Segmental left colectomy: a modified caudal-to-cranial approach.

BACKGROUND: We have designed a modified caudal-to-cranial approach to perform laparoscopic left colectomy preserving the inferior mesenteric artery for benign colorectal diseases. METHODS: IRB approval and informed consent have been obtained. A dissection is conducted to separate the descending mesocolon of the Gerota's plan from the medial aspect to the peritoneal lining to the left parietal gutter. The peritoneal layer is incised parallel to the vessel and close to the colonic wall. The dissection is continued anteriorly up to reach the resected parietal gutter. A passage into the mesentery

Single-access laparoscopic low anterior resection with vertical suspension of the rectum.

PURPOSE: Single-access laparoscopic surgery was first introduced for colectomy and later adapted for anterior resection. During single-access laparoscopic pelvic procedures, such as total mesorectal excision, it is often difficult to obtain an adequate operative field. By suspending the rectum vertically, we were able to execute a total mesorectal excision with single-access laparoscopy. We describe here the use of this new procedure to treat rectal cancer. METHODS: The selected 7 patients (1 male and 6 female) with stage II or III rectal cancer underwent the procedure. Single-port access to

Is Robotic Complete Mesocolic Excision Feasible for Transverse Colon Cancer?

Introduction: Laparoscopic complete mesocolic excision (CME) for transverse colon cancer is technically challenging. Robotic technology has been developed to reduce technical limitations of laparoscopy. Yet, no data are available on the role of robotic approach for CME of transverse colon cancer. The aim of this study is to evaluate the feasibility and short-term outcomes of robotic CME in this subset of colon cancer. Methods: A retrospective review of a prospectively maintained database of 29 consecutive patients undergoing robotic CME for transverse colon adenocarcinoma between December 2014

Variation and treatment of vessels in laparoscopic right hemicolectomy.

BACKGROUND: With the introduction of complete mesocolic excision (CME) and the application of laparoscopic technique, surgery for colon cancer has become more standardized and the curative effect has improved [1]. The key points in laparoscopic right hemicolectomy are high ligation of main vessels and root dissection of lymph nodes. The wide range of variations in vascular architecture and intraoperative bleeding are common causes of prolonged surgical time, wound hemorrhage, and even transfer to the opening operation. METHODS: The superior mesenteric vein (SMV) is the most important anatomic

Giant Mesenteric Cyst: A Case Series.

INTRODUCTION: Mesenteric cysts are rare benign intra-abdominal lesions that can present with a wide spectrum of clinical manifestations. They are most commonly located within the mesentery of the small intestine and are less frequently found in the mesocolon. Because their clinical presentation varies according to the size and location of the cyst, symptoms are often nonspecific, making diagnosis challenging. CASE PRESENTATION: We report two pediatric cases of mesenteric cysts. The first case involved a 10-month-old male infant who presented with abdominal distension and intermittent non-bili

Expanding the molecular spectrum of gene fusions in endometrial stromal sarcoma: Novel subunits of the chromatin remodeling complexes PRC2 and NuA4/TIP60 as alternative fusion partners.

Endometrial stromal sarcomas (ESS) are morphologically and molecularly heterogeneous. We report novel gene fusions (EPC1::EED, EPC1::EZH2, ING3::PHF1) identified by targeted RNA sequencing in five cases. The ING3::PHF1-fusion positive ESS presented in a 58-year-old female as extrauterine mesocolonic, ovarian masses, and displayed large, monomorphic ovoid-to-epithelioid cells arranged in solid sheets. The patient remained alive with disease 13 months after surgery. The three ESS with EPC1::EED occurred in the uterine corpus in patients with a median age of 58 years (range 27-62 years). One tumo

A case report of small bowel obstruction secondary to congenital peritoneal band in adult.

INTRODUCTION: Small bowel obstruction (SBO) is common in adult surgical procedures, mainly due to postoperative adhesions. Acute SBO in adults without history of abdominal surgery, trauma or clinical hernia is less common and has various etiologies. Congenital band is an extremely rare cause. PRESENTATION OF CASE: A 56-year-old man was admitted to our hospital with a two-day history of abdominal pain and bilious vomiting. He had no history of abdominal surgery or any other medical problems. A contrast-enhanced CT of the abdomen showed a distention of small bowel loops with transition point in

The origin of sensory innervation of the peritoneum in the rat.

The distribution of sensory neurons innervating the peritoneum was studied using axonal transport of fluoro-gold. The tracer was injected into parietal peritoneum, diaphragm, mesentery, mesocolon, visceral peritoneum covering the stomach, small intestine, colon, liver, spleen, kidney, urinary bladder or uterus. After ten days of survival bilateral dorsal root ganglia from C2 to S6, and the nodose ganglia were dissected. The cryostat sections of these ganglia were mounted on glass slides and observed with a fluorescence microscope. In cases where the tracer was placed on the peritoneum covering

Anatomy-guided computational framework for classifying vascular ligation and lymphadenectomy in oncologic sigmoidectomy: toward AI-supported surgical auditing.

PURPOSE: The optimal vascular ligation strategy and lymphadenectomy level in oncological sigmoidectomy remain controversial, with inconsistent definitions and a lack of standardized postoperative assessment. This study aimed to anatomically and radiologically define D2 and D3 lymphadenectomy in sigmoid colon cancer and to develop an objective multimodal protocol for postoperative classification of vascular ligation and recurrence patterns. METHODS: A three-phase multimodal anatomical study was conducted. Phase 1 involved cadaveric dissections simulating D2 lymphadenectomy and D3 dissection wi

SMARCA4-Deficient Undifferentiated Carcinoma: A Report of 2 Cases.

BACKGROUND SMARCA4-deficient undifferentiated carcinoma (SMARCA4-DUC) is a rare and aggressive malignancy caused by inactivation of the SMARCA4 (SWI/SNF-related, matrix-associated, actin-dependent regulator of chromatin subfamily A member 4) gene, a critical component of the SWI/SNF chromatin-remodeling complex. Initially identified in thoracic tumors, it is increasingly recognized in extrathoracic sites, including the gastrointestinal tract. These tumors often mimic other malignancies, such as poorly differentiated adenocarcinomas, malignant melanoma, hematolymphoid malignancies, or sarcomas,

Cranial-to-caudal approach for radical lymph node dissection along the surgical trunk in laparoscopic right hemicolectomy.

Complete mesocolic excision with central vascular ligation is considered to contribute to superior oncological outcomes after colon cancer surgery [1]. For advanced right-sided colon cancer, this surgery sometimes requires lymph node (LN) dissection along the superior mesenteric vein (SMV), with division of the middle colic vessels, or their right branches, at origin [2]. Here, we present cranially approached radical LN dissection along the surgical trunk during laparoscopic right hemicolectomy. The omental bursa is first opened wide, and the gastrocolic trunk of Henle is exposed, using the ri
