Recurrent pleural effusion is an important diagnostic clue to distinguish familial Mediterranean fever from Behçet's disease.

Cor triatriatum is a rare congenital anomaly in adults. The left atrium is devised by a fibro-muscular membrane. It is usually detected in infancy or early childhood, although some cases remain undetected until adult life. As a consequence of trans-membrane flow obstruction, the clinical features often mimic mitral stenosis. We describe an adult patient, in whom this anomaly was diagnosed in 55 year of life.

[Persistence of the right valve of the sinus venosus with partial division of the right atrium].

A case of persistent right valve of the sinus venosus (the so-called cor triatriatum dexter) is described. There were associated severe pulmonary valve stenosis and probe patent foramen ovale. The patient survived to the age of 25, without complaints. His death was due to complications of pulmonary thromboembolism. The anomaly was detected prior to death by echo-doppler-cardiogram. The postmortem findings of the heart are described in detail.

Successful conversion of fetal atrial flutter with digoxin: report of one case.

A fetus suffering from tachycardia of more than 200 beats per minute was found to have an atrial flutter with two-to-one atrioventricular conductional block by the fetal ultrasonogram at 33 weeks of gestation. The tachycardia was successfully converted to sinus rhythm with maternal digitalization. The heart rate stayed in a normal sinus rhythm after the baby was born, however, an incomplete form of cor triatriatum was noted on the echocardiogram examination.

[Two-dimensional transesophageal echocardiography: a better presentation of intra-atrial structures].

In seven patients with suspected or detected abnormal intra-atrial structures on 2-dimensional echocardiography, transoesophageal echocardiography was diagnostic of left atrial myxoma in two, left atrial thrombus in two, cor triatriatum in two and right atrial membrane in one. The transoesophageal technique extends diagnostic possibilities. The accurate demonstration of atrial abnormalities in such cases by the transoesophageal technique obviates the need of invasive pre-operative angiographic and haemodynamic procedures.

Persistent valve of systemic venous sinus: a cause of neonatal cyanosis.

Incomplete involution of valve of systemic venous sinus can present across a spectrum of anatomical lesions ranging from eustachian valve to division of right atrium (cor triatriatum dexter) with overlapping features. We present the case of a neonate presenting with cyanosis, having persistent valve of systemic venous sinus with anatomical details of the redundant tissue in right atrium suggesting an intermediate form between Chiari network and division of right atrium.

Obstructed right pulmonary venous drainage and ipsilateral lung hypoplasia.

A case of right lung hypoplasia and dyspnea, presented at our institution for cardiac evaluation, is described. Clinical evaluation and instrumental findings of normally connected but completely obstructed right pulmonary venous drainage have led us to diagnose an exceptional variant of subtotal cor triatriatum. Differential diagnosis was mainly carried out between pulmonary venous anomalies and drainage obstructions. Symptom relief was obtained with surgical excision of the atrial dividing membrane.

A patient with right and left atrial membranes: the role of transesophageal echocardiography and magnetic resonance imaging in diagnosis.

A patient who had right and left atrial membranes was examined. The diagnosis and the hemodynamic significance of both membranes were detailed by transesophageal echocardiography and were confirmed by magnetic resonance imaging. Transesophageal imaging was found to be superior to routine echocardiography in visualization of these atrial structures.

Divided left atrium with obstruction of pulmonary venous return in the setting of hypoplasia of the left heart.

We describe an infant with hypoplasia of the left heart diagnosed prenatally who, at birth, had signs of severe pulmonary venous obstruction. Echocardiography indicated normally connecting pulmonary veins, and showed a paradoxical right-to-left shunt across a patent oval foramen. Postmortem examination revealed that the obstruction was due to a divided left atrium, or cor triatriatum sinister, with an imperforate muscular diaphragm separating completely the two components of the divided atrium.

Imaging of pulmonary venous developmental anomalies.

OBJECTIVE: The purpose of this article is to review pulmonary venous embryology and to present the imaging findings of a variety of pulmonary venous developmental anomalies, including total anomalous pulmonary venous connection, partial anomalous pulmonary venous connection, pulmonary vein stenosis and hypoplasia/atresia, and cor triatriatum. CONCLUSION: There are numerous developmental pulmonary venous anomalies. Although these conditions have traditionally been evaluated with echocardiography and angiography, they can be accurately diagnosed using both MRI and MDCT.

Coexistence of three rare congenital heart defects in a single patient: a unique case with important embryologic implications.

The unique anatomy, physiology, and surgical repair of a patient with features of three rare congenital heart defects (total anomalous pulmonary venous return to the coronary sinus, cor triatriatum, and unroofed coronary sinus with persistent left superior vena cava to the left atrium) is described. Analysis of this case suggests that these three conditions are linked in that they all may result as a consequence of a defect in the same embryologic process (i.e., incorporation of the pulmonary venous confluence into the left atrium).

Pulmonary edema due to extreme left atrial compression.

Pulmonary edema is a common concomitant of valvular heart disease and ventricular dysfunction. In addition, left atrial dysfunction due to thrombus, myxoma, or cor triatriatum can produce the same clinical picture. We encountered a patient with intractable pulmonary edema secondary to obliteration of the left atrial cavity by an extrinsically compressing lung tumor. We believe extrinsic compression of the left atrium caused impaired left atrial filling, leading to pulmonary venous hypertension and pulmonary edema.

Decompressing vein and bilateral superior venae cavae in a patient with hypoplastic left heart syndrome.

The levoatrial cardinal vein (LACV), first described in 1926, acts as a decompressing vessel for pulmonary venous return in cases of severe left-sided obstruction with an intact or significantly restrictive atrial septum. The LACV and the persistent left superior vena cava (LSVC) are thought to share similar embryologic origins. To challenge this notion, we present a unique case of a neonate with hypoplastic left heart syndrome, cor triatriatum, and a decompressing LACV in the presence of bilateral superior venae cavae.

Giant eustachian valve with echocardiographic appearance of divided right atrium.

The eustachian valve is an embryologic remnant of the valve of the inferior vena cava. It may be prominent in some individuals, but the echocardiographic appearance of a divided right atrium, as the consequence of a large eustachian valve, is extremely rare. Herein we describe an unusual giant eustachian valve with an echocardiographic appearance of a septal structure dividing the right atrium into 2 separate chambers. This abnormality should be differentiated from cor triatriatum dexter, a very rare cardiac malformation for which it could be mistaken.

Multidetector CT of left atrium.

MDCT angiography has enabled visualization of left atrium in addition to coronary arteries. CT images can be utilized to demonstrate normal anatomy and pathologies. Left atrium variations and pathologies include atrial septal pouch, patent foramen ovale, atrial septal defect, atrial band, lipomatous hypertrophy, cor triatriatum, diverticulum, accessory appendages, atrial thrombus and masses. Left atrium should be carefully evaluated in patients undergoing coronary CT angiography. MDCT is becoming an alternative modality for the evaluation of left atrium.

Congenitally unguarded tricuspid valve orifice with multiple other defects in a child with refractory heart failure.

Congenitally unguarded tricuspid valve orifice, a variant of tricuspid valve dysplasia, is a rare malformation with protean manifestations. This report describes a 5-year old child with refractory heart failure who, on echocardiography, was found to have unguarded tricuspid valve orifice with severe tricuspid regurgitation, pulmonary valve dysplasia with severe pulmonary regurgitation , cor triatriatum dexter , myxomatous mitral valve , perimembranous ventricular septal defect and a diverticulum of the right ventricular outflow tract. The management options for this child are discussed.

A case report of atrial fibrillation in early adulthood: dig deeper.

BACKGROUND: Atrial fibrillation (AF) is a common cardiac arrhythmia, which is often associated with underlying risk factors and undiagnosed conditions, including congenital heart disease. Atrial septal defects (ASDs) come to mind, albeit arrhythmias usually present later in life. We present herewith a young patient with cor triatriatum sinister (CTS), with some delay in establishing the diagnosis, following new onset AF in early adulthood. CASE SUMMARY: A 31-year-old man presented with pre-syncope and coryzal symptoms and was newly diagnosed with AF in the context of an intercurrent viral ill
