotomy between 2020-

X-ray fluorescence microscopy: A method of measuring ion concentrations in the ear.

This technical note describes synchrotron x-ray fluorescence microscopy (XFM) as a method for measuring the concentrations of different elements in cross-sections of the ear at extremely high resolution. This method could be of great importance for addressing many open questions in hearing research. XFM uses synchrotron radiation to evoke emissions from many biologically relevant elements in the tissue. The intensity and wavelength of the emitted radiation provide a fingerprint of the tissue composition that can be used to measure the concentration of the elements in the sampled location. Here

Pars tensa and tympanicomalleal joint: proposal for a new anatomic classification.

PURPOSE: The tympanic membrane (TM) belongs to the ear. Despite its place in the ear anatomy, can we give it also a different anatomic classification? The main objective is to clarify the nature of TM, tympanic bone and malleus to propose a new anatomic classification. METHODS: This cadaveric study was performed in two human heads and six fresh temporal bones. A study of the temporomandibular joint, external acoustic meatus (EAM), TM and middle ear structures was conducted. A medical literature review englobing anatomy, embryology, histology and phylogeny of the ear was performed and the resu
