PURPOSE: This interdisciplinary perspective aims to identify key limitations in current Human-in-the-Loop Optimization (HILO) approaches for lower-limb exoskeletons (LLEs) and to propose a shift toward more human-centred optimization.
MATERIALS AND METHODS: Drawing on literature from wearable robotics, rehabilitation science, motor learning, and clinical practice, we critically discuss three main limitations of current HILO approaches: narrow optimization scope, limited consideration of individual capabilities, and disconnection from personalized training.
RESULTS: We argue that human-center

In recent years, the application of virtual reality (VR) in rehabilitation medicine has gained increasing attention. VR is generally classified into two types: non-immersive and immersive systems. Non-immersive VR allows patients to interact with virtual environments through screens while maintaining a connection with the real world. Several studies have reported that the combined application of non-immersive VR and adjunctive technologies such as transcranial direct current stimulation and hand exoskeleton devices could facilitate functional recovery of hemiparetic limbs after stroke. Brain-computer interfaces (BCIs) represent a promising technology for restoring lower limb motor functions and gait after stroke. The application of BCIs in this field is supported by a limited number of studies. The objective of the review was to systematically and critically evaluate the current evidence on the use of BCIs for lower limb function rehabilitation in stroke patients. A systematic literature search was conducted in PubMed, Scopus, and Web of Science databases. The inclusion criteria were as follows: studies involving adult patients with poststroke hemiparesis; implementation of

Brain-computer interface (BCI) systems based on spontaneous electroencephalography (EEG) hold the promise to implement human voluntary control of lower-extremity powered exoskeletons. However, current EEG-BCI paradigms do not consider the cooperation of upper and lower limbs during walking, which is inconsistent with natural human stepping patterns. To deal with this problem, this study proposed a stepping-matched human EEG-BCI paradigm that involved actions of both unilateral lower and contralateral upper limbs (also referred to as compound-limbs movement). Experiments were conducted in motor

Brain-machine interfaces (BMIs) have become increasingly popular in restoring the lost motor function in individuals with disabilities. Several research studies suggest that the CNS may employ synergies or movement primitives to reduce the complexity of control rather than controlling each DoF independently, and the synergies can be used as an optimal control mechanism by the CNS in simplifying and achieving complex movements. Our group has previously demonstrated neural decoding of synergy-based hand movements and used synergies effectively in driving hand exoskeletons. In this study, ten hea

With the development of Brain Machine Interface (BMI) systems, people with motor disabilities are able to control external devices to help them restore movement abilities. Longitudinal validation of these systems is critical not only to assess long-term performance reliability but also to investigate adaptations in electrocortical patterns due to learning to use the BMI system. In this paper, we decode the patterns of user's intended gait states (e.g., stop, walk, turn left, and turn right) from scalp electroencephalography (EEG) signals and simultaneously learn the relative importance of diff

Currently available therapies for essential tremor (ET) provide sufficient control only for less than a half of patients and many unmet needs exist. This is in part due to the empiric nature of existing treatment options and persisting uncertainties about the pathogenesis of ET. The emerging concept of ET as a possible neurodegenerative disorder, better understanding of associated biochemical changes, including alterations in the γ-aminobutyric acid (GABA)-ergic system and gap junctions, and the identification of the role of the leucine-rich repeat and immunoglobulin-like domain-containing 1 (

BACKGROUND: Advanced technologies are becoming increasingly accessible in rehabilitation. Current research suggests technology can increase therapy dosage, provide multisensory feedback, and reduce manual handling for clinicians. While more high-quality evidence regarding the effectiveness of rehabilitation technologies is needed, understanding of how to effectively integrate technology into clinical practice is also limited. Current implementation of rehabilitation technology is inconsistent, with low uptake among clinicians and frequent reports of technology abandonment. An Australian rehabi

Individuals with weak hip abductors commonly exhibit compensatory gait patterns and excessive lateral center of mass excursions, which may reduce walking efficiency and increase the risk of falling. In this study, we investigated the influence of hip abductor weakness and of ideal hip abduction assistive torque on gait patterns through an established predictive simulation framework and a full-body musculoskeletal model. Using this model and simulation framework, hip abductor muscle strength was reduced in several increments, and gait patterns were predicted and qualitatively compared to report

The use of exoskeletons in occupational settings has been proposed as a solution to reduce physical strain and improve ergonomics. This field study investigates the effect of a back-support exoskeleton on muscle activity, full-body kinematics and user acceptance among eighteen Danish masons during bricklaying. Eighteen participants completed a crossover study alternating between bricklaying with and without the exoskeleton. Using electromyography (EMG) and motion capture (Xsens), we assessed muscular activity and kinematics, while questionnaires evaluated acceptance and usability. Results show

After a neurological injury, neurorehabilitation aims to restore sensorimotor function of patients. Technological assessments can provide high-quality data on a patient's performance and support clinical decision making towards the most appropriate therapy. In this study, the ArmeoPower, a robotic exoskeleton for the upper extremities, was used to assess 12 neurological patients and 31 non-disabled participants performing various standardized single joint and frontal plane game-like exercises. From the collected data, kinematic metrics (End-Point Error, Hand-Path Ratio, reaction time, stabilit

Robotic systems, such as Lokomat® have shown promising results in people with severe motor impairments, who suffered a stroke or other neurological damage. Robotic devices have also been used by people with more challenging damages, such as Spinal Cord Injury (SCI), using feedback strategies that provide information about the brain activity in real-time. This study proposes a novel Motor Imagery (MI)-based Electroencephalogram (EEG) Visual Neurofeedback (VNFB) system for Lokomat® to teach individuals how to modulate their own μ (8-12 Hz) and β (15-20 Hz) rhythms during passive walking. BACKGROUND AND PURPOSE: People with spinal cord injury (SCI) experience secondary complications including low levels of cardiometabolic activity and associated health risks. It is unknown whether overground bionic ambulation (OBA) enhances cardiometabolic challenge during walking in those with motor-incomplete SCI, thereby providing additional therapeutic benefits.
CASE DESCRIPTIONS: One man and one woman with chronic motor-incomplete paraplegia due to SCI.

OBJECTIVE: Recently, human joint impedance-the instantaneous mechanical response to a perturbation-has been quantified during gait, providing new insight beyond the traditional biomechanical descriptions of kinetics and kinematics. However, the role of joint impedance in neuromotor control and the development of exoskeletons and other wearable robotic systems remains unknown. One approach to studying the role of impedance in neuromotor control involves characterizing the human ability to discriminate changes in external impedance properties. Thus, the purpose of this work is to quantify the mi

BACKGROUND: Robot-assisted therapy for poststroke rehabilitation of the upper limb is rapidly spreading. The need comes from the reduced number of therapists and the aim of defining a more involving therapy for the patients. Previous studies report the effectiveness of robotic therapy to provide intensive, repetitive and task-specific rehabilitation, as well as the ability to provide different modalities of training. However, it is not always clear how these modes are implemented and how they are defined, since different labels are sometimes used. As a consequence, it is difficult to define a

Decoding continuous human motion from surface electromyography (sEMG) in advance is crucial for improving the intelligence of exoskeleton robots. However, incomplete sEMG signals are prevalent on account of unstable data transmission, sensor malfunction, and electrode sheet detachment. These non-ideal factors severely compromise the accuracy of continuous motion recognition and the reliability of clinical applications. To tackle this challenge, this paper develops a multi-task parallel learning framework for continuous motion estimation with incomplete sEMG signals. Concretely, a residual netw

Prolong walking is a notable risk factor for work-related lower-limb disorders (WRLLD) in industries such as agriculture, construction, service profession, healthcare and retail works. It is one of the common causes of lower limb fatigue or muscular exhaustion leading to poor balance and fall. Exoskeleton technology is seen as a modern strategy to assist worker's in these professions to minimize or eliminate the risk of WRLLDs. Exoskeleton has potentials to benefit workers in prolong walking (amongst others) by augmenting their strength, increasing their endurance, and minimizing high muscular

Neurological injuries often cause degraded motor control. While rehabilitation efforts typically focus on movement kinematics, abnormal muscle activation patterns are often the primary source of impairment. Muscle-based therapies are likely more effective than joint-based therapy. In this paper, we examined the feasibility of biomimetic input mimicking the action of human musculotendons in altering hand muscle coordination. Twelve healthy subjects produced a submaximal isometric dorsal fingertip force, while a custom actuator provided assistance mirroring the actions of either the extrinsic ex
