Journal of cranio-maxillo-facial surgery : official publication of the European Association for Cranio-Maxillo-Facial Surgery2011eng
The surgical treatment and complications of patients with mandibular fractures in Amsterdam over a period of 10 years are analysed. Between January 2000 and January 2009 225 patients were surgically treated for a mandibular fracture (mean age of 32.6 (SD±14.6) years). A total of 426 fracture lines were identified. Of 213 dentate patients 29 patients were treated primarily with intermaxillary fixation (IMF). IMF combined with osteosynthesis was performed on 99 patients. Seventy-nine patients received IMF only per-operatively to make open reduction and internal fixation (ORIF) possible. Of 12 ed

A new classification of four types of TMJ involvement is proposed to help orient and direct the medical and surgical treatment of patients with rheumatoid disease and related arthropathies. Clinical symptoms, radiographic findings, facial deformity features, treatment planning, and results are discussed, with case reports that illustrate each type. Interpositional polymer laminates, a new glenoid fossa prosthesis, and a metallic condyle are introduced for use in selected cases where erosion and destruction of the condyle, meniscus, and fossa area have produced significant TMJ dysfunction and o

OBJECTIVES: Fractures of the atrophic edentulous mandible are a rare complication that can become severe after the insertion of dental implants. This in vitro study investigated the effects of different implant settings varying in number, diameter, and length. and the influence of a fixed bar.
MATERIALS AND METHODS: In biomechanical experiments on artificial mandibles, an unmodified reference group, four implant settings with two different implants, and the effect of adding a fixed bar to these settings were tested. All specimens were loaded with incisal biting forces until failure due to fra

OBJECTIVE: Improvement of stability of internal fixation of epiphyseo-metaphyseal fractures in children with an implant characterized by its small diameter, easy utilization and its versatility. It avoids the drawbacks of screw fixation particularly in respect to the growth plate.
INDICATIONS: Potentially all epiphyseo-metaphyseal fractures in children.
CONTRAINDICATIONS: None. Crossing of the growth cartilage by small pins (< 2 mm) is without consequences under the condition that no compression is applied and that correction of a faulty pin position is limited to one.
SURGICAL TECHNIQUE: I

INTRODUCTION: We report a case of total oral rehabilitation with basal implants (cortically anchored disk-design implants) on a patient who received a facial allograft 1 year earlier.
OBSERVATION: A 31-year-old patient was suffering from a plexiforme neurofibroma spread into the soft tissues of the oral cavity with huge deformations of the jaws. The operation consisted in pulling out numerous supernumerary impacted teeth, removing unnecessary soft tissues, settling six basal implants in the maxilla and seven in the mandible. The following day, two resin bridges were adjusted and cemented onto

PURPOSE: Attempts to reconstruct the severely resorbed mandible (<or=6 mm) have historically been unsuccessful and often been complicated by fracture, fistulas, and neurosensory loss. In the past, bone grafts to these severely resorbed mandibles have rapidly resorbed due to a soft tissue volume deficiency that contracts about the graft.
MATERIAL AND METHODS: A novel surgical approach using dental implants as "tent poles," along with bone grafting, is described, and the long-term follow-up of 64 cases is presented.
RESULTS: The results revealed that all 64 grafts maintained 15 mm of bone heig

PURPOSE: The inter-relationships between various implant and bone parameters were evaluated for their influence on the von Mises stress distribution within the mandible using the finite element procedure. The maximum compressive stresses in cancellous and cortical bone were compared to the published stress-strain data to determine bone fracturing status when the maximum (traumatic) loading is applied.
MATERIALS AND METHODS: Parameters considered herein include the implant diameter and length. Also considered are Young's modulus of cancellous bone and that of cortical bone, along with its thic

Full-arch implant-supported rehabilitations are widely recognized as an effective treatment option for edentulous patients. Nevertheless, clinical decision-making regarding patient selection, surgical planning, prosthetic material choice, and long-term maintenance protocols remains heterogeneous and requires structured evidence-based guidance. A modified Delphi consensus process was conducted involving 29 experts during the Italian Consensus Conference. A systematic literature review covering the period 2015-2024 was performed, and the certainty of evidence was assessed using the Grading of Re

Numerous oral and maxillofacial procedures in dentate patients begin with the fixation of occlusions. While several techniques exist to perform mandibulo-maxillary fixation, many surgeons use arch bars in common practice. In cases of severe craniofacial traumas or jaw malformations, such as temporomandibular joint ankylosis, it may be impossible to use rigid arch bars. This technical note reports on the development of a technique to produce pre-shaped rigid arch bars using 3D printing technology. We take the case of a patient who presents Le Fort 1, Le Fort 2 and Le Fort 3 fractures as well as

Objective: To explore the clinical application of home-made temporomandibular joint (TMJ) prostheses. Methods: Self-developed TMJ prostheses were applied for end stage osteoarthritis, TMJ ankylosis, condylar tumors after biomechanical examination. The stability of the prostheses toward bone was evaluated during operation. Post-operative CT scans and clinical follow-ups were taken to check changes of mouth opening, position of the prostheses, bone resorption around the screws, and heterotopic bone formation around artificial condyle. Results: There were 6 patients accepted 7 prostheses smoothly

Within the clinical context of carinomas of the upper respiratory/digestive tract (special underlying features, frequency of multiple localisations), pelvi-gingival tumours pose special problems essentially related to the proximity of the mandible. Over the past 15 years, treatment protocols at the Institut Gustave-Roussy have been progressively oriented towards a surgical solution preceded by short period of chemotherapy and followed by radiotherapy on an "as required" basis. In certain instances (bone resection with defect, especially naterior), such excision surgery results in serious funct

BACKGROUND: Factors influencing treatment outcomes in furcation defects remain to be studied. Therefore, the aim of the study was to evaluate the association between factors and clinical parameters that may influence treatment outcomes in mandibular Class II furcation defects.
METHODS: Twenty-seven systemically healthy subjects with a Class II buccal or lingual furcation defect in lower molars were treated. Clinical measurements (probing depth [PD], clinical attachment level [CAL], recession, mobility, plaque index [PI], and bleeding on probing [BOP]) and defect (vertical and horizontal defec

IMPORTANCE: Functional recovery for patients who undergo maxillomandibular reconstruction with vascularized bone free flaps (VBFFs) is potentially more attainable with computer-assisted implant rehabilitation. This prosthodontic-driven approach uses software planning and surgical templates for implant placement supporting fixed dental prostheses (FDP). Implant success with immediate load (IL) provisional and definitive FDP restorations in VBFFs is reported for the first time in a patient cohort.
OBJECTIVES: To determine implant success for FDP restorations and IL restorations. To determine fa

It has been a matter of debate as to whether dental implant therapies are suitable for patients subjected to long-term use of bisphosphonates (BPs). This report presents a case of a 76-year-old woman who developed BPs-related osteonecrosis of the jaw (BRONJ) in the left hemimandible after dental implant exposure. The implants and the necrotic crestal bone were removed, and postoperatively, a delay in tissue healing with bone exposure was noticed. The histologic analysis of the block biopsies revealed a lamellar bone tissue exhibiting necrotic areas and bacterial colonies associated with the bo

Combination syndrome (CS) is one of the most fascinating oral conditions yet is poorly understood and underappreciated in the literature and clinical practice. This article reviews the most important literature on this subject and analyzes the etiology, symptomatology, diagnosis, and current therapeutic modalities for treatment in an attempt to better understand CS. The syndrome represents an example ofa complex pathologic condition of the entire stomatognathic system with a multitude of hard-tissue, soft-tissue, and occlusal changes. These changes, initiated by a certain sequence of events, b

47 end-stage TMJ patients with high occlusal plane angulation, treated with TMJ custom-fitted total joint prostheses and simultaneous maxillo-mandibular counter-clockwise rotation were evaluated for pain and dysfunction presurgery (T1) and at the longest follow-up (T2). Patients subjectively rated their facial pain/headache, TMJ pain, jaw function, diet and disability. Objective functional changes were determined by measuring maximum interincisal opening (MIO) and laterotrusive movements. Patients were divided according to the number of previous failed TMJ surgeries: Group 1 (0-1), Group 2 (2

In a previous study (Haas et al. 1995), we reported on preliminary results of 76 Brånemark implants placed in 71 patients with single-tooth gaps. The present study examines the results of the same group over a longer period of time: 55 implants were followed for more than 5 years, 12 for a period between 4 and 5 years and 1 implant for 46 months. One patient died 3 years after implant placement. Five patients lost their implant within 24 months after insertion. The drop-out rate was 2 implants in 2 patients. The recall rate was thus 97%. Of the 5 implant failures, 2 were located in the maxilla

Temporomandibular joint (TMJ) dislocation is defined as an excessive forward movement of the condyle beyond the articular eminence with complete separation of the articular surfaces and fixation in that position. The aim of this study was to describe a modified miniplate designed for treating chronic mandibular dislocations and evaluate the results of its placement in one patient, who was followed for 18 months. The treatment of chronic mandibular dislocation using this modified miniplate was shown to be efficient in relation to the postoperative maximal mouth opening, recurrence and articular
