Is low dose radiotherapy an effective treatment for Baker's cyst?

PURPOSE: Osteoarthritis of the knee is a common disease, often associated with a Baker's cyst. Besides osteoarthritis, also other joint pathologies of the knee can be causative for a Baker's cyst. Radiotherapy is known to be an effective treatment for osteoarthritis, with an anti-inflammatory effect. As the excessive production of synovia usually is associated with intraarticular inflammation, our hypothesis was that radiotherapy might positively influence the synovial production and reduce the volume of a Baker's cyst. MATERIALS AND METHODS: We performed a prospective trial, including 20 kne

Ultrasound guided percutaneous treatment and follow-up of Baker's cyst in knee osteoarthritis.

OBJECTIVE: Purpose of this study is to assess sonographic changes and clinical response in different subgroups of Baker's cyst patients with knee osteoarthritis after a single session of ultrasound-guided percutaneous aspiration and corticosteroid injection. MATERIALS AND METHODS: Thirty-two knee osteoarthritis patients (46-85 years, mean 58.97±9.88) with symptomatic Baker's cyst diagnosed at ultrasonography were included in the study. To determine the grade of the symptoms, Visual Analogue Scale was applied. The patients were grouped in two, as simple (n=24) and complex (n=8) Baker's cyst. T

MR arthrography of the knee: how, why, when.

MR arthrography combines the techniques of arthrography with MR imaging to benefit from the added imaging information afforded by intra-articular distention. This article reviews technical considerations for MR arthrography, potential complications, indications, pitfalls in imaging diagnosis, and commonly encountered pathology. It is an elegant study that can offer precise diagnostic information in the appropriate clinical setting.

Ganglia of the posterior cruciate ligament. A report of three cases and a review of the literature.

Two cases of isolated symptomatic ganglia and one case of a combined asymptomatic ganglion arising from the posterior cruciate ligament (PCL) are reported. One patient was a 29-year-old male who complained of posteromedial radiating knee pain. The medial McMurray test was positive. A cystic mass, 1.5 cm in diameter, arising from the femoral insertion of the PCL was removed piece-by-piece. The second case involved a 29-year-old female who complained of medial knee pain and limitation of flexion. MR scans detected a soft mass around the PCL. This was found to be a ganglion cyst surrounding the P

[Baker's cysts. A follow-up study after surgical therapy].

Baker's cysts have to be regarded as a secondary phenomenon of knee-joint diseases. In the treatment of Baker's cysts, therefore, arthroscopy should be done before extirpation. In our study we wanted to look at the results of this therapeutic concept. In the surgical clinic within the Ruhr University of Bochum 34 patients with Baker's cysts underwent an operation between 1985 and 1992. In 22 patients the main symptom was swelling in the hollow of the knee, while the others reported effusions of the knee joint, pain or restricted joint movement. In 27 cases the diagnosis was made by ultrasound

[Baker's cyst--a pre-arthrotic factor?].

Between July 1989 and 1994 127 Baker-cysts have been operated and histologically examined with synovial biopsies out of the same knee. All Baker-cysts and synovial biopsies - including out of synovial membranes without macroscopically pathological findings and without other pathological intraarticular causal lesions - revealed a chronic synovitis. The Baker-cyst has to be regarded as a primary or secondary causative factor of chronic effusions and involves the development of a chronic synovitis by the disorder of the synovial and hydraulic system of the knee-joint. The exstirpation of the Bake

Contrast arthrography in symptomatic varus knees.

The authors carried out a study of contrast arthrography in 48 cases of primary genu varum, i.e. not secondary to trauma or to pathological conditions of the hip or foot. The patients all had symptoms and were classified according to the commonly used Trillat-Dejour method into four groups or stages depending on the degree of varus and the radiographic and arthrographic findings. Associated lesions secondary to the basic pathology were observed in all cases. These ranged from hypertrophic synovitis to torn medial or lateral menisci up to total compromise of the joint typical of stage 4 arthrit

Enteropathic arthropathies.

Musculoskeletal disease occurs in association with inflammatory bowel disorders including Crohn's disease and ulcerative colitis, as well as with Whipple's disease; with enteritis caused by Salmonella, Shigella, and Yersinia; and also following intestinal bypass surgery. Extraintestinal causes of musculoskeletal alterations include Laennec's and biliary cirrhosis and pancreatitis. Three types of musculoskeletal abnormalities are recognized in patients with inflammatory bowel diseases: peripheral joint arthritis, sacroiliitis and spondylitis identical to ankylosing spondylitis, and rarely, misc

[The knee joint].

The knee is very important in the treatment of rheumatic diseases. Differentiating between the indications for different forms of therapy is dependent upon knowledge of the prospects for success. Synovectomy, preventive and palliative procedure, is the most important joint-saving measure, as confirmed by the long-term results presented here. Synoviorthese and arthroscopic operative procedures are alternatives and have expanded the spectrum. However, their long-term effectiveness has not yet been proved. Rheumatic, recurring synovitis has not yet been defined satisfactorily. Additional soft tis

Cyst-like lesions of the knee joint and their relation to incident knee pain and development of radiographic osteoarthritis: the MOST study.

OBJECTIVE: To determine whether intra- and periarticular cyst-like lesions of the knee are associated with incident knee pain and incident radiographic knee osteoarthritis (OA). DESIGN: The Multicenter Osteoarthritis (MOST) Study is a cohort of individuals who have or are at high risk for knee OA. Using a nested case-control study design, we investigated the associations of cyst-like lesions (Baker's, meniscal and proximal tibiofibular joint (PTFJ) cysts, and prepatellar and anserine bursitides) with (1) incident pain at 15- or 30-month follow-up and (2) incident radiographic OA at 30-month f

Duplex-Doppler ultrasonography in the detection of lower extremities deep venous thrombosis and in the detection of alternative findings.

The diagnoses observed in patients referred for the Doppler ultrasonographic examination of peripheral and iliac veins for suspected deep venous thrombosis (DVT) are presented in this study. During 48 months 2,610 patients were examined by duplex Doppler ultrasonography (US). Among these, 1,879 were women (72%) and 731 men (28%), with the age-range 16-91 (mean 56, 2) years. Ultrasonic scanners Acuson 128 XP 10, ATL HDI 5000, GE Logiq 7, and GE Logiq 9 were used, with transducers in the frequency range from 2.5-14 MHz. Findings were categorized into four main categories: (1) deep venous thrombo

Pearls and Pitfalls for the Direct Posterior Approach to the Proximal Tibia and Knee Joint: A Clinical Experience From a Tertiary Center in Central India.

Background The posterior approach to the proximal tibia provides direct access to posterior knee and tibial pathologies, allowing controlled identification and protection of neurovascular structures within the popliteal fossa. Despite these advantages, concerns regarding prone positioning, potential neurovascular injury, wound complications, and limited surgeon familiarity have restricted its widespread use. This study aimed to evaluate indications, perioperative complications, and functional outcomes of the posterior approach to the proximal tibia, highlighting practical pearls and pitfalls.

Ruptured Baker Cyst on 18 F-FDG PET/CT.

A 78-year-old woman with diffuse large B-cell lymphoma was referred for an 18 F-FDG PET/CT to evaluate therapy response after 6 cycles of R-mini-CHOP. A new 18 F-FDG accumulation was noticed medial in the upper part of the right lower leg, spreading along the medial head of the gastrocnemius muscle. The shaft-bow-looking curvature, arch sign, of 18 F-FDG revealed a fluid collection on CT. This typical pattern and its specific location are indicative of a ruptured Baker's cyst. It can manifest in 1 or both legs.

Sonographic assessment of musculoskeletal causes of calf pain and swelling.

Calf pain or swelling is a common presentation to the emergency department. The differential diagnoses are wide. Deep vein thrombosis (DVT) is often the first diagnosis to be excluded given its potentially fatal complications. Musculoskeletal causes of calf pain or swelling such as Baker's cyst, muscle or tendon tear, soft tissue infection, and inflammation are not uncommon and can often be confidently diagnosed with ultrasonography (US). Familiarity with these conditions and the sonographic findings would be useful in making timely and correct diagnosis.

Thrombophlebitis and pseudothrombophlebitis in the ED.

The patient presenting to the emergency department (ED) with a painful swollen lower extremity is considered to have deep venous thrombosis (DVT) until this diagnosis can be ruled out. This clinical presentation, however, is far from specific and the differential diagnosis includes symptomatic Baker's cyst, also known as pseudothrombophlebitis syndrome (PTP). This article presents two cases of PTP and reviews the literature relevant to diagnosis of DVT and PTP. Ultrasonography is now the diagnostic test of choice for both DVT and PTP, being safe, accurate, noninvasive, and rapid, and should id

[Baker's cyst in osteoarticular tuberculosis of the knee joint].

A Baker cyst (synovial cyst) in the right knee-joint was demonstrated by ultrasound in a 70-year-old woman with recurrent knee-joint effusions after minimal trauma to the knee. Radiological examination in two planes showed degenerative changes corresponding to age. Needle puncture of the knee-joint demonstrated numerous neutrophil granulocytes. As the effusion recurred after one week, despite rest and avoidance of weight bearing, arthroscopy with removal of the cyst was indicated. But surprisingly histological examination of synovial tissue revealed epithelioid granulomas and Langerhans giant

Cystic adventitial disease of the popliteal artery in female: case report and literature review of a rare differential diagnose of artery stenosis.

BACKGROUND: Cystic adventitial disease (CAD) is a rare vascular condition that causes arterial stenosis due to the presence of a cyst in the adventitia (outer layer) of the artery. It is most commonly seen in young and middle-aged men, with an occurrence rate of around 0.1% among vascular diseases. Symptoms may include intermittent claudication, rest pain, and, in severe cases, ulcers or limb ischemia. CAD is often underdiagnosed due to its rarity and the broad range of differential diagnoses for arterial obstruction. CASE PRESENTATION: This case describes a 36-year-old female who presented w

Assessment of knee osteoarthritis by ultrasonography and its association with knee pain.

BACKGROUND: Pain is a significant cause of disability in knee osteoarthritis. Conventional radiography is widely used in the assessment of knee osteoarthritis, however radiographic findings do not correlate well with pain. Ultrasonography can be used to evaluate the soft tissue structures of the knee that can be related to pain. OBJECTIVE: To evaluate pain-related soft tissue structures of the knee with ultrasonography. METHODS: This cross-sectional study included a total of 198 knees from 99 patients with knee osteoarthritis. Knee pain and functional status were evaluated by performing visu
