Antimicrobial agents in the prevention of travelers' diarrhea.

Each of 433 adults traveling to Guadalajara, Mexico, from the United States during summer months was enrolled in one of four clinical trials of the protective effect of antimicrobial agents against travelers' diarrhea. Only one (2%) of 57 subjects taking trimethoprim-sulfamethoxazole (160 mg/800 mg daily) experienced diarrhea during a two-week study, whereas eight (14%) of 58 subjects taking trimethoprim alone (200 mg daily) and 10 (33%) of 30 taking placebo developed illness (P less than .05 and P less than .0001, respectively). Diarrhea occurred significantly less frequently among subjects r

Total flavonoids of Potentilla discolor alleviate type 2 diabetes by regulating gut microbiota and endogenous metabolites.

BACKGROUND AND AIMS: Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease with an high incidence rate worldwide. It has recently shown a trend toward affecting younger populations. Total flavonoids from Potentilla discolor Bunge (PB) are associated with antipyretic and antidiarrheal properties. Several studies have also reported that PB exhibits anti-T2DM effects. However, the interactions between PB, T2DM, and gut microbiota, and the underlying mechanisms responsible for PB-mediated regulation of gut microbiota, have not been fully elucidated. This study performed in vitro fermentat

Pentagalloylglucose and tannic acid improve intestinal health by protecting the mucus barrier from Clostridium perfringens-induced degradation.

BACKGROUND: Clostridium perfringens degrades the intestinal mucus layer, further contributing to intestinal infections and diarrhea. Gallotannins protect the intestinal tract and alleviate diarrhea, but the mechanism involved is unknown. OBJECTIVE: This study aimed to elucidate the mechanisms by which pentagalloylglucose (PGG) and tannic acid (TA) inhibit C. perfringens, protect the intestinal mucus layer, and alleviate diarrhea. METHODS: The bactericidal activity and biofilm disruption were studied via proteomics, Raman spectroscopy, and Zeta potential. The mucus layer was analyzed through

[Primary bile acid diarrhea in a community gastroenterology practice].

When first described in 1976, primary bile acid diarrhea (BAD Type 1) was regarded as a very rare cause of chronic diarrhea. Today, the incidence is estimated as > 1 %. Availability of diagnostic tools varies widely and, in Germany, there is no generally consented recommendation for their use. BAD is still widely underdiagnosed.Since the beginning of the '90 s, we have added a therapeutic trial with cholestyramine to our diagnostic approach of chronic diarrhea. Patients with a positive test were offered a Selenium-homocholic acid taurine (SeHCAT) test for confirmation of bile-acid-diarrhea (BA

Batatasin III alleviates slow transit constipation by regulating gut microbiota and inhibiting the NLRP3-IL-1β pathway.

Slow transit constipation (STC) is a functional gastrointestinal disorder characterized by impaired intestinal motility, inflammatory responses, and gut microbiota dysbiosis. Batatasin III, a natural stilbene compound, has been demonstrated to exhibit anti-inflammatory and neuroprotective properties; however, its role in STC remains unclear. In this study, a loperamide-induced STC mouse model was established and treated with low and high doses of Batatasin III. The therapeutic effects were evaluated through general phenotypic observation, small intestinal transit rate measurement, hematoxylin-

Identify old drugs as selective bacterial β-GUS inhibitors by structural-based virtual screening and bio-evaluations.

Irinotecan (CPT-11) is a cytotoxic drug that has wide applicability and usage in cancer treatment. Despite its success, patients suffer dose-dependent diarrhea, limiting the drug's efficacy. No effective therapy is available for this unmet medical need. The bacterial β-glucuronidase (β-GUS) plays pivotal role in CPT-11-induced diarrhea (CID) via activating the non-toxic SN-38G to toxic SN-38 inside intestine. By using structural-based virtual screening, three old drugs (N-Desmethylclozapine, Aspartame, and Gemifloxacin) were firstly identified as selective bacterial β-GUS inhibitors. The IC50

Diagnosis and Management of Foodborne Illness.

The Centers for Disease Control and Prevention estimates that each year, one in six Americans will experience a foodborne illness. The most common causes in the United States are viruses, such as norovirus; bacteria, such as Salmonella, Escherichia coli, Campylobacter, and Listeria; and parasites, such as Toxoplasma gondii and Giardia. Resources are available to educate consumers on food recalls and proper handling, storage, and cooking of foods. Diagnosis and management of a foodborne illness are based on the history and physical examination. Common symptoms of foodborne illnesses include vom

Antispasmodic and anti-diarrhoeal effect of Satureja hortensis L. essential oil.

Satureja hortensis L. (Lamiaceae) is an annual herb that is used in the traditional medicine of Iran for treating stomach and intestinal disorders. The antispasmodic activity of S. hortensis essential oil (SHEO) was assessed on contractions of isolated ileum, induced by KCl and acetylcholine, and compared with the effect of atropine and dicyclomine. SHEO inhibited the response to 80 mM KCl in a concentration-dependent manner (pD(2)=1.55+/-0.09 microg/ml; this is negative log concentration of SHEO causing 50% of maximum inhibition) and attenuating the maximum inducible response of acetylcholine

Bicozamycin, a poorly absorbable antibiotic, effectively treats travelers' diarrhea.

The efficacy of bicozamycin, a poorly absorbable antibiotic, in the treatment of acute diarrhea was assessed in a prospective, double-blind study of 140 adults from the United States visiting Guadalajara, Mexico. Patients randomly received bicozamycin (500 mg orally four times daily) or placebo for 3 days. The mean duration of illness was shorter in the bicozamycin than the placebo treatment groups for patients with diarrhea due to Shigella (37 versus 96 hours; p = 0.01), toxigenic Escherichia coli (31 versus 60 hours; p = 0.003), and unknown pathogens (18 versus 41 hours; p = 0.02). Inhibition of CFTR-mediated intestinal chloride secretion by a fungus-derived arthropsolide A: Mechanism of action and anti-diarrheal efficacy.

Secretory diarrhea is one of the most common types of diarrhea with high morbidity and mortality. Previous studies showed that inhibition of cystic fibrosis transmembrane conductance regulator (CFTR) Cl- channels alleviated fluid loss in secretory diarrheas. This study aimed to identify novel CFTR inhibitors from fungal metabolites and explore its underlying mechanisms and potential utility in secretory diarrheas. Electrophysiological analyses in human intestinal epithelial (T84) cells were performed to investigate the effect and mechanism of fungal metabolites on CFTR-mediated Cl- secretion.

Traveler's diarrhea.

Three hundred million people, mostly tourists, participate in international travel each year. Development of an acute diarrheal syndrome abroad, while returning home, or shortly after arriving home is referred to as traveler's diarrhea (TD). TD is not a specific diagnosis but, rather, a clinical syndrome with multiple etiologies. In this article, clinical and epidemiological features of TD, specific etiologies and their pathogenesis, as well as current means of diagnosis, treatment, and prevention will be reviewed.

Calcined lotus leaf-derived carbon dots: enhanced hemostasis, anti-inflammatory, immunomodulatory properties for ulcerative colitis management.

Ulcerative Colitis (UC) is a chronic condition characterized by damage to the intestinal mucosal barrier, resulting in bleeding, increased oxidative stress, persistent inflammation, and immune dysregulation. Lotus Leaf (LL), recognized for its dual role as both food and medicine, has demonstrated significant antioxidant and anti-inflammatory properties. Furthermore, its calcined derivative, Lotus Leaf Charcoal (LLC), enhances its astringent, hemostatic, and antidiarrheal effects, positioning it as a promising candidate for the management of UC in both dietary and medicinal contexts. Clinical characteristics and outcomes of tyrosine kinase inhibitor-related lower GI adverse effects.

PURPOSE: A variety of tyrosine kinase inhibitors (TKIs) are currently approved for the treatment of solid tumors and hematological cancers. However, TKIs are often associated with gastrointestinal (GI) adverse effects (AEs), especially diarrhea. Therefore, in the present study, we aimed to describe the clinical features and outcomes of TKI-associated lower GI AEs. METHODS: This was a retrospective single-center cohort study of patients with cancer treated with TKIs from March 2016 to September 2020 who experienced diarrhea without other identifiable causes. Basic and GI AE-related characteris
