The Treatment of Acne Scars, a 30-Year Journey.

Over the past 30 years, the treatment of acne scars has undergone changes that have been significantly influenced by the concurrent development of new devices. The advent of fractional resurfacing lasers was a watershed moment for acne scarring therapy. The author recounts a career history of considerations of acne scarring treatments as well as the literature supporting the experiences causing changes in practice. Fractional ablative and nonablative lasers, sublative radiofrequency, picosecond lasers, microneedling with and without radiofrequency and fillers are the bulk of the treatments cov

Hyperdilute Calcium Hydroxylapatite for the Treatment of Perioral Rhytids: A Pilot Study.

BACKGROUND: The perioral region plays a crucial role in facial aesthetics and is susceptible to age-related changes, such as wrinkles and fine lines due to its dynamic nature. Type I collagen is crucial in providing structural integrity and resilience to the skin. Calcium hydroxylapatite (CaHA-CMC) is a widely used dermal filler whose particles stimulate fibroblastic responses within the skin. When diluted, CaHA-CMC has emerged as a useful treatment for collagen stimulation. OBJECTIVES: The objective of the study is to evaluate the efficacy and safety of hyperdilute CaHA-CMC at a 1:3 ratio, s

The Application of the Acellular Dermal Matrix in the Correction of the Tear Trough Deformity.

OBJECTIVE: The acellular dermal matrix (ADM) used in correcting the tear trough deformity has been reported, but there were only a few cases. The long-term effectiveness of ADM was not clear. We aim to discuss the technique and the effect of using ADM to correct the tear trough deformity through more cases. METHODS: A retrospective study was conducted from January 2012 to January 2017. Twenty-six patients who showed obvious tear trough deformity with moderate or severe orbital fat bulging and excess of lower eyelid skin were treated with ADM to improve the appearance of the midface. Follow-up

Facial arterial depth and relationship with the facial musculature layer.

BACKGROUND: Previous studies have revealed a variation in the origin and distribution patterns of the facial artery. However, the relationship between the facial artery and the facial muscles has not been well described. The purpose of this study was to determine the facial artery depth and relationship with the facial musculature layer, which represents critical information for dermal filler injection and oral and maxillofacial surgery. METHODS: Fifty-four embalmed adult faces from Korean cadavers (36 male and 18 female cadavers; mean age, 73.3 years) were used in this study. A detailed diss

Ocular Adipostructuring in Complications From Orbital Fillers: Managing the Orbital Base.

The periocular contour is one of the most complex and vulnerable anatomical regions of the face and is highly prone to complications following the injection of biomaterials such as liquid polycaprolactone (PCL), hyaluronic acid (HA), and polylactic acid (PLA). These complications include persistent hyperpigmentation, malar edema, fibrosis, sensation of pressure, structural irregularities, and lymphatic dysfunction. Periocular adipostructuring is a specialized variant of facial adipostructuring. The technique uses blunt-tipped cannulas (25G and 27G × 50 mm), precise maneuvers for controlled tis

Regenerative Therapies for Cosmetic Dermatology for Patients with Diabetes Mellitus: Skin Aging, Aesthetic Concerns, and Evidence-Based Best Practices.

Diabetes mellitus affects an estimated 589 million adults globally, and cutaneous manifestations occur in up to 70% of affected individuals during the course of the disease. The objective of this narrative review is to examine the intersection of diabetes mellitus, skin aging, cosmetic dermatologic procedures, and regenerative therapies, with an emphasis on evidence-based best practices and clinical considerations. While the impaired wound healing associated with diabetes has been extensively studied, the aesthetic implications of diabetic skin disease remain comparatively underexplored. Indiv

Understanding the experience of reconstructive treatments from the perspective of people who suffer from facial lipoatrophy: a qualitative study.

BACKGROUND: Facial lipoatrophy has been described as the most distressing and stigmatizing expression of the lipodystrophy syndrome, a syndrome that is caused by antiretroviral combination therapy. In recent years, reconstructive treatments (such as poly-l-lactic acid and polyalkylimide) have been increasingly considered for this condition. These treatments allow for facial contours and facial fullness to be restored while being minimally invasive. OBJECTIVE: The main objective of this qualitative research was to explore and describe the experience of people who suffer from facial lipoatrophy

Pulmonary Foreign Body Granulomatosis after Cosmetic Injection.

INTRODUCTION: We occasionally encounter irregular marginated masses discovered incidentally in young individuals. In most cases, further investigations are conducted to assess the presence of a primary malignancy, as these masses often raise suspicions of malignancy. However, rare exceptional cases leave us perplexed. Granulomas arising from common lung infections and those induced by foreign substances can often pose challenge in distinguishing them from lung cancer. Therefore, we aimed to present a case of multiple pulmonary granulomatosis following cosmetic procedure. CASE PRESENTATION: A

The U-Turn Tournedos (UTT) Flap: A Technical Surgical Evolution for Deep Complex Calvarial Defects.

Patients treated for complex oncological calvarial defects are at a higher risk of severe complications (38%): infection, meningitis, dehiscence, and hardware/brain exposure. The patient cohorts at our center have led to the development of the "U-Turn" technical (UTT) addition of our previously reported turnover "tournedos" myocutaneous latissimus dorsi free flap. This allows for an improved ability to fill these large, round, complex defects, maintaining the safety of our original surgical technique, while improving aesthetic outcomes.A single-institution case series of complex microsurgical

Transverse Facial Artery Perforators: Anatomical, Two- and Three-Dimensional Radiographic Study.

BACKGROUND: Increased anatomical knowledge of skin vascularization, such as the recent description of angiosome and perforasome concepts, has led to important innovations in flap surgery. In this sense, few studies have been performed on face vascularization especially for facial artery perforasomes. The aim of this study was to analyze the number, size, and localization of transverse facial artery perforators and their perfusion area. METHODS: Fourteen hemifaces of fresh adult cadavers from the Department of Anatomy of Lyon University were harvested. Transverse facial artery perforators were
