Effects of improvements of poorly fitting dentures and new dentures on masticatory performance.

The effect of four modifications to improve the fit and maxillomandibular relationships of poorly fitting dentures and the insertion of new dentures on masticatory performance was assessed in 21 denture wearers. A 2-week adaptation period was allowed for each of the four modifications and 3 weeks and 12 weeks of adaptation for new dentures. The preferred side masticatory performances were not appreciably affected by either the modifications to improve the fit of the original dentures or the new dentures. In most instances there was a slight decline in performance. Three denture modifications c

Unconventional implant placement. V: Implant placement through impacted teeth; results from 10 cases with an 8- to 1-year follow-up.

The aim of this paper is to document additional cases to an unconventional protocol published in 2009. In 9 patients, sites rendered edentulous by the presence of an impacted maxillary canine were treated with 12 implants placed through the impacted canines. In another patient, 3 implants were inserted in the mandible to rehabilitate a failing bridge. Implants were placed encroaching upon either the root or the crown. No postoperative pain was reported. Healing was uneventful in all but one patient; the latter underwent soft tissue infection 2 weeks after implant placement and was successfully

A Three-Dimensional Finite Element Analysis of Stress Distribution in Cement-Retained and Screw-Retained Implant-Supported Restorations in Mandible Using Porcelain-Fused-to-Metal and Zirconia Crowns.

Introduction Dental implants are widely used to replace missing teeth; however, the biomechanical performance of implant-supported restorations may be influenced by the prosthesis retention type, restorative material, and direction of occlusal loading. Unfavorable stress distribution may lead to biological and mechanical complications, affecting long-term implant success. The present study aimed to evaluate and compare the stress distribution patterns in cement-retained and screw-retained implant-supported restorations fabricated with porcelain-fused-to-metal and zirconia crowns under axial an

Risk Factors of DAIR Failure and Validation of the KLIC Score: A Multicenter Study of Four Hundred Fifty-Five Patients.

Background: Debridement, antibiotic agents, and implant retention (DAIR) is a currently accepted approach for the treatment of early prosthetic joint infections (PJI). The success of a DAIR procedure has shown variable results throughout the published literature. Scoring systems such as the Kidney, Liver, Index surgery, Cemented prosthesis, and C-reactive protein value (KLIC) score for the selection of patients that are likely to benefit from DAIR have proved to be helpful in decision making. Our study aims to further validate the KLIC score using a large external multicentric cohort and to ev

[Long-term results of wall stent implantation in benign prostatic hyperplasia and high risk status].

The implantation of a Wallstent prosthesis is a minimally invasive alternative to conventional TUR-P for the treatment of infravesical obstruction owing to benign prostatic hyperplasia. We report on the long-term follow-up of 37 patients with a high-risk status for TUR-P who have been implanted since May 1990. The ASA status was: ASA I: 0, ASA II: 2, ASA III: 17 and ASA IV: 18 patients. The median follow-up is now 38.5 months. Directly after implantation, a medium increase in the maximum flow-rate from 7.5 ml/s to 16.9 ml/s was documented. Residual urine was reduced from 120 to 24.2 ml on aver

Use of Compressive Osseointegration Endoprostheses for Massive Bone Loss From Tumor and Failed Arthroplasty: A Viable Option in the Upper Extremity.

BACKGROUND: Endoprostheses using principles of compressive osseointegration have shown good survivorship in several studies involving the lower extremity; however, no series to our knowledge have documented the use of this technology in the management of massive bone loss in the upper limb. QUESTIONS/PURPOSES: (1) What proportion of upper extremity implants using compressive osseointegration fixation principles achieved durable short-term fixation, and what were the modes of failure? (2) What surgical complications resulted from reconstruction using this technique? METHODS: A multiinstitutio

Reconstruction of the maxilla following hemimaxillectomy defects with scapular tip grafts and dental implants.

BACKGROUND: Treatment of post-resective defects of the maxilla can be challenging and usually requires dental obturation or microvascular reconstruction. As compared to soft-tissue microvascular grafts, bone reconstruction can additionally allow for facial support and retention of dental implants. The aim of this study was to evaluate scapular tip grafts with respect to their applicability for maxillary reconstruction and their potential to retain dental implants for later dental rehabilitation. MATERIALS AND METHODS: In this retrospective study, 14 patients with hemimaxillectomy defects were
