BACKGROUND: Lower limb exoskeleton robots provide quantifiable training to overcome the intensity limitations of conventional rehabilitation for post-stroke gait dysfunction. Employing a 2 × 2 factorial design, this study aimed to systematically compare the therapeutic effects of four fixed intervention regimens (combined by two robot-assisted walking training speeds [high vs. routine] and two single-session durations [long vs. routine]) on lower limb walking function recovery in people with subacute stroke.
METHODS: This was a randomized, assessor-blind, parallel-group controlled trial with

PURPOSE: Brain-computer interfaces (BCIs) offer a pathway to restore ambulation in individuals with spinal cord injury (SCI). However, existing BCI systems for gait are unidirectional and lack sensory feedback. This study aimed to demonstrate that a bidirectional brain-computer interface (BDBCI) can simultaneously enable real-time brain-controlled walking and artificial leg sensation via electrical stimulation of the sensory cortex.
METHODS: Epilepsy patients undergoing bilateral interhemispheric subdural electrocorticography (ECoG) implantation were recruited for this proof-of-concept study.

The past few years have seen an exponential growth of the robot-assisted rehabilitation field and new technological developments allowing the integration of the user's intention through detection of physiological information. The inclusion of motor intention is thought to be promising for motor rehabilitation and to facilitate neuroplasticity potentially by stimulating the cortical circuitry more than, or at least differently from, non-voluntary passive motion. Yet, contrasting results are reported in the literature. We aimed here to investigate the importance of the integration of motor inten

INTRODUCTION: Traumatic brain injury (TBI) is a traumatic head injury that can lead to gait-related disabilities, and exoskeleton training may be beneficial for the recovery of this function in these patients. The objective of this study is to determine the predictions for a clinical trial, through recruitment, evaluation and intervention of gait training with Lokomat in patients after severe TBI, involving the improvement of gait speed and their social participation, intrinsically linked to their quality of life.
METHODS AND ANALYSIS: This is a prospective, single-group, feasibility study wi

The objective of this study was to propose a novel strategy for detecting upper-limb motion intentions from mechanical sensor signals using deep and heterogeneous transfer learning techniques. Three sensor types, surface electromyography (sEMG), force-sensitive resistors (FSRs), and inertial measurement units (IMUs), were combined to capture biometric signals during arm-up, hold, and arm-down movements. To distinguish motion intentions, deep learning models were constructed using the CIFAR-ResNet18 and CIFAR-MobileNetV2 architectures. The input features of the source models were sEMG, FSR, and

INTRODUCTION: Persons with non-ambulatory spinal cord injury (SCI) undergo immediate unloading of the skeleton and, as a result, have marked loss of bone mineral density below the level of lesion that is directly associated with increased risk of long-bone fractures. There is a paucity of research that has successfully implemented rehabilitation and/or exercise training interventions to mitigate bone loss after acute SCI or reverse bone loss that has already occurred in chronic SCI. This paper describes a research protocol to compare the effect of exoskeletal-assisted walking (EAW) alone versu

This study evaluated the effects of a back-support exoskeleton on the trunk and hip joint angles, lower back muscle activity and heart rate during four patient handling tasks: assisting a patient from sitting to lying, laterally repositioning the patient and turning the patient in two directions. Eight participants performed these tasks with and without the exoskeleton. Results demonstrated a significant reduction in the lower back muscle activity, but less pronounced effects for other tasks involving minimal trunk flexion. Hip flexion angles were reduced for all tasks when the exoskeleton was

BACKGROUNDS: Exoskeletons development arises with a leading role in neurorehabilitation technologies; however, very few prototypes for upper limbs have been tested, contrasted and duly certified in terms of their effectiveness in clinical environments in order to incorporate into the health system. The purpose of this pilot study was to determine if robotic therapy of Hemiplegic Shoulder Pain (HSP) could lead to functional improvement in terms of diminishing of pain, spasticity, subluxation, the increasing of tone and muscle strength, and the satisfaction degree.

Tool use leads to plastic changes in sensorimotor body representations underlying tactile perception. The neural correlates of this tool-induced plasticity in humans have not been adequately characterized. This study used ERPs to investigate the stage of sensory processing modulated by tool use. Somatosensory evoked potentials, elicited by median nerve stimulation, were recorded before and after two forms of object interaction: tool use and hand use. Compared with baseline, tool use-but not use of the hand alone-modulated the amplitude of the P100. The P100 is a mid-latency component that inde

A 64-year-old man was diagnosed with spinal and bulbar muscular atrophy (SBMA) in 2014, and began gait training with the hybrid assistive limb (HAL) in 2017. We conducted 2 courses of HAL-based gait training and temporary improvement was confirmed both before and after intervention based on evaluation of the 2-minute walking distance, walking speed, and the Timed Up and Go test. HAL-based gait training may be effective for improving and maintaining walking ability in SBMA. However, long-term observation of this patient will be performed.

Osseointegrated, percutaneous implants as the force bearer for exoprosthetics after limb amputation have been used in individual cases for clinical rehabilitation of amputees during the past years. Most experience in this field in Germany has been accumulated at the Sana Klinik in Lübeck with the so-called endo-exo prosthesis (EEP) system. The two-step implantation procedure can now be considered as reliable. Following a well-documented learning curve initial soft tissue problems concerning the cutaneous stoma can now be regarded as exceptions. The retrospective examination of the results conc

Objective.Supernumerary robotic limbs are body augmentation robotic devices by adding extra limbs or fingers to the human body different from the traditional wearable robotic devices such as prosthesis and exoskeleton. We proposed a novel motor imagery (MI)-based brain-computer interface (BCI) paradigm based on the sixth-finger which imagines controlling the extra finger movements. The goal of this work is to investigate the electromyographic (EEG) characteristics and the application potential of MI-based BCI systems based on the new imagination paradigm (the sixth finger MI).Approach.Fourteen

BACKGROUND: Robotic rehabilitation is a highly promising approach to recover lost functions after stroke or other neurological disorders. Unfortunately, robotic rehabilitation currently suffers from "motor slacking", a phenomenon in which the human motor system reduces muscle activation levels and movement excursions, ostensibly to minimize metabolic- and movement-related costs. Consequently, the patient remains passive and is not fully engaged during therapy. To overcome this limitation, we envision a new class of body-powered robots and hypothesize that motor slacking could be reduced if ind

Traumatic brachial plexus injury (BPI) results in significant disability, often hindering functionality in the patient's daily life. Post- surgery, muscle strength recovery can take up to two years, with 40% of patients requiring even longer. A powered elbow orthosis can enhance functionality during activities of daily living (ADLs). This study tested a novel powered myoelectric elbow orthosis (PMEO) during ADLs. Subjects with BPI were fitted with the PMEO and divided into two groups: more impaired (Manual Muscle Test (MMT) < 3, N = 5) and less impaired (MMT≤ 3, N = 4). They performed four ADL

The objective of this study was to determine the utility of a passive gravity-balanced arm orthosis, the Wilmington robotic exoskeleton (WREX), for patients with neuromuscular diseases. The WREX, a four-degrees-of-freedom functional orthosis, is energized by rubber bands to eliminate gravity and is attached to the wheelchair. The development and clinical testing of WREX is described in this report. Seventeen patients (14 boys and 3 girls) with muscular disabilities participated in the study. Ages ranged from 4 to 20 years. Criteria for inclusion included a weakened arm, use of a wheelchair, th

OBJECTIVE: Recently, human joint impedance-the instantaneous mechanical response to a perturbation-has been quantified during gait, providing new insight beyond the traditional biomechanical descriptions of kinetics and kinematics. However, the role of joint impedance in neuromotor control and the development of exoskeletons and other wearable robotic systems remains unknown. One approach to studying the role of impedance in neuromotor control involves characterizing the human ability to discriminate changes in external impedance properties. Thus, the purpose of this work is to quantify the mi

BACKGROUND: Robot-assisted therapy for poststroke rehabilitation of the upper limb is rapidly spreading. The need comes from the reduced number of therapists and the aim of defining a more involving therapy for the patients. Previous studies report the effectiveness of robotic therapy to provide intensive, repetitive and task-specific rehabilitation, as well as the ability to provide different modalities of training. However, it is not always clear how these modes are implemented and how they are defined, since different labels are sometimes used. As a consequence, it is difficult to define a

BACKGROUND: Advanced technologies are becoming increasingly accessible in rehabilitation. Current research suggests technology can increase therapy dosage, provide multisensory feedback, and reduce manual handling for clinicians. While more high-quality evidence regarding the effectiveness of rehabilitation technologies is needed, understanding of how to effectively integrate technology into clinical practice is also limited. Current implementation of rehabilitation technology is inconsistent, with low uptake among clinicians and frequent reports of technology abandonment. An Australian rehabi

PURPOSE: This interdisciplinary perspective aims to identify key limitations in current Human-in-the-Loop Optimization (HILO) approaches for lower-limb exoskeletons (LLEs) and to propose a shift toward more human-centred optimization.
MATERIALS AND METHODS: Drawing on literature from wearable robotics, rehabilitation science, motor learning, and clinical practice, we critically discuss three main limitations of current HILO approaches: narrow optimization scope, limited consideration of individual capabilities, and disconnection from personalized training.
RESULTS: We argue that human-center
