Cell-mediated and humoral immune responses to BCG and rubella vaccinations and to recall antigens in onchocerciasis patients.

In seven Liberian villages of different endemicity for onchocerciasis 629 persons were tested for tuberculin reactions using 5 i.u. purified protein derivative or old tuberculin. No correlation between infection with Onchocerca volvulus and reactivity to tuberculin was detected on a community level, but a direct comparison of results of infected and non-infected individuals in two neighboring villages revealed a lower responsiveness in persons with onchocerciasis. The conversion rate three months after BCG vaccination was significantly lower in children with onchocerciasis (48%) than in contro

[Artesunate and Severe Malaria: The Importance of Proper Treatment Steps and Laboratory Monitoring].

Malaria is a severe infectious disease caused by Plasmodium parasites transmitted by infected female Anopheles mosquitoes. Plasmodium falciparum is the leading cause of malaria-related deaths worldwide and can lead to severe complications such as coma, metabolic acidosis and renal failure. In this report a case of high-parasitemia P.falciparum infection diagnosed and treated with intravenous (IV) artesunate was presented. The patient was a 52-year-old male with a travel history to Liberia who was referred to a university hospital due to thrombocytopenia and hematochezia. Subsequently, the pati

Partnership for Research on Ebola VACcination (PREVAC): protocol of a randomized, double-blind, placebo-controlled phase 2 clinical trial evaluating three vaccine strategies against Ebola in healthy volunteers in four West African countries.

INTRODUCTION: The Ebola virus disease (EVD) outbreak in 2014-2016 in West Africa was the largest on record and provided an opportunity for large clinical trials and accelerated efforts to develop an effective and safe preventative vaccine. Multiple questions regarding the safety, immunogenicity, and efficacy of EVD vaccines remain unanswered. To address these gaps in the evidence base, the Partnership for Research on Ebola Vaccines (PREVAC) trial was designed. This paper describes the design, methods, and baseline results of the PREVAC trial and discusses challenges that led to different proto

[Difficulty in diagnosing pediatric tuberculosis].

Tuberculosis (TB) in children is an important warning sign in a community, as it could signal recent infection of a cavitary form in an adult. Thus, while early diagnosis is crucial for effective treatment in children, it is also imperative for the control of tuberculosis at the public health level since it allows rapid identification of contagious adult cases. Here we report four cases of difficult and delayed diagnosis of TB in children. From this experience we highlight the need for an extensive medical history of the patient during diagnostic work-up. This includes: the positive history fo

Neglected Tropical Diseases as a 'litmus test' for Universal Health Coverage? Understanding who is left behind and why in Mass Drug Administration: Lessons from four country contexts.

INTRODUCTION: Individuals and communities affected by NTDs are often the poorest and most marginalised; ensuring a gender and equity lens is centre stage will be critical for the NTD community to reach elimination goals and inform Universal Health Coverage (UHC). NTDs amenable to preventive chemotherapy have been described as a 'litmus test' for UHC due to the high mass drug administration (MDA) coverage rates needed to be effective and their model of community engagement. However, until now highly aggregated coverage data may have masked inequities in availability, accessibility and acceptabi

When context matters: Multilevel determinants of self-reported sexually transmitted infections symptoms among men engaged in transactional sex in 26 Sub-Saharan African countries.

BACKGROUND: Men engaged in transactional sex (METS) represent a neglected key population in sub-Saharan Africa (SSA), yet little is known about their burden of sexually transmitted infections (STIs) and associated factors. This study assessed the prevalence of self-reported STIs (SR-STI) and identified individual, community, and country-level determinants among this group. METHODS: We analyzed pooled recent nationally representative Demographic and Health Survey (DHS) data from 26 SSA countries. This study included 10,128 men who reported engagement in transactional sex within the past 12 mont

Effect of age and sex on the associations between potential modifiable risk factors and both type 2 diabetes and impaired fasting glycaemia among West African adults.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is becoming one of the leading causes of morbidity and mortality worldwide, including among Africans. Knowledge of the association between traditional risk factors and both diabetes and pre-diabetes, and whether these differ by age and sex, is important for designing targeted interventions. However, little is known about these associations for African populations. METHODS: The study used data from WHO STEPS surveys, comprising 15,520 participants (6,774 men and 8,746 women) aged 25-64 years, from 5 different West African countries, namely Burkina Fa

Association of community engagement with vaccination confidence and uptake: A cross-sectional survey in Sierra Leone, 2019.

BACKGROUND: The 2014-2016 Ebola epidemic disrupted childhood immunization in Sierra Leone, Liberia, and Guinea. After the epidemic, the Government of Sierra Leone prioritized community engagement to increase vaccination confidence and uptake. To support these efforts, we examined potential drivers of vaccination confidence and uptake in Sierra Leone. METHODS: We conducted a population-based household survey with primary caregivers of children in a birth cohort of 12 to 23 months in four districts with low vaccination coverage in Sierra Leone in 2019. Modified Poisson regression modeling with

The Burden of Malnutrition and Fatal COVID-19: A Global Burden of Disease Analysis.

Background: Although reasonable to assume, it is not yet clear whether malnourished countries are at higher risk for severe or fatal coronavirus disease 2019 (COVID-19). This study aims to identify the countries where prevalent malnutrition may be a driving factor for fatal disease after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Methods: Using estimates from the Global Burden of Disease 2019, country-level burden of malnutrition was quantified using four indicators: death rates for child growth failure (underweight, stunting, and/or wasting) and years lived with d

Risk maps of Lassa fever in West Africa.

BACKGROUND: Lassa fever is caused by a viral haemorrhagic arenavirus that affects two to three million people in West Africa, causing a mortality of between 5,000 and 10,000 each year. The natural reservoir of Lassa virus is the multi-mammate rat Mastomys natalensis, which lives in houses and surrounding fields. With the aim of gaining more information to control this disease, we here carry out a spatial analysis of Lassa fever data from human cases and infected rodent hosts covering the period 1965-2007. Information on contemporary environmental conditions (temperature, rainfall, vegetation)

A copula-based geo-additive approach to childhood health outcomes in five West African nations.

BACKGROUND: Anemia and malnutrition are major contributors to child morbidity and mortality in Sub-Saharan Africa, particularly in West Africa where their prevalence remains high due to multifaceted socio-economic and environmental factors. Malnutrition, as defined by WHO, includes both undernutrition and overnutrition and is linked to increased risk of non-communicable diseases. Anemia, characterized by low hemoglobin levels, impairs oxygen transport and is associated with developmental delays and increased mortality in children. Given their interrelated nature, a joint modeling approach is e
