Deep brain stimulation for control of intractable pain in humans, present and future: a ten-year follow-up.

Deep brain stimulation with chronically implanted electrodes has provided satisfactory control of pain in patients with intractable chronic pain syndromes, which have been refractory to medication and other conventional modalities of management. In this series the authors present their experience with 48 patients who have been followed for periods ranging from 6 months to 10 years. Long-term pain control was achieved in 30 patients (63%). Both the periventricular gray and specific sensory thalamic nuclei have been used as targets. Our results indicate that there is an initial 2-year fall-off o

Potentiation of sensory responses in the anterior cingulate cortex following digit amputation in the anaesthetised rat.

The anterior cingulate cortex (ACC) is important for processing different types of information, including sensory inputs. In the present study on anaesthetised rats, we recorded in vivo sensory responses of the ACC to peripheral electrical shocks. Peripheral electrical stimulation at high intensities sufficient to activate nociceptive sensory fibres elicited EPSPs within the ACC. Digit amputation caused long-lasting potentiation of ACC responses to peripheral electrical stimulation. Evoked field EPSPs remained enhanced for at least 120 min after the amputation. Because electrical shocks were d

An increased serum level of free Apo(a) in renal patients is more striking than that of Lp(a) and is influenced by homocysteine.

PURPOSE: Musical hallucinations (MH) are a subset of complex auditory hallucinations in which individuals perceive music in the absence of an external auditory stimulus. It is a rare phenomenon, first described by Ballinger in 1846, with diverse presentations from familiar childhood melodies to a simple pitch which evolved into the harmonies Robert Schumann incorporated in his sole Violin Concerto. VIEWS: This uncommon phenomenon has diverse etiologies, including psychiatric and neurological backgrounds, which guide its classification and methods of treatment. The pathophysiological basis of

Predictive Value of Preoperative Pain Sketches in Lower Extremity Amputees Undergoing Secondary Targeted Muscle Reinnervation for Treatment of Neuropathic Pain.

BACKGROUND: Targeted muscle reinnervation (TMR) is an effective surgical treatment for neuropathic pain in amputees. Qualitative descriptions of pain, depicted by pain sketches, could enhance the understanding of symptomatic improvement after surgery. Our aim is to assess whether preoperative pain sketches, drawn by lower extremity (LE) amputees, can predict surgical outcomes after secondary TMR surgery. STUDY DESIGN: Eligible patients were LE amputees who underwent secondary TMR surgery between 2017 and 2023. Pain sketches and pain scores were prospectively collected both before and after su

Progressive ossification due to retained surgical sponge after upper leg amputation: a case report.

INTRODUCTION: Numerous cases are described of patients in whom foreign objects were found after surgery. Foreign body granuloma caused by retained surgical sponge, also called gossypiboma, mostly occur in the abdominal cavity but very seldom in limbs. CASE PRESENTATION: A 29-year-old Caucasian man presented with asymmetrical walking pattern and progressive pain in his leg, which was severely injured and amputated seven years before. A firm swelling of soft tissue with calcifications was localized in the stump. Roentgenogram and MRI showed a retained surgical sponge with calcifications. Open s

The calculation of induced currents and absorbed power in a realistic, heterogeneous model of the lower leg for applied electric fields from 60 Hz to 30 MHz.

The ankle consists mainly of bone and tendon with little muscle. Currents will tend to preferentially flow through the high-conductivity muscle and this can result in very high local values of the specific energy-absorption rate (SAR). This paper presents a finite-difference method to calculate SAR in a realistic, heterogeneous model of the leg below the knee. The structure of the leg is defined by cross-sectional slices from an anatomical atlas which are converted into a 3D model of over 14,000 cells. Four types of tissue are included in the model--muscle, cortical bone, trabecular bone and c

Does a torsion adapter improve functional mobility, pain, and fatigue in patients with transtibial amputation?

BACKGROUND: Turning gait is an integral part of daily ambulation and likely poses a greater challenge for patients with transtibial amputation compared with walking a straight pathway. A torsion adapter is a prosthetic component that can increase transverse plane compliance of the prosthesis and decrease the torque applied to the residual limb, but whether this will improve patients' mobility, pain, and fatigue remains unknown. QUESTIONS/PURPOSES: Does prescription of a torsion adapter translate to improvements in (1) functional mobility and (2) self-perceived pain and fatigue in moderately a

Does targeted nerve implantation reduce neuroma pain in amputees?

BACKGROUND: Symptomatic neuroma occurs in 13% to 32% of amputees, causing pain and limiting or preventing the use of prosthetic devices. Targeted nerve implantation (TNI) is a procedure that seeks to prevent or treat neuroma-related pain in amputees by implanting the proximal amputated nerve stump onto a surgically denervated portion of a nearby muscle at a secondary motor point so that regenerating axons might arborize into the intramuscular motor nerve branches rather than form a neuroma. However, the efficacy of this approach has not been demonstrated. QUESTIONS/PURPOSES: We asked: Does TN

Regenerative Peripheral Nerve Interface (RPNI) Surgery for Mitigation of Neuroma and Postamputation Pain.

BACKGROUND: A neuroma occurs when a regenerating transected peripheral nerve has no distal target to reinnervate. Symptomatic neuromas are a common cause of postamputation pain that can lead to substantial disability1-3. Regenerative peripheral nerve interface (RPNI) surgery may benefit patients through the use of free nonvascularized muscle grafts as physiologic targets for peripheral nerve reinnervation for mitigation of neuroma and postamputation pain. DESCRIPTION: An RPNI is constructed by implanting the distal end of a transected peripheral nerve into a free nonvascularized skeletal musc

Assessment of the potential impact of embedded radioactive fragments following the use of a crude radiological dispersal device ('dirty bomb').

This work was undertaken to understand what would happen if a high-activity radioactive fragment became embedded in an individual following the use of a crude radiological dispersal device ('dirty bomb'). Two areas were addressed: how would a high-activity fragment be viewed on modern digital x-ray imaging systems; and, what would be the impact on medical management for the patient? A set of experimental trials were undertaken using an iridium-192 source and a DRagon mobile x-ray set equipped with a Canon CXDI-50G portable flat panel digital detector plate. In addition, the potential doses to

Primary Targeted Muscle Reinnervation after Below-Knee Amputation Is Not Associated with an Increased Risk of Major or Minor Surgical Complications: A Multi-Institutional, Propensity Score-Matched Analysis.

BACKGROUND: Targeted muscle reinnervation has emerged as a technique to reduce neuroma and phantom limb pain after below-knee amputation; however, the incidence of postoperative complications remains unknown. This multi-institutional study assessed the risk of postoperative complications among patients who underwent targeted muscle reinnervation at the time of below-knee amputation. METHODS: Patients who underwent below-knee amputation with targeted muscle reinnervation were propensity score-matched 1:3 to patients who underwent below-knee amputation alone. Study outcomes included the inciden

Deep brain stimulation for chronic pain: a narrative review.

Although Deep brain stimulation (DBS) is commonly used in movement disorders, it was originally developed in the 1970s to treat medically-refractory chronic pain. There are still indications for DBS, including refractory facial neuropathic pain, central neuropathic pain, brachial plexus avulsion pain, and phantom limb pain. Brain DBS targets used to treat pain are either involved in nociception pathways, belonging to the "Pain Matrix", such as the sensitive nuclei of the thalamus, the insula or the anterior cingulate; or are involved in pain inhibitory pathways such as the periaqueductal and p

Experiences of living with through-knee or above-knee amputation: a qualitative interview study.

PURPOSE: Through-knee amputation (TKA) preserves a longer residuum and enables end-weight bearing and prosthetic self-suspension, unlike above-knee amputation (AKA). Despite this, TKA is rarely performed. The lived experience of individuals with TKA remains unexplored. PARTICIPANTS AND METHODS: This cross-sectional qualitative study used semi-structured interviews to explore the experiences of 20 community-dwelling individuals with TKA or AKA, sampled for sex and mobility from two UK limb centres. Reflexive thematic analysis was used to compare and contrast groups. RESULTS: "Hide or Pride" e

A World Update of Progress in Lower Extremity Transplantation: What's Hot and What's Not.

The field of vascularized composite allotransplantation (VCA) is the new frontier of solid organ transplantation (SOT). VCA spans life-enhancing/life-changing procedures such as upper extremity, craniofacial (including eye), laryngeal, tracheal, abdominal wall, penis, and lower extremity transplants. VCAs such as uterus transplants are life giving unlike any other SOT. Of all VCAs that have shown successful intermediate- to long-term graft survival with functional and immunologic outcomes, lower extremity VCAs have remained largely underexplored. Lower extremity transplantation (LET) can offer
