Management of tympanosclerosis with ossicular fixation: review and presentation of long-term results of 30 new cases.

OBJECTIVE: Surgery for tympanosclerosis has always been challenged with the risk of deterioration of hearing or the recurrence of the disease. If surgery is indicated, controversy exists regarding staging versus nonstaging and whether to perform stapedectomy or mobilize the fixed ossicles by removal of plaques. The aim of this study was to review the previous reports and to analyze the long-term surgical outcome of 30 patients who have been operated on owing to tympanosclerosis. METHODS: Nine hundred twelve cases with chronic otitis media with or without cholesteatoma that have undergone eith

Decision making in patients with natural myringostapediopexy: A study of the contralateral ear.

Naturally occurring myringostapediopexy frequently results in minimal hearing loss and is asymptomatic. Management decisions in such ears, however, often hinge on an appraisal of evolution toward cholesteatoma. The study of the contralateral ear has been used by our research team to infer the progression of chronic otitis media. This cross-sectional, comparative study describes the clinical findings of the contralateral ear in a series of patients with myringostapediopexy. This study included a historical and current sample of 46 patients divided into a pediatric (≤18 years) and an adult group

An audit of 'dead ear' after ear surgery.

INTRODUCTION: 'Dead ear' is a rare and serious complication of ear surgery. This paper presents an audit of this complication. METHOD: Over 6 years, data for all 617 middle-ear operations performed under the care of a single consultant were recorded for the International Otology Audit. All cases of dead ear were identified and assessed. RESULTS: A post-operative dead ear occurred in 6 cases (approximately 1 per cent). No cases of post-operative dead ear occurred following the 83 otosclerosis operations and the 62 children's procedures. Amongst 187 adult patients undergoing mastoid surgery fo

Facial nerve palsy as a complication of ear syringing.

INTRODUCTION: Syringing of the ear is one of the most common procedures performed for cleaning cerumen from the external auditory canal. Common complications following syringing are pain, external auditory canal trauma and otitis externa. Hearing and vestibular loss have also been reported as complications. However, we are unaware of any report of facial nerve palsy as a complication of ear syringing. Such a case is reported. CASE PRESENTATION: We describe a case of facial nerve palsy as a complication of syringing, which demonstrates the dramatic presentation of this condition and emphasises

Effects of adenoidectomy in children with symptoms of adenoidal hypertrophy.

The purpose of this study was to compare the preoperative symptoms of children who had adenoid hypertrophy with postadenoidectomy symptoms. Sixty children undergoing adenoidectomy were included in this prospective uncontrolled study at the Farabi Hospital of Karadeniz Technical University, an academic tertiary medical center. The symptoms of each child were described by their parents. Adenoidectomy with myringotomy alone or with tympanostomy tube placement was performed in all children. Two months after the operation, the children were re-evaluated for remaining or residual symptoms. Nasal obs

Transtympanic endoscopy for diagnosis of middle ear pathology.

Office-based transtympanic endoscopy through the perforation made by laser-assisted myringotomy (LAM) has been introduced for diagnosis of ossicular interruption, stapes fixation, tympanosclerosis, congenital cholesteatoma, and perilymphatic fistula. Endoscopy of the middle ear reveals minor anomalies around the incudostapedial joint and the superstructure of the stapes. This procedure can be also applied to diagnose stapes fixation and perilymphatic fistula not diagnosed presurgically, and requires exploratory tympanotomy to make a definite diagnosis. Transtympanic endoscopy using LAM is a di

Basic fibroblast growth factor combined with atelocollagen for closing chronic tympanic membrane perforations in 87 patients.

OBJECTIVE: To present the clinical results of closing chronic tympanic membrane (TM) perforations using basic fibroblast growth factor (bFGF) combined with an atelocollagen/silicone bilayer membrane patch. STUDY DESIGN: Closure of TM perforations in 87 patients was attempted using bFGF, which is thought to promote the regeneration of TM tissues by facilitating the growth of fibroblasts and collagen fibers. METHODS: Under an operating microscope, the margin of the perforation was trimmed, and a piece of an atelocollagen/silicone bilayer membrane was placed in the perforation with the silicon

Stem cells and enhanced healing of chronic tympanic membrane perforation.

CONCLUSIONS: Important information about the basic reparative process of tympanic membrane (TM) healing is shown, which can be incorporated for further clinical understanding. This provides a basis for the exploration of stem cell treatment for TM perforations and holds promise for future improvements. OBJECTIVES: This study aimed to analyse the healing of TM perforation by using stem cells and the stiffness of the membrane was tested in an acute and long-term study. MATERIALS AND METHODS: Sprague-Dawley rats were used in a model of TM perforation. The perforation was performed with a laser

Pediatric tympanoplasty: the role of adenoidectomy.

A chronic dry perforation in a child presents a dilemma with regard to timing of intervention or whether intervention is appropriate at all. Many studies have looked at elements associated with eustachian tube function in hopes of finding prognostic factors. Adenoidectomy has been shown to be very effective in the treatment of chronic and recurrent otitis media. Intuitively, it would seem to play a role in pediatric tympanoplasty; however, no study has thoroughly investigated this issue to date. To evaluate the role of adenoidectomy in pediatric tympanoplasty, we performed a retrospective revi
