Topical tacrolimus for the treatment of severe allergic keratoconjunctivitis in children.

PURPOSE:: Administration of eye drops containing antihistamines or sodium cromoglycate and its derivatives for the treatment of allergic keratoconjunctivitis is often insufficient and usually requires the addition of corticosteroids. However, the risk of complications, such as glaucoma and cataract, limits the use of corticosteroids to short courses, resulting in inadequate long-term treatment response. Immunosuppressive drugs have been considered as a valid alternative to steroids for atopic keratoconjunctivitis and vernal keratoconjunctivitis. This study aimed to evaluate the use of topical

The usefulness of measuring tear periostin for the diagnosis and management of ocular allergic diseases.

BACKGROUND: Chronic ocular allergic diseases such as vernal keratoconjunctivitis (VKC) and atopic keratoconjunctivitis (AKC) are accompanied by serious comorbidities; however, the underlying pathogenesis remains obscure. Furthermore, diagnosing conjunctival lesions in patients with atopic dermatitis and estimating the severity in AKC are important for the treatment of ocular allergic diseases. OBJECTIVE: We addressed whether periostin, a novel mediator and biomarker in allergic inflammation, is involved in the pathogenesis of ocular allergic diseases and whether periostin can be a biomarker f

Characterisation of the phenotype and function of monocyte-derived dendritic cells in allergic conjunctiva.

BACKGROUND: Dendritic cells (DCs) are the most potent antigen-presenting cells involved in initiating the immune response, presenting antigens to T cells and leading to T cell proliferation. In an immature state, DCs lack accessory signals required for T cell stimulation but are highly specialised to capture antigens. Full DC maturation changes the cell surface phenotype and facilitates stimulation of T cell proliferative responses. To examine the degree of DC maturity associated with vernal keratoconjunctivitis (VKC), the authors examined the phenotype and antigen-presentation capability of b

Prevalence of allergic sensitization to indoor fungi in West Virginia.

Exposure to indoor fungi is of growing concern in residential and occupational environments in the United States. The purpose of this study was to determine the prevalence of sensitization to common indoor fungal species in an atopic population. We evaluated 102 patients (73 female and 29 male patients) for immunoglobulin E (IgE) reactivity to a panel of skin-prick test (SPT) reagents used for routine allergy testing. Patients also were tested for six additional fungi that are common indoor contaminants. All patients had symptoms consistent with allergic rhinitis or asthma. The presence of spe

Allergy and the eye.

The eye represents an ideal and frequent site for the allergic reactions. The term 'allergic conjunctivitis' refers to a collection of disorders that affect the lid, conjunctiva and/or cornea. Even though the diagnosis is essentially clinical, local tests such as cytology, conjunctival provocation and tear mediator analysis can be performed. The immunoglobulin E (IgE)-mediated mechanism does not explain completely the severity and the clinical course of chronic allergic ocular diseases such as vernal (VKC) and atopic keratoconjunctivitis (AKC), which are probably also related to T cell-mediate

Roles of T-cells in the development of allergic conjunctival diseases.

Massive infiltration of eosinophils is the typical histopathologic feature of severe forms of allergic conjunctival diseases (ACDs) such as vernal keratoconjunctivitis. Although ACD is an antigen (Ag)-specific disease, eosinophils lack Ag-specific receptors. Therefore, my group studied the likely roles of immunocompetent cells bearing Ag-specific receptors in inducing conjunctival eosinophil infiltration in ACDs. To induce experimental conjunctivitis (EC), Brown Norway rats and C57BL/6 and Balb/c mice were passively immunized by the transfer of Ag-specific or primed IgE, splenocytes, or purifi

Corneal blindness: a global perspective.

Diseases affecting the cornea are a major cause of blindness worldwide, second only to cataract in overall importance. The epidemiology of corneal blindness is complicated and encompasses a wide variety of infectious and inflammatory eye diseses that cause corneal scarring, which ultimately leads to functional blindness. In addition, the prevalence of corneal disease varies from country to country and even from one population to another. While cataract is responsible for nearly 20 million of the 45 million blind people in the world, the next major cause is trachoma which blinds 4.9 million ind

Inhibition of eotaxin expression in human corneal fibroblasts by interferon-gamma.

BACKGROUND: The chemokine eotaxin is a potent and selective chemoattractant for eosinophils. The production of eotaxin by corneal fibroblasts likely contributes to eosinophil infiltration into the corneal stroma. The regulation of eotaxin synthesis in these cells was investigated by examining the effect of interferon-gamma (IFN-gamma), a T helper cell 1-derived cytokine, on eotaxin expression in cultured human corneal fibroblasts. METHODS: The release of eotaxin from cultured corneal fibroblasts was measured by enzyme-linked immunosorbent assay, and the abundance of eotaxin mRNA in these cell

Simulated overwintering of encephalitis viruses in diapausing female Culex tarsalis (Diptera: Culicidae).

Female Culex tarsalis Coquillett in reproductive diapause were infected per os or by intrathoracic inoculation with western equine encephalomyelitis (WEE) or St. Louis encephalitis (SLE) viruses during "fall," maintained over a simulated "winter," and then tested for virus infection and transmission in vitro and in vivo after "vernal" termination. Exposure of F1 progeny of field-collected females to cool temperatures and short daylength produced females in reproductive diapause that were reluctant to imbibe infectious virus from pledgets soaked with suspensions of virus, blood and sucrose (2.5

RANTES production in a conjunctival epithelial cell line.

PURPOSE: Although corneal tissue damage in allergic ocular diseases is thought to be induced by inflammatory cells that infiltrate from conjunctival tissue, the mechanisms of recruiting these cells remain unclear. The objective of this study was to demonstrate whether conjunctival epithelial cells have the ability to produce "regulated on activation, normal T-cell expressed and secreted" (RANTES). To test this hypothesis, we investigated RANTES expression in the conjunctival tissue and also RANTES production by cytokine stimulation in a human conjunctival epithelial cell line. METHODS: We inv

Time of the sidereal year affects responsiveness to the phase-resetting effects of photoperiod in the ewe.

Two groups of ovary-intact ewes were placed in separate photochambers on the day of the vernal equinox (VE). One group was exposed to a 16 h light:8 h dark (16L:8D) photoperiod and the other to 8L:16D. On the day of the summer solstice (SS) and at 90-91-day intervals thereafter [autumnal equinox (AE), winter solstice (WS), VE and SS], each group was changed to the opposite photoperiod. The latent period between each change and either onset or cessation of cycles, as determined by measuring blood progesterone concentrations, was recorded. The latent period between change to 8L:16D and onset of

Decoding Allergic Conjunctivitis: Latest Perspectives on Etiological Drivers and Immunopathological Mechanisms.

Allergic conjunctivitis (AC) is a common ophthalmic disease, ranging from mild, self-limiting forms to severe, vision-threatening conditions such as vernal keratoconjunctivitis and atopic keratoconjunctivitis. Its pathogenesis originates from the interplay between genetic predisposition and complex environmental factors, manifesting as a complex pathophysiological network. This review comprehensively elucidates recent advancements in the etiology and pathogenesis of AC. Key etiological drivers include Ig(Ig: Immunoglobulin) E-mediated reactions due to allergen sensitization, genetic susceptibi

Use of explainable AI on slit-lamp images of anterior surface of eyes to diagnose allergic conjunctival diseases.

BACKGROUND: Artificial intelligence (AI) is a promising new technology that has the potential of diagnosing allergic conjunctival diseases (ACDs). However, its development is slowed by the absence of a tailored image database and explainable AI models. Thus, the purpose of this study was to develop an explainable AI model that can not only diagnose ACDs but also present the basis for the diagnosis. METHODS: A dataset of 4942 slit-lamp images from 10 ophthalmological institutions across Japan were used as the image database. A sequential pipeline of segmentation AI was constructed to identify

Analysis of Cytokine Production Profiles of Local and Systemic Lymphocytes in Sick Building Syndrome Compared with Ocular Allergy.

PURPOSE: We have previously studied clinical and allergological aspects of sick building syndrome (SBS) cases with ocular disorders and found that SBS is suggested to be partially induced by an allergic response. We analyzed the cytokine production profiles of conjunctival and peripheral blood lymphocytes in patients with SBS with ocular manifestations to further evaluate the pathophysiology of SBS from an immunological standpoint. METHODS: We obtained conjunctival samples and peripheral blood mononuclear cells (PBMC) from 15 cases of SBS with ocular findings, 49 cases of allergic conjunctiva
