Trigeminal neuropathy in vestibular schwannoma: a treatment algorithm to avoid long-term morbidity.

BACKGROUND: Trigeminal neuropathy (TGN) can occur as a presenting feature of vestibular schwannoma (VS) or as an adverse effect of radiosurgery. This study was designed to evaluate a treatment algorithm for presenting symptoms of TGN in patients with VS, and a new radiosurgery dosimetric tolerance to avoid TGN after treatment. Outcome was measured after microsurgery (MS), stereotactic radiosurgery (SRS), hypofractionated stereotactic radiotherapy (HSRT), and fractionated radiotherapy (FRT). METHODS: A prospectively held VS database was retrospectively analysed from 2011 to 2016 at a tertiary

Hypofractionated irradiation in elderly patients with breast cancer after breast conserving surgery and mastectomy : Analysis of 205 cases.

BACKGROUND: Several randomized trials and meta-analyses confirmed a wide benefit of radiotherapy (RT), both after breast conserving surgery (BCS) and mastectomy. However, many elderly women don't receive RT. Hypofractionated (HF) RT allows « simplified » and more accessible treatments with equivalent results to classic RT in three large randomized trials. However, there are few available data on HF-RT for nodal irradiation, as well as for the boost. METHODS: We evaluated patients treated for IBC by HF-RT between 2004 and 2012 in two regional cancer centres. We used an original scheme deliveri

Histologic Classifier of Radiosensitivity to Spine Stereotactic Body Radiation Therapy.

PURPOSE: Spine stereotactic body radiation therapy (SBRT) outperforms conventional radiation therapy in preventing local failure (LF). Data comparing dose-fractionation schemes on the likelihood of LF and vertebral compression fracture (VCF) are limited. METHODS AND MATERIALS: This is a retrospective cohort study of 1838 patients (2702 lesions) treated between 2014 and 2023 at a single institution with de novo spine SBRT. LF was defined as progressive disease on magnetic resonance imaging. VCF was defined as progressive or new fracture on magnetic resonance imaging without LF. Death was consi

Obstructive voiding symptoms following stereotactic body radiation therapy for prostate cancer.

BACKGROUND: Obstructive voiding symptoms (OVS) are common following prostate cancer treatment with radiation therapy. The risk of urinary retention (UR) following hypofractionated radiotherapy has yet to be fully elucidated. This study sought to evaluate OVS and UR requiring catheterization following SBRT for prostate cancer. METHODS: Patients treated with SBRT for localized prostate cancer from February 2008 to July 2011 at Georgetown University were included in this study. Treatment was delivered using the CyberKnife® with doses of 35 Gy-36.25 Gy in 5 fractions. UR was prospectively scored

Hypofractionated radiotherapy (RT) combined with dihydroartemisinin (DHA): No synergistic effect observed in a preliminary animal study.

PURPOSE: Colorectal cancer (CRC) is one of the leading causes of cancer-related death worldwide. Dihydroartemisinin (DHA) is an anti-malaria agent and recent evidence indicates a broad anti-cancer activity. Radiotherapy (RT) is the cornerstone of cancer treatment and often has synergistic effects when combined with chemotherapy or targeted therapy. We aim to investigate whether synergistic effect exists for RT combined with DHA in a preliminary animal study of CRC treatment. METHODS: Twenty-four BALB/c nude mice were subcutaneously injected with 5.0 × 106 cells of the murine CRC cell line CT2

Metastasis-directed radiotherapy (MDRT) for men with de novo, oligometastatic prostate cancer treated with long-term androgen deprivation therapy in the STAMPEDE trial (METANOVA): protocol for a multicentre, phase II randomised controlled trial in the USA.

INTRODUCTION: Prostate cancer (PCa) is the most commonly diagnosed cancer in men worldwide. A rising proportion of men present with de novo metastatic disease, a state associated with poor survival outcomes and limited curative treatment options. Within this group, a subset of patients exhibit oligometastatic disease, defined by a limited number of metastases, which may represent a distinct clinical entity with potential for long-term disease control. Emerging evidence suggests that metastasis-directed radiotherapy (MDRT), including stereotactic body radiation therapy (SBRT), may improve progr

Combination treatment with hypofractionated radiotherapy plus IL-2/anti-IL-2 complexes and its theranostic evaluation.

BACKGROUND: Immunogenic radiotherapy (RT) can act synergistically with immune checkpoint blockers (ICBs). However, alternatives are needed for non-responding patients and those with pre-existing or ICB-induced autoimmune symptoms. Combination of RT with IL-2 could be an alternative. But IL-2 has a short half-life, and, by binding to its high-affinity receptor, it strongly stimulates immunosuppressive CD4+ Tregs and seems to promote potentially life-threatening vascular leakage. IL-2/anti-IL-2 complexes (IL-2c), which bind to the low-affinity receptor, have been reported to circumvent these dis

In Situ Vaccination after Accelerated Hypofractionated Radiation and Surgery in a Mesothelioma Mouse Model.

Purpose: How best to sequence and integrate immunotherapy into standard of care is currently unknown. Clinical protocols with accelerated nonablative hypofractionated radiation followed by surgery could provide an opportunity to implement immune checkpoint blockade.Experimental Design: We therefore assessed the impact of nonablative hypofractionated radiation on the immune system in combination with surgery in a mouse mesothelioma model. Blunt surgery (R1 resection) was used to analyze the short-term effect, and radical surgery (R0 resection) was used to analyze the long-term effect of this ra

Focal boosted IMRT treatment of prostate cancer to 84 Gy in 28 fractions: preliminary clinical outcomes, toxicity, and dosimetry.

INTRODUCTION: The FLAME trial reported that focal boosting of prostate tumors up to 95 Gy in 35 fractions improves biochemical control (disease-free survival). However, this treatment (regimen) is not commonly used in the United States. We investigated a focally boosted treatment of 84 Gy in 28 fractions (EQD2-108 Gy, BED 252 Gy). METHODS: We retrospectively evaluated men with unfavorable intermediate-risk (uIR) and high-risk (HR) prostate cancer treated with focal boost intensity-modulated radiotherapy (IMRT) between 2019 and 2022. The dose levels were 84 Gy to the gross tumor volume (GTV),

Tailored treatment strategies for cancer patients during COVID-19 pandemic.

The global pandemic of respiratory disease caused by the novel human coronavirus (SARS-CoV-2) has caused indefinite global distress, uncertainty, and disturbance. This pandemic has had direct and indirect impacts for the healthcare systems across the world, but certain subgroups of patients have been particularly affected. Among these groups are patients with cancer, who as a result of their immunosuppressed status either from the disease itself or as a consequence of treatment, are at increased risk of severe COVID-19 infection and complications. The pandemic has also led to limited resources

Palliative radiotherapy indications during the COVID-19 pandemic and in future complex logistic settings: the NORMALITY model.

IntroductionThe COVID-19 pandemic has challenged healthcare systems worldwide over the last few months, and it continues to do so. Although some restrictions are being removed, it is not certain when the pandemic is going to be definitively over. Pandemics can be seen as a highly complex logistic scenario. From this perspective, some of the indications provided for palliative radiotherapy (PRT) during the COVID-19 pandemic could be maintained in the future in settings that limit the possibility of patients achieving symptom relief by radiotherapy. This paper has two aims: (1) to provide a summ
