Modification of mandibular advancement osteotomy in a patient with Hanhart syndrome.

The oromandibular limb hypogenesis syndrome is a group of anomalies affecting the mandible, tongue, and maxilla with or without reductive limb anomalies. It was first described by Hanhart in 1950. In severe syndromic cases of mandibular hypoplasia, a number of techniques have been described for mandibular advancement including sagittal split osteotomies, segmental osteotomies, or distraction osteogenesis just to name a few. A 25-year-old male patient presented to our clinic with symptoms including difficulty in speech and eating, disability in opening the mouth, together with hand and foot abn

Cephalometric evaluation of surgical mandibular advancement.

The treatment of Class II adult individuals with mandibular deficiency has been the combination of orthodontic treatment and orthognathic surgery. Therefore, a study was conducted in which cephalometric analysis was used to evaluate the influence of dentoalveolar decompensation in Class II patients submitted to orthodontic and surgical treatment for mandibular advancement, by bilateral osteotomy of the mandibular ramus. A sample of 15 leukoderma adult female patients were selected and three cephalometric radiographs of each patient, taken before the orthodontic treatment, before surgery and af

Architectural characteristics of the normal and deformity mandible revealed by three-dimensional functional unit analysis.

The 3D architecture of the mandible contributes to the functional and morphological characteristics of the lower one third of craniofacial region. The mandible has six distinct functional units, and its architecture is the sum of balanced growth of each functional unit and surrounding matrix. A dentofacial deformity (DFD) with malocclusion can be interpreted as their unbalanced growth. In order to characterize the mandibular 3D architecture, we analyzed the 3D reconstructed computed tomography (CT) images in terms of functional units. We evaluated both sides of 30 datasets of 3D CT scans of no

Obstructive sleep apnoea syndrome: results and conclusions of a principal component analysis.

A cephalometric analysis according to Hasund, supplemented by special obstructive sleep apnoea syndrome (OSAS) parameters, was performed on 169 patients who had been referred from the sleep laboratory. Statistical analysis showed a correlation between specific cephalometric landmarks including posterior airway space (PAS), a soft palate length, hyoid position and posterior growth development of the mandible and OSAS severity. A principal component analysis differentiated between four subgroups of OSAS patients: (1) orthognathic obese subjects; (2) patients with a long soft palate and low-posit

Latex elastic-induced periodontal damage: a case report on the subsequent orthodontic management.

Proper management of a malocclusion relies on a basic understanding of the development of the dentition and the craniofacial complex. Diagnosis should only be formulated after all the relevant diagnostic records, including clinical examination and history, are reviewed. Armed with this information, the dentist forms the treatment plan. The mere occurrence of a malocclusion does not constitute a need for treatment. Injudicious intervention can be counterproductive or destructive. Through a case report, the deleterious effects of improper case management and questionable treatment mechanics are

The stability of facial osteotomies. 2. Mandibular advancement with bicortical screw fixation.

Surgical repositioning of the dento-skeletal components of the lower third of face, combined with appropriate orthodontic treatment, can be used to improve function and aesthetics. However, the attainment of three-dimensional stability following corrective jaw surgery continues to be a major problem in the post-surgical period. This paper examines the short-term (6 week post-operative) and long-term (12 months postoperative) horizontal skeletal stability of bilateral sagittal split mandibular advancement in 15 patients. The mean horizontal advancement of the mandible was 6.1 mm. Six weeks late

A modified clear twin block appliance for treatment of class II malocclusion: a randomized clinical trial.

BACKGROUND: Skeletal Class II malocclusion is commonly treated with functional appliances, but bulkiness and discomfort usually affect the compliance. Modifications of these appliances using clear thermoplastic materials could improve comfort and esthetics; though clinical evidence remains limited. OBJECTIVE: To assess the clinical effectiveness of a Modified Twin Block (MTB) appliance, fabricated from 2 mm biocompatible polyethylene terephthalate modified with Glycol (PETG) thermoplastic material, in comparison with the Conventional Twin Block (CTB), for treatment of skeletal Class II due to

Predictors of sleep disordered breathing in children: the PANIC study.

OBJECTIVE: We studied longitudinally the associations of craniofacial morphology, mouth breathing, orthodontic treatment, and body fat content with the risk of having and developing sleep disordered breathing (SDB) in childhood. We hypothesized that deviant craniofacial morphology, mouth breathing, and adiposity predict SDB among children. MATERIALS AND METHODS: The participants were 412 children 6-8 years of age examined at baseline and 329 children aged 9-11 years re-examined at an average 2.2-year follow-up. An experienced orthodontist evaluated facial proportions, dental occlusion, soft t

[Relationship between the obstructive character of the tonsils and the sagittal cephalometric measurements].

INTRODUCTION: The role of obstructive tonsils in the sagittal dimension of the skeleton-dental abnormalities is widely discussed in the literature but remains controversial. Data on the probable relationship between obstructive tonsils and the presence of these abnormalities were subjective. The objective of this study was to quantify the relationship between the obstructive character of the tonsils and the sagittal cephalometric measurements. MATERIALS AND METHODS: A cross-sectional study was performed in children aged between 6 to 12 years divided into 2 groups (A and B) according to the ob

A new protocol of Tweed-Merrifield directional force technology with microimplant anchorage.

Tweed-Merrifield directional force technology with microimplant anchorage is a useful treatment approach for a patient with a Class I or Class II dentoalveolar-protrusion malocclusion. It can create a favorable counterclockwise skeletal change and a balanced face without patient compliance. In contrast, headgear force with high-pull J-hook can obtain similar results but depends on patient cooperation. This case report presents the treatment of a patient with Class I canine and molar relationships, a convex profile with retrognathic mandible and marked lip protrusion, and excessive lower anteri

Estimated natural head position and facial morphology.

Estimated natural head position, or natural head orientation (NHO), has been proposed as a preferred reference position for assessing facial morphology. However, because this position is subjectively defined by the orthodontist, it could be influenced by facial form, which would be an undesirable attribute. The aim of this study was to assess whether NHO is influenced by facial morphology. Lateral photographs of 14 patients were used. Each was warped to produce 2 new images, with the chin positioned backwards or forwards relative to the original (+/-2 degrees change of the soft tissue N-Pg lin

Original sagittal split osteotomy revisited for mandibular distraction.

INTRODUCTION: A malformed mandible and an abnormally positioned mandibular foramen make it difficult to plan an ideal osteotomy line for mandibular distraction. In addition, there have been reports of such complications as nonunion, damage and stretch injury of the inferior alveolar nerve and tooth germ damage when conventional osteotomy or corticotomy are used for mandibular distraction. The authors utilized the original sagittal split ramus osteotomy for mandibular distraction. PATIENTS AND METHODS: Five patients (three unilateral hemifacial microsomia, one bilateral hemifacial microsomia,

Roentgencephalometric analysis of the skulls of the Luxemburgs, the second Bohemian Royal Dynasty.

X-ray films of the skull of the king and emperor Charles IV and his four wives and two sons were made under standard conditions with the mandibles fixed in a centric occlusion. The results of roentgenocephalometric analysis were compared with norms valid for the contemporary Czech population. Consistent with the somatic condition of Charles IV, the findings indicate an above-average large face with the usual anteriorotational developmental type. The facial skeleton is structurally harmonious, but the sagittal intermaxillary relations belong into skeletal class II with a relatively retruded low

Cephalometric predictors for orthopaedic mandibular advancement in obstructive sleep apnoea.

The cephalometric analysis of two groups: 30 sleep apnoea patients, and 30 age- and sex-matched control patients, established predictors for the orthopaedic mandibular advancement by means of the Esmarch device (ED) in treating obstructive sleep apnoea (OSA). Polysomnographic sleep and ventilation data gathered for each patient with and without mandibular advancement by means of the ED were compared. Cephalometric data and treatment efficacy for each patient were submitted to a regression analysis. Maximal efficacy is predicted by: 1. The combination of an orthognathic to prognathic maxilla (S
