Posterior fossa epithelial cyst: case report and review of the literature.

A 49-year old woman with progressive cranial nerve signs and hemiparesis was found at MR imaging and at surgery to have a cyst at the foramen magnum. Immunohistochemistry and electron microscopy showed an epithelial cyst of endodermal origin. MR findings were of an extraaxial mass, with short T1 and T2 times. Unless immunohistochemistry and electron microscopy are used in the final diagnosis of such cysts, all posterior fossa cysts lined by a single layer of epithelium should be described simply as epithelial cysts.

Resection of dorsal midbrain cavernous malformation through Reil's (lemniscal) trigone.

BACKGROUND: Cavernous malformations of the dorsal midbrain present surgical challenges due to proximity to critical structures. CASE DESCRIPTION: We present the case of a 37-year-old female who presented with left trochlear nerve palsy, left hemibody sensory loss, and left upper and lower extremity 4/5 weakness following ventriculoperitoneal shunting at an outside hospital for treatment of a brainstem hemorrhage. Magnetic resonance imaging showed a hemorrhagic lesion, consistent with cavernous malformation, in the dorsal midbrain. Her symptomatic growing cavernous malformation and significant

Electrophysiological and pathological changes in the vastus medialis and vastus lateralis muscles after early patellar reduction and nerve growth factor injection in rabbits with patellar dislocation.

BACKGROUND: Patellar dislocation can cause a series of changes in the trochlear groove and patella. However, the influence of patellar dislocation on the medialis (VM) and vastus lateralis (VL) muscles and whether nerve growth factor (NGF) is beneficial to proprioceptive rehabilitation for patellar dislocation are unknown. The purpose of this study was to investigate the effects on VM and VL after the injection of NGF and early reduction in rabbits for patellar dislocation with electrophysiological and pathological analysis. METHODS: Sixty 2-month-old rabbits were randomly divided into four g

Auditory midbrain implant: a combined approach for vestibular schwannoma surgery and device implantation.

HYPOTHESIS: The lateral suboccipital approach is a well-established route for safe removal of vestibular schwannomas in neurofibromatosis Type 2 (NF2) patients. The goal of this study was to assess if this approach can be extended to a lateral supracerebellar infratentorial approach to enable insertion of an auditory midbrain implant (AMI) penetrating array along the tonotopic gradient of the inferior colliculus central nucleus (ICC). BACKGROUND: The AMI is a new auditory prosthesis designed for penetrating stimulation of the ICC in patients with neural deafness. The initial candidates are NF

A retrospective study of cavernous sinus syndrome in 4 dogs and 8 cats.

Cavernous sinus syndrome (CSS) is characterized by deficits in more than one of the cranial nerves (CN) that traverse the cavernous sinus at the base of the cranial vault: CN III (oculomotor), IV (trochlear), VI (abducens), and the first two branches of CN V (trigeminal). Records from 4 dogs and 8 cats with CSS diagnosed over a 14-year period were reviewed. The most common clinical signs were ophthalmoparesis or ophthalmoplegia, mydriasis with no direct or consensual pupillary light reflexes, ptosis, decreased corneal sensation, and decreased retractor oculi reflex. All cats had initial signs

The efficacy of superior rectus recession with simultaneous inferior oblique disinsertion on superior oblique palsy with superior rectus contracture.

PURPOSE: The information about superior rectus (SR) recession in cases with SR contracture coexisting with superior oblique palsy (SOP) is very limited in previous literature. The aim of this study is to evaluate the effect of SR recession, as a combined procedure with inferior oblique (IO) disinsertion, in long-standing SOP with secondary SR contracture. METHODS: The medical records of the 145 patients operated for SOP were retrospectively reviewed and 15 patients who underwent SR recession met the inclusion criteria. The mean follow-up was 50.4 months. RESULTS: The preoperative angle of de

Clinicoradiological profile and outcome of cavernous sinus syndrome with coronavirus disease-2019-associated rhino-orbito-cerebral mucormycosis.

OBJECTIVE: With coronavirus disease 2019 (COVID-19) pandemic across the world, there had been an exponential increase in rhino-orbito-cerebral mucormycosis (ROCM). Extension of infection to cavernous sinus leads to cavernous sinus syndrome (CSS). This study aims to describe incidence, clinicoradiological profile, and outcome of CSS positive along with comparative analysis of CSS negative COVID-19-associated ROCM. MATERIAL AND METHOD: This was a prospective and observational study conducted from May 1, 2021, to July 31, 2021. Subjects included ROCM with active or recovered COVID-19 (past 6 wee

Diagnostic approach to diplopia.

PURPOSE OF REVIEW: This article offers a pragmatic roadmap to the practicing neurologist on how to approach the patient with double vision. Strategies of history taking and examination techniques are reviewed, followed by a broad overview of the causes of diplopia organized by neuroanatomic localization. Diplopia may be the first symptom of serious vision or life-threatening neurologic disease, and its correct localization and diagnosis are therefore essential. The systems responsible for ocular movement and alignment in the vertical and horizontal plane include complex supranuclear circuitry,

The Superior Medullary Velum and its Contribution to Obstructive Hydrocephalus: Anatomical Characterization and Surgical Targeting Considerations in an Image-Based Case Review.

BACKGROUND: Obstructive hydrocephalus caused by a membranous or synechial occlusion at the level of the superior medullary velum (SMV) is likely under-recognized and may be mislabeled as aqueductal stenosis. We aimed to describe the anatomical pattern of these "velar obstructions" and discuss their surgical targeting implications. METHODS: We retrospectively rereviewed 94 patients previously classified as primary aqueductal stenosis (1995-2015) and identified 5 cases (5%) with an obstruction caudal to the aqueductal tectum, at the SMV. A targeted literature search retrieved 18 additional repo
