Experimental tonic hand pain modulates the corticospinal plasticity induced by a subsequent hand deafferentation.

Sensorimotor reorganization is believed to play an important role in the development and maintenance of phantom limb pain, but pain itself might modulate sensorimotor plasticity induced by deafferentation. Clinical and basic research support this idea, as pain prior to amputation increases the risk of developing post-amputation pain. The aim of this study was to examine the influence of experimental tonic cutaneous hand pain on the plasticity induced by temporary ischemic hand deafferentation. Sixteen healthy subjects participated in two experimental sessions (Pain, No Pain) in which transcran

[Phantom sensations and peripheral induced neglect following implantation of total hip prosthesis].

OBJECTIVES: O. Sacks describes in his book "A leg to stand on" the inability to move his leg for along period after rupture of the tendon of m. quadriceps. He refers the history of similar cases, especially of a woman with a hip fracture. He discusses this as a result of disruption of proprioception (peripheral induced neglect). The aim of this investigation is to answer the question how frequently this symptom may be found in patients with hip fracture and/or arthrosis perioperativly after implantation of a total prosthesis. METHODS: we examined 106 patients aged 70.35 +/- 10.01 years (36-ma

Restoration of Proprioceptive and Cutaneous Sensation Using Regenerative Peripheral Nerve Interfaces in Humans with Upper Limb Amputations.

Without meaningful and intuitive sensory feedback, even the most advanced prosthetic limbs remain insensate and impose an enormous cognitive burden during use. The regenerative peripheral nerve interface can serve as a novel bidirectional motor and sensory neuroprosthetic interface. In previous human studies, regenerative peripheral nerve interfaces demonstrated stable high-amplitude motor electromyography signals with excellent signal-to-noise ratio for prosthetic control. In addition, they can treat and prevent postamputation pain by mitigating neuroma formation. In this study, the authors i

Repetitive Magnetic Stimulation for the Management of Peripheral Neuropathic Pain: A Systematic Review.

INTRODUCTION: Repetitive magnetic stimulation (rMS) is a safe and well-tolerated intervention. Transcranial magnetic stimulation (TMS) is used for the treatment of depression and for the treatment and prevention of migraine. Over the last few years, several reports and randomised controlled studies of the use of rMS for the treatment of pain have been published. The aim of this systematic review was to identify the available literature regarding the use of rMS in the treatment of peripheral neuropathic pain. METHODS: After a systematic Medline search we identified 12 papers eligible to be inc

Influence of multiple stenoses on echo-Doppler functional diagnosis of peripheral arterial disease: a numerical and experimental study.

The objective of this paper was to evaluate the ability of the peak systolic velocity ratio (PSVR) and pressure drop (DeltaP) to detect and grade multiple stenoses in lower limb mimicking arteries. Numerical simulations and experiments in vascular phantoms allowing ultrasound duplex scanning and pressure measurements were used to investigate simple and double stenotic arterial segments. Inter-stenotic distance, severity of the distal stenosis, flow rate and flow profile (steady or pulsatile) were the tested parameters. The three-dimensional simulations considered the turbulent two-equation Wil

Development of a treatment planning system for superficial x-ray radiotherapy using Monte Carlo database.

Objective. Currently, superficial x-ray radiotherapy does not take advantage of modern treatment planning technologies. To address the problem, a treatment planning system for superficial x-ray radiotherapy has been developed using a database generated through Monte Carlo simulations. The system, called SXRTDose, can be used to optimize irradiation strategies by adjusting energy, filtration, and applicator aiming to deliver a planned dose to the target volume while minimizing radiation risk to surrounding normal tissues.Approach. TOPAS Monte Carlo code was used for establishing the dosimetric

To sleep, perchance to dream. Jean-Martin Charcot's "Interpretation of Dreams".

During his lessons, Jean-Martin Charcot (1825-1893) explored the role of dreams in neurological and psychiatric conditions. While often overshadowed by Sigmund Freud's later contributions, Charcot's observations provide valuable insights into the intersection of neurology and dream analysis. Charcot identified characteristic dream patterns linked to specific conditions: aggressive animal dreams in hysteria, recurrent traumatic nightmares in traumatic neurosis, auditory hallucinations in chronic alcoholism, and enhanced motor imagery in phantom limb syndrome. Conversely, he viewed the lack of d

Effects of Transcranial Direct Current Stimulation on pain and pain-related outcomes: an umbrella review.

BACKGROUND: This study examines Transcranial Direct Current Stimulation (tDCS), a non-invasive brain stimulation technique, as an alternative for pain management given the limitations of pharmacological treatments. METHODS: We searched PubMed, Scopus, and the Cochrane Library for systematic reviews and meta-analyses (SRMAs) on the efficacy of tDCS across pain-related conditions. Outcomes included treatment regimens, mechanisms, effects, and adverse events. Methodological quality was assessed with AMSTAR-2 and evidence levels with GRADE. RESULTS: Forty-one SRMAs were included. Positive effect

[Neuropathic pain and neuroplasticity in functional imaging studies].

Neuropathic pain syndromes are characterised by the occurrence of spontaneous ongoing and stimulus-induced pain. Stimulus-induced pain (hyperalgesia and allodynia) may result from sensitisation processes in the peripheral (primary hyperalgesia) or central (secondary hyperalgesia) nervous system. The underlying pathophysiological mechanisms at the nociceptor itself and at spinal synapses have become better understood. However, the cerebral processing of hyperalgesia and allodynia is still controversially discussed. In recent years, neuroimaging methods (functional magnetic resonance imaging, fM

Could calcitonin be a useful therapeutic agent for trigeminal neuralgia?

Trigeminal neuralgia (TN) has been recognized as one of the most common neurovascular syndromes caused by the vascular contact of the trigeminal nerve in its root entry zone (REZ) with a branch of the superior or anterior inferior cerebellar arteries, leading to a demyelinization of trigeminal sensory fibers within either the nerve root or, less commonly, the brainstem. There is a lack of certainty regarding the aetiology and pathophysiology of TN, therefore the treatment of trigeminal neuropathic pain disorders continues to be a major therapeutic challenge. The identification of novel therape

[Severe lightning pain during spinal anesthesia in a patient with diabetic neuropathy].

A 71-year-old woman with diabetic neuropathy who had undergone amputation of the right lower leg for diabetic gangrene 4 years previously, experienced severe lightning pain in both legs during spinal anesthesia. She was scheduled for skin grafting for a burn ulcer on her left foot. Her preoperative physical examination revealed hypesthesia in both legs due to diabetic neuropathy. Spinal anesthesia was performed with a combined spinal-epidural needle at the L 4-5 interspace using 2.0 ml of 0.3% hyperbaric dibucaine in the left lateral position. The region of hypesthesia was spread below Th 4. T

Prophylactic Regenerative Peripheral Nerve Interfaces in Elective Lower Limb Amputations.

Regenerative peripheral nerve interface (RPNI) is a relatively new surgical technique to manage neuromas and phantom pain after limb amputation. This study evaluates prophylactic RPNI efficacy in managing post-amputation pain and neuroma formation in amputees compared with patients in which lower limb amputation was performed without this procedure. We included 28 patients who underwent above the knee amputation (AKA) or below the knee amputation (BKA) for severe soft tissue infection from July 2019 till December 2020. All patients had insulin-dependent diabetes. The patients were divided into

Virtual Reality as a Clinical Tool for Pain Management.

PURPOSE OF REVIEW: To evaluate the use of virtual reality (VR) therapies as a clinical tool for the management of acute and chronic pain. RECENT FINDINGS: Recent articles support the hypothesis that VR therapies can effectively distract patients who suffer from chronic pain and from acute pain stimulated in trials. Clinical studies yield promising results in the application of VR therapies to a variety of acute and chronic pain conditions, including fibromyalgia, phantom limb pain, and regional specific pain from past injuries and illnesses. Current management techniques for acute and chronic

Memantine for the treatment of general neuropathic pain: a narrative review.

Neuropathic pain (NP) is difficult to treat and is associated with a decline in quality of life. NP aetiologies are numerous and a number of pathologies display neuropathic characteristics. Of the various N-methyl-d-aspartate antagonists that are alternatives to be recommended in first-line NP treatment, memantine has the safest side-effect profile and has long been approved in Alzheimer's disease. The review covers memantine studies in postherpetic neuralgia, diabetic pain, postoperative pain, complex regional pain syndrome, chronic phantom limb pain, opioid-refractory pain and fibromyalgia.
