Minimizing Risk While Adapting to Treatment Complications: A Case Report.

Functional diagnoses can sometimes be ambiguous. An occlusal disorder may be paired with a parafunctional condition, as in the case presented, which features a patient who experienced anterior tooth chipping and thinning of maxillary incisal edges. The case demonstrates the principles of risk-based treatment planning and discusses challenges involved in identifying and evaluating all the evidence to maximize restorative treatment plan success. In this case, functional risk during mastication was reduced using orthodontic therapy, and a nightguard was employed to combat destructive forces durin

Patients' adherence to hard acrylic interocclusal appliance treatment in general dental practice in Sweden.

The aims of the present study were to investigate patient adherence to treatment with hard acrylic interocclusal appliance in general dentistry in Sweden and to see if some general factors could predict patient adherence or non-adherence. During the period January - May 2009 a postal questionnaire was sent to all adult patients (> or = 20 years of age) that had received a hard acrylic interocclusal appliance from the public dental health service in the County of Uppsala during 2007 (n=388). The same questionnaire was also sent to all adult patients that had received a hard acrylic interocclusa

Obstructive sleep apnea and its management.

Obstructive sleep apnea (OSA) is an increasingly common disorder. It is characterized by frequent episodes of airway obstruction associated with a reduced caliber of the upper airway and is vulnerable to further narrowing and collapse. Acute and repetitive effects of apnea and hypopnea include oxygen desaturation, reduction in intrathoracic pressure, excessive daytime sleepiness, impaired executive function and central nervous system arousals. The apnea-hypopnea index and respiratory distress index help quantify the severity of the condition. The condition is associated with several clinical s

[Sleep-related breathing disorders].

BACKGROUND: Sleep-related breathing disorders (SRBD) is a term that includes several different diagnoses. This paper describes aetiology, diagnostics, treatment options and consequences of untreated disease, in children and adults. MATERIAL AND METHOD: The paper is based on own clinical experience and literature identified through a non-systematic search of PubMed and various books. RESULTS: The prevalence of SRDB in children and adults is about 10-15% and that for the most serious type, OSAS (Obstructive Sleep Apnoea Syndrome), is about 2-4%. A sleep study is required to confirm the diagnos

A study of factors for successful splint capture of anteriorly displaced temporomandibular joint disc with disc repositioning appliance.

The purpose of this study was to analyse the factors which influenced the success of disc recapture by the insertion of a disc repositioning appliance. Fifty-one joints with joint clicking that occurred at both middle to late opening and late closing (near maximum cuspation) of the mandible were splinted with a mandibular full-coverage repositioning appliance. The clinical and MR findings were compared between the joints with successful and unsuccessful splint disc capture. Thirty-two clicking joints with reducibly displaced discs (DDWR) had successful disc recapture, while six of 19 joints wi

Effects and adverse events of a dental appliance for treatment of obstructive sleep apnoea.

In a prospective study, 95 patients with mild to moderate obstructive sleep apnoea (OSA) were randomised to receive either surgical treatment, uvulopalatopharyngoplasty, (4-6 patients) or treatment with a nocturnal dental appliance for mandibular advancement (49 patients). Of the 49 dental appliance patients, 37 completed the 12-month follow-up. The aim of this study was to evaluate the effects and adverse events of dental appliance treatment from a one-year perspective. Somnography was employed to measure treatment effects before and 12 months post-treatment. At the 12-month control, somnogra

Improvement of masticatory function after orthodontic treatment. Two case reports.

In patients with temporomandibular joints dysfunction (TMD), the masticatory function is often altered in that: (1) the intercuspal position (ICP) is less frequently achieved; (2) the mean vertical displacement from ICP at the end of opening is above normal; and (3) the isometric muscle contraction pattern during closure tends to be suppressed. We describe two cases of growing patients with severe malocclusion, who showed the same functional alterations as patients with TMJ before treatment, and normalization of the masticatory function after orthodontics.

The treatment of mandibular fractures in children.

92 children with mandibular fracture were treated at our hospital between 1980 and 1989. Distribution and treatment of the fractures focus on a new classification based upon the stage of the dentition, site of the fracture and displacement of the tooth-bearing part of the mandible. The treatment rationale includes intermaxillary fixation by specially designed mini-arch bars with acrylic resin stiffening in cases with undisplaced fractures in the early stages of the dentition. With the help of this technique, invasive procedures for fracture immobilization become less common. The indication for

A 22-year follow-up of an endodontic implant.

Root fractures in the middle and apical thirds of the root are treated by repositioning and for approximately 6 weeks of immobilization while those in the cervical third are immobilized for 3 months. Even though the results are good, some root-fractured teeth are lost and replaced by dental implants or fixed partial dentures. One historic but effective treatment option for those root fractures with unfavorable crown to root ratios is an endodontic implant in middle and apical third root fractures. This method offers immediate stable fixation of a crown and its coronal root segment to the under

Clinical treatment of a ruptured temporomandibular joint disc: morphological changes at 5-year follow-up.

Osteoarthrosis is a disease that affects the temporomandibular joint (TMJ). This case report chronicles the diagnosis and treatment of a patient for whom this pathological condition was accompanied by a rupture of the articular disc. The patient presented with loud sounds in the left TMJ and an irregular mandibular occlusal plane due to condylar intrusion in the glenoid fossa on the ipsilateral side. A noninvasive treatment was selected. A 4-month follow-up revealed remission of the articular sounds, and tissue regeneration was noted. These improvements remained visible at 5-year follow-up.

Sleep bruxism: a comprehensive overview for the dental clinician interested in sleep medicine.

Sleep bruxism (SB) is a common sleep-related motor disorder characterized by tooth grinding and clenching. SB diagnosis is made on history of tooth grinding and confirmed by polysomnographic recording of electromyographic (EMG) episodes in the masseter and temporalis muscles. The typical EMG activity pattern in patients with SB is known as rhythmic masticatory muscle activity (RMMA). The authors observed that most RMMA episodes occur in association with sleep arousal and are preceded by physiologic activation of the central nervous and sympathetic cardiac systems. This article provides a compr

Long-term results of early condylar fracture correction: case report.

Condylar fractures in childhood occur frequently, often with minimal pain and discomfort and therefore the diagnosis is not made at the time of injury. Management may be surgical or non-surgical. Non-surgical therapy in children is the method of choice if the condyle can translate normally. In fact, there is an excellent chance of regeneration and continued normal development after fracture in growing patients. The purpose of this article is to describe the long-term clinical and radiological evaluation of a conservatively treated unilateral condylar fracture, a result of trauma, in a 6-year-o
