[Kleine-Levin syndrome: state of the art].

INTRODUCTION: Kleine-Levin syndrome is a rare neurological disorder (1-2 cases per million inhabitants) primarily affecting young subjects. It is characterized by relapsing-remitting episodes of hypersomnia in association with cognitive and behavioral disturbances. Case-reports, small series, meta-analysis and a recent large, prospective trio study are consistent with a homogeneous, genuine disease entity. STATE OF THE ART: Patients are mostly male (68-78%) and adolescents (81%), with mean onset at 15 years (range 4-82 years). The first episode is triggered by an infection in 72% of patients.

Interaction profile of armodafinil with medications metabolized by cytochrome P450 enzymes 1A2, 3A4 and 2C19 in healthy subjects.

BACKGROUND AND OBJECTIVE: Armodafinil, a wakefulness-promoting agent, is the pure R-enantiomer of racemic modafinil. The objective of this article is to summarize the results of three clinical drug-interaction studies assessing the potential for drug interactions of armodafinil with agents metabolized by cytochrome P450 (CYP) enzymes 1A2, 3A4 and 2C19. Study 1 evaluated the potential for armodafinil to induce the activity of CYP1A2 using oral caffeine as the probe substrate. Study 2 evaluated the potential for armodafinil to induce gastrointestinal and hepatic CYP3A4 activity using intravenous

Progress in agonist therapy for substance use disorders: Lessons learned from methadone and buprenorphine.

Substance use disorders (SUD) are serious public health problems worldwide. Although significant progress has been made in understanding the neurobiology of drug reward and the transition to addiction, effective pharmacotherapies for SUD remain limited and a majority of drug users relapse even after a period of treatment. The United States Food and Drug Administration (FDA) has approved several medications for opioid, nicotine, and alcohol use disorders, whereas none are approved for the treatment of cocaine or other psychostimulant use disorders. The medications approved by the FDA for the tr

[Stimulant and non-stimulant medication in current and future therapy for ADHD].

The current pharmacotherapy for attention-deficit hyperactivity disorder (ADHD) is mainly based on the stimulant methylphenidate and to a small extent on amphetamines which are not approved in Germany. The only approved non-stimulant so far is atomoxetin (Strattera®), a norepinephrine reuptake inhibitor. There is no approved pharmacotherapy for adults. The aim of the available medication is a reduction of impulsivity, hyperactivity, and attention deficits. Neurobiological correlates of these effects are still not fully understood, however, a functional implication of dopaminergic and noradrene

Neurophysiology of Sleep-Deprivation Part 1: Effects of Sleep-Deprivation on Event-Related Potentials (ERPs)-Systematic and Mechanistic Review.

Background: Sleep deprivation is one of the major public health and performance risk factors, with documented effects on vigilance, executive function, emotional regulation, and safety-critical behaviour. This review examines how event-related potentials (ERPs)-which provide millisecond-level resolution of cognitive processing stages-can clarify which neural processes are most affected by sleep loss, from early sensory encoding to later evaluative and control-related stages. Materials and Methods: This study was conducted as a systematic review of human studies on sleep deprivation and ERPs. E

Molecular genetic advances in sleep research and their relevance to sleep medicine.

Neurobiology has been profoundly impacted by the availability of molecular genetic techniques within the past decade, and the sleep research field is no exception to this trend. Genetic influences on normal and pathologic sleep have long been recognized, but only recently have the contributions of discrete genetic loci to the regulation of sleep timing and sleep-related physiologic phenomena begun to be demonstrated, particularly through the use of genetically engineered strains of rodents. The ability to monitor the expression of a large number of genes has also accelerated in recent years, l

Association Between Stimulant Medication Use in Attention-Deficit/Hyperactivity Disorder (ADHD) and the Increased Risk of Upper Respiratory Tract Infections: A Retrospective Study.

Introduction Attention-deficit/hyperactivity disorder (ADHD) is a common neurodevelopmental condition that can persist from childhood into adulthood. Stimulant medications such as methylphenidate and amphetamine derivatives are the mainstay of treatment, yet their potential immunomodulatory effects remain unclear. While most upper respiratory tract infections (URTIs) are benign and self-limiting, some may result in healthcare visits and lost productivity. Proposed mechanisms linking stimulant use to infection risk include sympathetic nervous system activation, hypothalamic-pituitary-adrenal ax

Fatigue in multiple sclerosis - a brief review.

Fatigue is the most common and debilitating symptom in multiple sclerosis (MS) and is believed to be distinctly different from fatigue seen in other chronic conditions. It can affect a patient's mood, sleep and have a detrimental effect on their quality of life. In the recent years much literature has emerged in an attempt to elucidate the potential causes and treatment of this common symptom. This review article aims to examine the most recent theories on the pathophysiology of fatigue in MS as well as its association with sleep and depression. We describe the pharmacological and non-pharmaco

Sleep-disordered breathing and psychiatric comorbidities in narcolepsy: A 10-year retrospective study from Saudi Arabia.

BACKGROUND: Narcolepsy is a chronic disorder of central hypersomnia that is frequently under-recognized. Diagnostic delay, comorbid sleep-disordered breathing (SDB), and psychiatric comorbidities are well described internationally but remain poorly characterized in Saudi Arabia. METHODS: We retrospectively reviewed all consecutive adult patients diagnosed with narcolepsy at the sleep disorders center of King Fahad Medical City (KFMC), a tertiary referral hospital in Riyadh, between January 2015 and April 2026. Diagnosis followed the third edition of the International Classification of Sleep D

Daytime sleepiness and myotonic dystrophy.

Myotonic dystrophy type 1 (DM1) represents the 1 chronic neuromuscular disease with the most prominent sleep disorders, including excessive daytime sleepiness (EDS), sleep apneas, periodic leg movements during sleep, and rapid eye movement sleep dysregulation. The large majority of DM1 patients complain about EDS, which may have a deleterious impact on work, domestic responsibilities, social life, and quality of life. Here, we review the extant literature and report that studies are largely supportive of the view that DM1-related EDS is primarily caused by a central dysfunction of sleep regula

Narcolepsy: clinical decision making for the primary care physician.

Narcolepsy is a serious neurological condition in which patients are overcome by persistent, excessive feelings of fatigue and drowsiness. In addition to chronic fatigue, patients with narcolepsy often succumb to intermittent, uncontrollable periods where they abruptly fall asleep during waking hours. In addition to episodic bouts of daytime sleeping, narcoleptics also exhibit cataplexy, sleep paralysis, and hypnagogic and hypnopompic hallucinations. Unfortunately, many individuals with narcolepsy remain undiagnosed and therefore, untreated, posing a risk to themselves and those around them. T

[Rest for safety: which stakes?].

In 2003 were promulgated the texts regulating rest and safety, in the USA (approved by the ACGME) and in France (January 9th, 2001 and September 14th, 2001). The institution of the "rest for safety", an eleven hours duration interruption of activity, immediately after a night-call, can be viewed as a progress in the search for safety. Several studies showed a link between excessive work hours and occurrence of medical incidents related to tiredness. However published data do not show a link between tiredness and patients endangering. The tiredness resulting from sleep deprivation and disturban

Pharmacological interventions for ADHD: a systematic review and dose-effect network meta-analysis.

BACKGROUND: Optimising the dosage of pharmacological treatments for ADHD is key to maximising their benefits, yet most clinical guidelines provide only limited information on this issue. Dose-effect relationships have not been comprehensively assessed across all ADHD medications and age groups, despite growing concerns about subtherapeutic prescribing. We aimed to estimate dose-effect curves for efficacy and tolerability of ADHD medications (stimulants and non-stimulants) across age groups.
