Interocclusal appliances--indications and clinical routines in general dental practice in Sweden.

The aim of this study was to analyse what kind of interocclusal appliances that are used in general dental practice in Sweden, the indications for such treatment, to what extent a clinical status is recorded before treatment, as well as the routines for evaluation of treatment results. The study was a retrospective analysis of patient records from 320 adult patients who had been treated by general dental practitioners in the Public Dental Service in Sweden. For comparison a group of 88 consecutive patients who had received interocclusal appliance therapy at a specialist clinic was used. The mo

A prospective study of the short-term treatment effects of the acrylic-splint rapid maxillary expander combined with the lower Schwarz appliance.

This prospective clinical study evaluated the short-term treatment effects of acrylic-splint rapid maxillary expander in conjunction with lower Schwarz appliance (RME-Sz) therapy to the acrylic-splint rapid maxillary expansion alone (RME-only group). Pretreatment and posttreatment lateral cephalograms were analyzed for 25 RME patients and 19 RME-Sz patients. The average time between films ranged between nine and 12 months. Statistical comparisons of the treatment changes in the RME-only and RME-Sz groups were performed by means of independent sample t-tests (P < .05). The largest difference be

The oral implications of caustic soda ingestion in children.

The morbidity related to caustic soda (sodium hydroxide) ingestion is well described in the literature. The majority of publications have concentrated on the effects to the trachea and gastrointestinal tract, with little reference to the oral and peri-oral areas. Accidental ingestion of sodium hydroxide-containing substances is fortunately rare; however the consequences can be devastating. Three cases of children who drank caustic substances are described. Treatment included fitting splints, injecting steroids, local surgical procedures, and the use of dynamic appliances to maintain mouth open

Cephalometric evaluation of craniofacial and upper airway structures in Japanese patients with obstructive sleep apnea.

In order to investigate the morphological characteristics of Japanese patients with obstructive sleep apnea (OSA) and basis for selection of an oral appliance (OA) therapy, the craniofacial skeleton, soft tissue and upper airway were analyzed on lateral cephalograms from 103 patients with OSA and 98 control subjects. At first, the 103 OSA patients were divided into two groups according to their body mass index (BMI). The results were the following: Micrognathia was a major risk factor of Japanese OSA patients instead of obesity; the narrowing in the hypopharynx was observed due to the inferior

Effect of a prosthetic appliance for treatment of sleep apnea syndrome on masticatory and tongue muscle activity.

STATEMENT OF PROBLEM: The efficiency of an appliance for treatment of sleep apnea shows inordinate interindividual difference. The mechanism of its therapeutic effects remains unresolved. PURPOSE: This study examined the effect of the device on sleep apnea, and masticatory and tongue muscles. MATERIAL AND METHODS: Fifteen patients with sleep apnea syndrome were evaluated polysomnographically, with and without the appliance. Electromyograms (EMG) of genioglossal, masseter, and lateral pterygoid muscles were recorded and EMG amplitudes measured before, during, and after the apneas. Apneas were

Management of orofacial manifestations of Parkinson's disease with splint therapy: a case report.

Patients with Parkinson's Disease display a number of orofacial manifestations. These manifestations are secondary to motor and sensory deficits, resulting in bradykinesia, muscle rigidity and tremor, and difficulties with speech, swallowing, proprioception, tactile sensitivity, and hard- and soft-tissue trauma. This case describes the use of a bruxism splint which benefited a PD patient by diminishing the occurrence of orofacial pain secondary to muscle tremor and rigidity while reducing the cumulative damage to the oral structures common with the disease. The impact of therapy on sleep, spee

Digital Workflow Integration in Acute Panfacial Fracture Management.

BACKGROUND: Panfacial fractures are among the most technically demanding injuries in craniomaxillofacial trauma surgery. Digital workflows - including virtual surgical planning (VSP), three-dimensional printing, and intraoperative navigation - have demonstrated benefit in elective and delayed surgical settings, yet their integration into acute panfacial fracture management remains poorly characterized. PURPOSE: This systematic review evaluated the evidence for digital workflow integration in acute primary panfacial fracture management, characterizing achievable outcomes, assessing implementat

[Neurologist, otolaryngologist...? Which specialist should treat facial pain?].

Pain is a major public health problem. The management of orofacial pain may be a difficult challenge to the medical and dental professions. Ideally, severe cases of this type of pain should be treated by a team drawn from several disciplines such as neurology, otolaryngology, dentistry and psychiatry. Trigeminal neuralgia patients develop brief, very severe unilateral pain, usually radiating from the upper or lower jaw toward the ear, and confined to the distribution of the trigeminal nerve. The pain may be triggered by chewing, shaving or exposure to cold wind. Most patients respond to carbam

Prediction of non-responders to oral appliance treatment of obstructive sleep apnea: a pilot study.

PURPOSE: Several clinically available variables have been identified as predictors of non-response to oral appliance (OA) treatment, including endotypical traits such as severe upper airway collapsibility, unstable ventilatory control, and low arousal threshold. This study aimed to identify potential predictors of non-response to OA treatment in patients with OSA non-adherent to treatment with positive airway pressure. METHODS: Patients in this study were initially treated with OAs with and without elastic bands in a crossover design. Subsequently, each patient selected their preferred treatm

Professional musicians with craniomandibular dysfunctions treated with oral splints.

Craniomandibular dysfunction (CMD) symptoms occur frequently in violin/viola and wind players and can be associated with pain in the neck, shoulders and arm. In the current study, the effect of oral splint treatment of CMD on reducing pain and symptoms especially in these areas was investigated. Thirty (30) musicians undergoing CMD treatment with oral splints participated in this study. They completed a questionnaire that addressed CMD symptoms, localization of pain, and subjective changes in symptoms. Pain in the shoulder and/or upper extremity was the most frequent symptom reported by 83% of

Oral appliances in the management of temporomandibular disorders.

Various types of oral appliances (OAs) have been used for over half a century to treat temporomandibular disorders (TMDs), but there has been considerable debate about how OAs should be designed, how they should be used, and what they actually do therapeutically. However, there is enough information in the scientific literature at this time to reach some evidence-based conclusions about these issues. The main focus of this review is on the materials and designs of various OAs in terms of their proposed mechanisms of action and their claimed clinical objectives. Based on current scientific evid

Introduction of a new removable adjustable intraoral maxillary distraction system for correction of maxillary hypoplasia.

Distraction osteogenesis has become a treatment alternative to treat severe craniofacial skeletal dysplasias. A rigid external distraction device has been successfully used to advance the maxilla as well as the maxillary, orbital, and forehead complex (monobloc) in children as young as 2 years, adolescents, and adults. For this severe group of patients, the technique has been found to be simpler and safer than traditional surgical methods. Maxillary and midfacial advancement through distraction has been found to be extremely stable in the patients in whom the technique was used. The authors int

Cranial base considerations between apnoeics and non-apnoeic snorers, and associated effects of long-term mandibular advancement on condylar and natural head position.

One hundred consecutively medically referred patients (58 apnoeic and 42 asymptomatic snorers) were reviewed cephalometrically at six-monthly intervals (6-30 months) following treatment for obstructive sleep apnoea (OSA) and/or habitual snoring by mandibular advancement. Eighty-seven males and 13 females (mean age 49 years, SD 8.5, range 33-74) were included in this study. Reference points and planes in the cranial base, nasopharynx, and mandibular condyle were digitized with a Reflex Metrograph and their means converted to linear and angular measurements. Effects of a mandibular repositioner on obstructive sleep apnea.

The purpose of this study was to investigate the effects of a mandibular repositioner on airway, sleep, and respiratory variables in patients with obstructive sleep apnea. Twenty-two patients selected for this study were confirmed with a diagnosis of obstructive sleep apnea based on initial nocturnal polysomnography. The patients were fitted with a mandibular repositioner designed to hold the mandible anteroinferiorly. Six months later, an outcome polysomnographic study was undertaken for each patient with the appliance in place. Lateral cephalometric radiographs in the upright position were a

Effect of two types of mandibular advancement splints on snoring and obstructive sleep apnoea.

Snoring and obstructive sleep apnoea (OSA) both seem at least to be associated with narrowing of the upper airway and sleep-induced loss of muscle-tone. Mandibular advancement splints (MAS) have been proposed as a relatively simple method to increase oro- and hypo-pharyngeal dimensions thereby increasing the size of the airway. However, data on their effectiveness are conflicting and there are no clear indications as to which design is most effective or when they should be used. The effects of two designs of splint (types A and B) have been evaluated in 14 and nine subjects, respectively, usin

[Osteotomy of the median tubercule as a consequence of bilateral complete labio-maxillo-palatal cleft].

In the bilateral total labio-maxillary cleft the premaxilla is sometimes very protrusive at the completion of facial growth. This kind of malposition cannot be treated by orthodontics. Surgical repositioning of the premaxilla with simultaneous bilateral alveolar maxillary bone graft is done before the revision of lip and nose. Its goal is the reconstruction of a solid and harmonious dental and maxillary arch, closure of bucconasal fistulas, improvement of facial profile by closure of nasolabial angle. The authors describe the operation technique and illustrate it by two cases.
