Further evidence for the involvement of Na+ channels in the release of adrenal catecholamine: the effect of scorpion venom and grayanotoxin I.

1 The effects of venom from the scorpion, Leiurus quinquestriatus, and grayanotoxin I on catecholamine secretion were studied in the perfused adrenal glands of guinea-pig. 2 Scorpion venom (0.1 to 10 micrograms/ml) caused a dose-dependent increase in catecholamine output. The response to the venom was partially inhibited by atropine (0.5 mM) plus hexamethonium (1mM). The dose-response curve was shifted to the right in the presence of these blocking agents. 3 Grayanotoxin I (0.1 to 0.5 mM) caused a dose-dependent increase in catecholamine output which was significantly reduced by atropine (0.5

Malignant Extra-adrenal Paraganglioma of the Bladder: A Rare Clinical Entity.

Bladder paragangliomas are uncommon extra-adrenal neuroendocrine tumors that arise from chromaffin tissue within the bladder wall. Clinical presentation varies from hematuria to catecholamine-related paroxysms, and malignant transformation, although uncommon, carries significant morbidity. We report a 45-year-old woman who presented with painless hematuria. Contrast-enhanced computed tomography (CECT) revealed a 3.6×3 cm lobulated, enhancing posterior bladder wall mass. Transurethral resection of the bladder tumor (TURBT) was performed, and histopathology confirmed paraganglioma. The patient w

Pheochromocytoma: A Clinicopathologic and Molecular Study of 390 Cases From a Single Center.

Pheochromocytomas are rare neuroendocrine tumors arising from chromaffin cells in the adrenal medulla. They may occur sporadically or in the context of hereditary syndromes. All pheochromocytomas are considered to have malignant potential (defined as risk of metastasis, not local invasion). The use of grading systems with incorporated clinical and histopathologic parameters can help but not definitively predict the metastatic potential of pheochromocytomas. The recent discovery of susceptibility genes provided new insights into the pathogenesis and introduced additional approaches to estimate

Transperitoneal laparoscopic adrenalectomy for the resection of large size pheochromocytoma: Case report and literature review.

INTRODUCTION: Pheochromocytomas are rare neuroendocrine tumors that form in chromaffin cells of the neural crest during the embryological period. In the overall population, incidence rate is 0.1%; lack of early diagnosis or adequate treatment can lead to life-threatening complications. The secretion of catecholamines, such as adrenaline or norepinephrine, produces paroxysmal headache, palpitations and arterial hypertension. Secondary, hyperglycemic crises may also appear, thus contributing to misdiagnosis of diabetes mellitus or diabetic ketoacidosis. Surgery prevails as primary treatment; des

Phaeochromocytoma in Nepal--a single centre experience.

BACKGROUND: Phaeochromocytomas are rare tumors of chromaffin cells of neural crest that classically present with symptoms of catecholamine excess such as palpitations, headache and sweating. They are diagnosed by measuring plasma or urinary levels of catecholamines or their metabolites. Anatomic localization is done by computed tomographic scan or magnetic resonance imaging, or meta-iodobenzyl guanidine scan in certain cases. Adequate preoperative catecholamine blockade prevents perioperative hemodynamic instability. OBJECTIVES: To study the clinical spectrum and management of phaeochromocyto

Effects of reserpine on phenylethanolamine N-methyltransferase mRNA levels in rat adrenal gland: Role of steroids.

Reserpine treatment has been shown to cause a long-lasting decrease in phenylethanolamine N-methyltransferase mRNA levels and a simultaneous increase in tyrosine hydroxylase and neuropeptide tyrosine mRNA levels in chromaffin cells of rat adrenal medulla. In this study, in situ hybridization histochemistry was used to further investigate factors involved in the differential regulation of the catecholamine synthesizing enzymes and the coexisting peptide neuropeptide tyrosine. Pretreatment with the synthetic glucocorticoid analogue dexamethasone followed by the administration of a single dose of

Regulation of the heart of a teleost, Gadus morhua, by autonomic nerves and circulating catecholamines.

The cod heart regulation has been investigated by anatomical studies, fluorescent histochemistry, nerve stimulation and drug effects on the perfused heart and electrically paced strips. The effect on the heart of catecholamine release from the head kidney has been studied in a perfused head kidney and heart preparation. Branches of the vagi and the fused 1st and 2nd spinal nerves innervate the heart. Specifically fluorescent fibres were found in these nerve branches, in the ducts of Cuvier, all parts of the heart, bulbus arteriosus and the ventral aorta. Weakly autofluorescent ganglion cells s

Perifornical hypothalamic pathway to the adrenal gland: Role for glutamatergic transmission in the glucose counter-regulatory response.

Adrenaline is an important counter-regulatory hormone that helps restore glucose homeostasis during hypoglycaemia. However, the neurocircuitry that connects the brain glucose sensors and the adrenal sympathetic outflow to the chromaffin cells is poorly understood. We used electrical microstimulation of the perifornical hypothalamus (PeH) and the rostral ventrolateral medulla (RVLM) combined with adrenal sympathetic nerve activity (ASNA) recording to examine the relationship between the RVLM, the PeH and ASNA. In urethane-anaesthetised male Sprague-Dawley rats, intermittent single pulse electri

Ultrastructural localization of sympathetic axons in experimental rat sciatic nerve neuromas.

The ultrastructural localization of sympathetic axons was investigated in normal rat sciatic nerves and experimental sciatic nerve neuromas. The best ultrastructural localization of noradrenaline in the dense-cored vesicles of sympathetic axons was accomplished following pretreatment of rats with nialamide and 5-hydroxy dopamine, followed by fixation according to the modified chromaffin technique of Tranzer and Richards (1976). After such preparation, sympathetic axons containing 5-hydroxy dopamine-labelled dense-cored vesicles could be identified in normal sciatic nerve. Large accumulations o

Approach to the patient with metastatic pheochromocytoma and paraganglioma: advances in systemic therapy.

Pheochromocytomas and paragangliomas (PPGLs) are rare tumors of the adrenal and extra-adrenal chromaffin cells. Although most PPGLs remain localized, approximately 25% develop metastatic disease (MPPGL), leading to substantial morbidity due to tumor burden and catecholamine excess. These tumors have a heterogeneous biologic activity, with some having an aggressive course requiring intensive treatment, and others behaving indolently, not requiring treatment over many years. Because there are no sufficiently accurate predictors of future behavior, all PPGLs are considered to have the potential f

More Than Myocarditis? Cardiac Involvement in Phaeochromocytoma-A Case Report.

AIMS/BACKGROUND: Phaeochromocytoma is a rare catecholamine-secreting neuroendocrine tumour arising from chromaffin cells of the adrenal medulla or from extra-adrenal paraganglionic tissue, in which case it is termed paraganglioma. Its clinical presentations are highly variable with up to 10-20% of patients being asymptomatic or having non-specific symptoms, therefore leading to frequent misdiagnosis. The aim of this case report is to present a rare and diagnostically challenging presentation as well as highlight the need to maintain a high index of suspicion in patients with unexplained cardio

The Immune Landscape of Pheochromocytoma and Paraganglioma: Current Advances and Perspectives.

Pheochromocytomas and paragangliomas (PPGLs) are rare neuroendocrine tumors derived from neural crest cells from adrenal medullary chromaffin tissues and extra-adrenal paraganglia, respectively. Although the current treatment for PPGLs is surgery, optimal treatment options for advanced and metastatic cases have been limited. Hence, understanding the role of the immune system in PPGL tumorigenesis can provide essential knowledge for the development of better therapeutic and tumor management strategies, especially for those with advanced and metastatic PPGLs. The first part of this review outlin

Electrical Storm in a Case of Bilateral Pheochromocytomas.

BACKGROUND Pheochromocytomas are catecholamine-secreting tumors that develop within the chromaffin cells of the adrenal glands. They most commonly present with hypertension, and the classic triad of symptoms is headaches, palpitations, and diaphoresis. Electrical storm (ES) is defined as at least 3 sustained episodes of ventricular tachycardia (VT), ventricular fibrillation (VF), or appropriate shocks from an implanted cardioverter-defibrillator (ICD) within 24 h. We discuss the case of a 63-year-old man with known bilateral pheochromocytomas who presented with ES prompting multiple ICD shocks

Risk of metastatic pheochromocytoma and paraganglioma in SDHx mutation carriers: a systematic review and updated meta-analysis.

BACKGROUND: Pheochromocytoma and paraganglioma (PPGL) are tumours that arise from chromaffin cells. Some genetic mutations influence PPGL, among which, those in genes encoding subunits of succinate dehydrogenase (SDHA, SDHB, SDHC and SDHD) and assembly factor (SDHAF2) are the most relevant. However, the risk of metastasis posed by these mutations is not reported except for SDHB and SDHD mutations. This study aimed to update the metastatic risks, considering prevalence and incidence of each SDHx mutation, which were dealt formerly all together. METHODS: We searched EMBASE and MEDLINE and selec

Pheochromocytomas and secreting paragangliomas.

Catecholamine-producing tumors may arise in the adrenal medulla (pheochromocytomas) or in extraadrenal chromaffin cells (secreting paragangliomas). Their prevalence is about 0.1% in patients with hypertension and 4% in patients with a fortuitously discovered adrenal mass. An increase in the production of catecholamines causes symptoms (mainly headaches, palpitations and excess sweating) and signs (mainly hypertension, weight loss and diabetes) reflecting the effects of epinephrine and norepinephrine on alpha- and beta-adrenergic receptors. Catecholamine-producing tumors mimic paroxysmal condit

[Serotonin: a novel marker for atherosclerotic vascular disease].

Serotonin is released from nerve terminals distributed at the intestinal chromaffin cells, which are taken up into platelets. The serotonin is released at the site where platelets are activated in such situations as atherosclerotic vascular lesions. Therefore, we assumed that the serotonin level could be a suitable marker for atherosclerosis. We developed a new high-performance liquid chromatographic (HPLC) method, including a column-switching system and a post-column reaction with benzylamine, for measurement of serotonin levels in samples from patients with coronary heart disease. The vacuum

Opioid inhibition of nicotine-induced 45Ca2(+)-uptake into cultured bovine adrenal medullary cells.

The ability of a number of opioid agonists and antagonists to affect nicotine-induced 45Ca2(+)-uptake into cultured bovine adrenal medullary cells has been investigated. High (10 microM) concentrations of the opioid agonist bremazocine produced a significant inhibition of nicotine-induced 45Ca2(+)-uptake throughout the 15 min time course examined. The opioid subtype-selectivity of this inhibition was investigated; mu and delta selective agonists produced only minor effects whereas the kappa selective agonist U50-488H and the endogenous opioid peptides dynorphin(1-13) and metorphamide almost ab

Argyrophilic cells in mammary carcinoma.

Breast tumor tissues were treated by the Grimelius procedure and examined for the presence of argyrophilic cells. Carcinomas found to contain argyrophilic cells did not include classic carcinoid tumors; the group was, in fact, heterogeneous, comprising poorly differentiated ductal carcinomas, lobular carcinomas, carcinomas of uncertain origin, and colloid carcinomas. Colloid tumors were the most frequently encountered. The prominence of argyrophilic cells in colloid carcinomas raises the possibility that development into mucin-producing cells is propitious for endocrine differentiation.
