Intrathoracic fluid extravasation after hip arthroscopy.

The case of intrathoracic extravasation of irrigation fluid after hip arthroscopy in a 21-year-old woman is presented. In this patient intraperitoneal and retroperitoneal fluid collection developed, as seen in other case reports documenting irrigation fluid extravasation during hip arthroscopy. The patient presented to the emergency department on the first postoperative day complaining of shortness of breath. Computed tomography of the chest, abdomen, and pelvis showed retroperitoneal fluid, extensive abdominal ascites, and bilateral pleural effusions within the chest. The fluid diminished pul

Segmentation of the proximal femur in radial MR scans using a random forest classifier and deformable model registration.

BACKGROUND: Radial 2D MRI scans of the hip are routinely used for the diagnosis of the cam type of femoroacetabular impingement (FAI) and of avascular necrosis (AVN) of the femoral head, both considered causes of hip joint osteoarthritis in young and active patients. A method for automated and accurate segmentation of the proximal femur from radial MRI scans could be very useful in both clinical routine and biomechanical studies. However, to our knowledge, no such method has been published before. PURPOSE: The aims of this study are the development of a system for the segmentation of the prox

Anterior Longitudinal Osteotomy of the Greater Trochanter in Total Hip Arthroplasty.

The extra-articular impingement of the greater trochanter against the ileum is an underrated cause of early dislocation in total hip arthroplasty. In this preliminary study, the authors assess the effectiveness of an anterior longitudinal osteotomy of the greater trochanter for preventing dislocation. A total of 115 patients underwent a total hip arthroplasty through a posterolateral approach. All patients underwent clinical and radiological follow-up at 1, 3, and 6 months. No dislocation was reported. All patients demonstrated fast recovery of range of motion and walking. No trochanter fractu

Three-dimensional morphology and bony range of movement in hip joints in patients with hip dysplasia.

To confirm whether developmental dysplasia of the hip has a risk of hip impingement, we analysed maximum ranges of movement to the point of bony impingement, and impingement location using three-dimensional (3D) surface models of the pelvis and femur in combination with 3D morphology of the hip joint using computer-assisted methods. Results of computed tomography were examined for 52 hip joints with DDH and 73 normal healthy hip joints. DDH shows larger maximum extension (p = 0.001) and internal rotation at 90° flexion (p < 0.001). Similar maximum flexion (p = 0.835) and external rotation (p =

Current concepts in management of slipped capital femoral epiphysis.

Slipped capital femoral epiphysis (SCFE) is a common hip condition that can be disabling. In this review, we provide an orientation on current trends in the clinical management of SCFE including conventional procedures and specialised surgical developments. Different methods of fixation of the epiphysis, risks of complications, and the rationale of addressing deformity, primarily or secondarily, are presented. Although improved understanding of the anatomy, vascularity and implications of residual deformity have changed management strategies, the best modality of treatment that would restore c

Epidemiology, identification, treatment and return to play of musculoskeletal-based ice hockey injuries.

UNLABELLED: Ice hockey is a high contact sport where players are inherently at an increased risk for traumatic and time-loss injury. With its increasing popularity and high incidence of injury, further research is necessary to understand the risks and injuries associated with the sport and to develop performance-based outcome measures to guide return to play. This review, tailored to the practicing sports medicine team physician, focuses on the stepwise identification, treatment, time loss, return to play and subsequent risk of injury for the most common areas of injury: the head, shoulder, hi

Clinical and radiographic predictors of intra-articular hip disease in arthroscopy.

BACKGROUND: The arthroscopic treatment of intra-articular hip disease and associated structural abnormalities continues to evolve. Nevertheless, contemporary diagnostic tools have significant limitations in predicting severity of disease preoperatively. HYPOTHESIS: Clinical characteristics and radiographic parameters correlate with and predict intra-articular disease patterns in patients undergoing hip arthroscopy. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: In sum, 355 hips in 338 patients undergoing hip arthroscopy by a single surgeon were retrospectively reviewed. Clinical

The John Charnley Award: Redefining the Natural History of Osteoarthritis in Patients With Hip Dysplasia and Impingement.

BACKGROUND: Structural hip deformities including developmental dysplasia of the hip (DDH) and femoroacetabular impingement (FAI) are thought to predispose patients to degenerative joint changes. However, the natural history of these malformations is not clearly delineated. QUESTIONS/PURPOSES: (1) Among patients undergoing unilateral THA who have a contralateral hip without any radiographic evidence of hip disease, what is the natural history and progression of osteoarthritis in the native hip based on morphological characteristics? (2) Among patients undergoing unilateral THA who have a contr

The 2015 Frank Stinchfield Award: Radiographic Abnormalities Common in Senior Athletes With Well-functioning Hips but Not Associated With Osteoarthritis.

BACKGROUND: It is not known whether morphological abnormalities of the hip are compatible with lifelong hip function and avoidance of osteoarthritis (OA). Our purpose was to investigate the prevalence of radiographic findings consistent with femoroacetabular impingement (FAI) and dysplasia (DDH) in senior athletes with well-functioning hips. QUESTIONS/PURPOSES: (1) What is the prevalence of FAI and DDH in senior athletes with well-functioning hips? (2) Are radiographic findings of FAI and DDH associated with OA? (3) Is a history of longer duration or more intense activity associated with hip

Evidence and Approach for Management of Labral Deficiency: The Role for Labral Reconstruction.

The acetabular labrum is a complex fibrocartilaginous structure with unique anatomy. Its unique anatomy imparts biomechanical functions important in hip stability, synovial fluid dynamics, contact pressure dissipation, proprioception, and nociception. Although the importance of labral preservation through surgical repair techniques has been increasingly recognized, situations do occur where the labrum is either absent, deficient, or simply irreparable. In these situations, both open and arthroscopic reconstructive techniques have been described to help restore the function of the native labral

Arthroscopic Management of Dysplastic Hip Deformities: Predictors of Success and Failures With Comparison to an Arthroscopic FAI Cohort.

BACKGROUND: Reports regarding arthroscopy for mild hip dysplasia have conflicting results. HYPOTHESIS: Arthroscopy for borderline/mild hip dysplasia would lead to improved outcomes but be inferior to arthroscopy for femoroacetabular impingement (FAI). STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: A total of 88 hips (77 patients, 71% female; mean age, 33.9 years) with dysplastic radiographic findings were retrospectively reviewed at a mean follow-up of 26.0 months after hip arthroscopy. Specific procedures included labral repair (76%), labral debridement (23%), capsular repair/p

Is the acetabulum retroverted in slipped capital femoral epiphysis?

BACKGROUND: Recent biplanar radiographic studies have demonstrated acetabular retroversion and increased superolateral femoral head coverage in hips with slipped capital femoral epiphysis (SCFE), seemingly divergent from earlier CT-based studies suggesting normal acetabular version. QUESTION/PURPOSES: We therefore asked: Are there differences in (1) acetabular version at the superior ¼ of the acetabular dome (AV(sup)), (2) acetabular version at the center of the femoral head (AV(cen)), and (3) superolateral femoral head coverage (lateral center-edge angle [LCEA]) among affected SCFE hips, una

Radiographic Prevalence of Symphysis Pubis Abnormalities and Clinical Outcomes in Patients With Femoroacetabular Impingement Syndrome.

BACKGROUND: The decreased hip range of motion seen in femoroacetabular impingement syndrome (FAIS) may lead to compensatory increased motion at the symphysis pubis (SP) with resultant increased stress on the joint, which can subsequently lead to osteitis pubis. PURPOSE: To quantify the prevalence of SP abnormalities in patients with FAIS through the use of imaging modalities and to compare outcomes based on the presence of SP abnormalities. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Radiographs and magnetic resonance imaging (MRI) scans of 1009 consecutive patients who under
