The Use of Peripheral Nerve Stimulation in Conjunction with TMR for Neuropathic Pain.

Targeted muscle reinnervation and regenerative peripheral nerve interfaces are increasingly utilized strategies to mitigate phantom and residual limb pain in amputees. These interventions are successful, yet often imperfect in completely ameliorating neuropathic pain following amputation. Implantable peripheral nerve stimulators are another tool in the armamentarium for management of neuropathic pain. These devices have been utilized adjacent to the spinal cord and more recently in the extremities with good results, and there has been additional interest in their utility for nerve regeneration

Clinical study of combined mirror and extracorporeal shock wave therapy on upper limb spasticity in poststroke patients.

Mirror therapy is a simple, inexpensive, and patient-oriented method that has been shown to reduce phantom sensations and pain caused by amputation and improve range of motion, speed, and accuracy of arm movement and function. Extracorporeal shock wave therapy (ESWT) is a new, reversible, and noninvasive method for the treatment of spasticity after stroke. To investigate the therapeutic effect of the combination of mirror and extracorporeal shock wave therapy on upper limb spasticity in poststroke patients. We randomly assigned 120 patients into four groups: A, B, C, and D. All groups received

[Amputation of ischaemic extremities--a vascular surgeon's point of view].

The amputation surgery being one of the oldest branches of surgery itself is often times discredited since amputations are frequently considered as a result of failure of alternative therapy options. Worldwide hundreds of thousands amputations are performed annually even though especially in vascular surgery great progress in revascularisation techniques and abilities has been made. Presumably due to rising of life expectancies and increase of diabetes mellitus the number of amputations will in future even rise. Peripheral artery disease and diabetes mellitus which are often associated with a

The Berlin Brain-Computer Interface: EEG-based communication without subject training.

The Berlin Brain-Computer Interface (BBCI) project develops a noninvasive BCI system whose key features are 1) the use of well-established motor competences as control paradigms, 2) high-dimensional features from 128-channel electroencephalogram (EEG), and 3) advanced machine learning techniques. As reported earlier, our experiments demonstrate that very high information transfer rates can be achieved using the readiness potential (RP) when predicting the laterality of upcoming left- versus right-hand movements in healthy subjects. A more recent study showed that the RP similarily accompanies

Efficacy of Liposomal Bupivacaine in Regional Nerve Blocks for the Below-Knee Amputation: A Retrospective Cohort Study.

INTRODUCTION: Below-knee amputation (BKA) patients often struggle with postoperative pain as well as phantom limb pain. As excessive postoperative opioid use increases the risk of dependence, hyperalgesia, and decreased quality of life, regional nerve blocks using liposomal bupivacaine (LB) offer the promise of providing postoperative pain control while minimizing the need for opioids. This study aims to evaluate whether the use of LB in regional nerve blocks for BKA surgery prolongs postoperative analgesia compared with conventional local anesthetic blocks and no nerve blocks. METHODS: A ret

Cryoneurolysis: A Comprehensive Review of Applications in Pain Management.

Pain management is an integral part of healthcare, and cryoneurolysis, a technique that uses extreme cold to disrupt nerve conduction, has demonstrated potential in managing both acute and chronic pain. It is an alternative for patients who do not respond to traditional pain management therapies. This review examines the efficacy, application, mechanism, limitations, challenges, and advancements in cryoneurolysis. Cryoneurolysis has broad applications, including acute pain management, reducing opioid dependency, and shortening hospital stays. It effectively treats chronic pain conditions like

Time since onset might be of essence: A recommendation to assess the effects of combination of non-pharmacological neuromodulatory approaches at early stage since symptoms onset.

In the past decade researchers began to assess the potential beneficial effects of non-invasive brain stimulation (NIBS) combined with a behavioral task as a treatment approach for various medical conditions. Transcranial direct current stimulation (tDCS) applied to the motor cortex combined with another treatment approach has been assessed as analgesic treatment in neuropathic and non-neuropathic pain conditions, and was found to exert only modest pain relief. Our group results show that combined tDCS and mirror therapy dramatically reduced acute phantom limb pain intensity with long-lasting

Dilute lidocaine suppresses ectopic neuropathic discharge in dorsal root ganglia without blocking axonal propagation: a new approach to selective pain control.

Ectopic impulse discharge (ectopia) generated in the soma of afferent neurons in dorsal root ganglia (DRG) after nerve injury is believed to be a major contributor to neuropathic pain. The DRG is thus a prime interventional target. The process of electrogenesis (impulse generation) in the DRG is far more sensitive to systemically administered Na channel blockers than the process of impulse propagation along sensory axons. It should therefore be possible to selectively suppress DRG ectopia with local application of membrane-stabilizing agents without blocking normal impulse traffic. Results fro

Conus medullaris nerve root avulsions.

The association of avulsive lesions and pain has been well established in avulsions of the brachial plexus from the cervical spinal cord, but avulsive lesions of the conus medullaris have not previously been recognized or documented by direct observation. Six patients with intractable lower-extremity pain due to avulsion of nerve roots from the conus medullaris were treated by thoracolumbar laminectomy and dorsal root entry zone (DREZ) lesions. Patients with avulsion of lumbosacral roots from the conus medullaris have a characteristic clinical presentation. They are usually young men who, as a

Thermographic correlates of chronic pain: analysis of 125 patients incorporating evaluations by a blind panel.

Series of thermograms from 125 sequential participants were analyzed to determine the usefulness of thermography as a tool for evaluating chronic pain. The stability and symmetry of thermographic patterns over time among both healthy subjects and subjects whose pain remained at the same intensity across several recordings were found to be both high and consistent. This was true only if sensitivity was limited to no greater than 0.5C per color band. Greater sensitivity resulted in the creation of inconsistent asymmetrical patterns among healthy and pained subjects. Thermograms were evaluated by

A highly integrated bionic hand with neural control and feedback for use in daily life.

Restoration of sensorimotor function after amputation has remained challenging because of the lack of human-machine interfaces that provide reliable control, feedback, and attachment. Here, we present the clinical implementation of a transradial neuromusculoskeletal prosthesis-a bionic hand connected directly to the user's nervous and skeletal systems. In one person with unilateral below-elbow amputation, titanium implants were placed intramedullary in the radius and ulna bones, and electromuscular constructs were created surgically by transferring the severed nerves to free muscle grafts. The

[Musical hallucinations: 7 cases].

Musical hallucinations (MH) occurred in 7 patients (5 women and 2 men, age 58-90 yrs) with mild to severe unilateral or bilateral deafness. The hallucinations usually consisted of musical memories (childhood songs, past "hits"). They started abruptly and were identified, sometimes after a period of doubt, as hallucinations. They became "louder" in the silence and, when iterative, could be distressing. By concentrating, 3 patients could change the ongoing tune for another. Elementary (1 case) or verbal (3 cases) auditory hallucinations could be associated and, in one case, vivid visual hallucin
