Anatomy of the female pelvic nerves: a macroscopic study of the hypogastric plexus and their relations and variations.

The autonomic nerves of the lesser pelvis are particularly prone to iatrogenic lesions due to their exposed position during manifold surgical interventions. Nevertheless, the cause of rectal and urinary incontinence or sexual dysfunctions, for example after rectal cancer resection or hysterectomy, remains largely understudied, particularly with regard to the female pelvic autonomic plexuses. This study focused on the macroscopic description of the superior hypogastric plexus, hypogastric nerves, inferior hypogastric plexus, the parasympathetic pelvic splanchnic nerves and the sympathetic fibre

A comparative study of bone biopsies from the iliac crest, the tibial bone, and the lumbar spine.

BACKGROUND: Patients with an impaired renal function show a high incidence of bone and mineral disturbances. These 'chronic kidney disease - mineral and bone disorders' (CKD-MBD) range from high turnover osteoporosis to adynamic bone disease. Currently, the histomorphometric analysis of a bone biopsy taken from the iliac crest is viewed as the gold standard for CKD-MBD subtype differentiation. However, the clinical relevance of such a biopsy is questionable since iliac crest fractures are an extremely rare finding. Therefore, we aimed to elucidate if the histomorphometric parameter 'trabecular

Variations in the Origin and Course of Right Hepatic Artery and its Surgical Significance.

INTRODUCTION: Variations in the Right Hepatic Artery (RHA) are exceedingly common and these variations are of great importance to surgeons and interventional radiologists. A thorough knowledge of the right hepatic arterial anatomy is mandatory while performing hepatic surgery and hepatic arteriography. AIM: To find out the variations in the origin and course of the RHA and its surgical significance. MATERIALS AND METHODS: This study was conducted in 60 embalmed adult cadavers of both sexes of age group between 50 to 80 years. Abdomen was opened by a linear midline incision extending from the

Comparative analysis of the anterior transpetrosal approach with the endoscopic endonasal approach to the petroclival region.

OBJECTIVE The endoscopic endonasal approach (EEA) offers direct access to midline skull base lesions, and the anterior transpetrosal approach (ATPA) stands out as a method for granting entry into the upper and middle clival areas. This study evaluated the feasibility of performing EEA for tumors located in the petroclival region in comparison with ATPA. METHODS On 8 embalmed cadaver heads, EEA to the petroclival region was performed utilizing a 4-mm endoscope with either 0° or 30° lenses, and an ATPA was performed under microscopic visualization. A comparison was executed based on measurements

Evaluation of Percutaneous First Annular Pulley Release: Efficacy and Complications in a Perfused Cadaveric Study.

PURPOSE: Trigger finger is the most common entrapment tendinopathy, with a lifetime risk of 2% to 3%. Open surgical release of the flexor tendon sheath is a commonly performed procedure associated with a high rate of success. Despite reported success rates of over 94%, percutaneous trigger finger release (PFTR) remains a controversial procedure because of the risk of iatrogenic digital neurovascular injury. This study aimed to evaluate the safety and efficacy of traditional percutaneous and ultrasound (US)-guided first annular (A1) pulley releases performed on a perfused cadaveric model. METH

Fibular head transfixion wire and its relationship to common peroneal nerve: cadaveric analysis.

Proximal tibio-fibular joint is routinely stabilised during leg lengthening, peri-articular fractures and deformity corrections of tibia. Potential injury to the common peroneal nerve at the level of the fibula head/neck junction during wire insertion is a recognised complication. Previous studies have mapped the course of the common peroneal nerve and its branches at the level of the fibular head, and guidelines are published regarding placement of proximal tibial wires. This study aims to relate the course of the common peroneal nerve to the placement of a lateral insertion fibula head trans

Cervical and retrosternal approach to the left thoracic cavity using a flexible endoscope.

OBJECTIVES: Mediastinoscopy remains the gold standard for surgical exploration of the mediastinum. The use of this approach to access the left thoracic cavity could be complicated by vascular or neurological lesion. The aim of this experimental work was to describe a new approach to the left thoracic cavity through a cervical incision and retrosternal space using a flexible endoscope as a unique instrument. METHODS: We conducted an experimental work on 12 refrigerated and non-embalmed cadavers. Through a cervical incision, we dissected the retrosternal space to the level of Louis angle and th

Relationship of the dorsalis pedis artery to the tarsal navicular.

When undertaking surgical maneuvers about the midfoot, caution must be used to avoid injury to the dorsalis pedis artery, which courses along the dorsum of the foot on the dorsolateral aspect of the tarsal navicular. In an effort to better understand the relationship of the course of the dorsalis pedis artery relative to the tarsal navicular, 128 embalmed feet (31 [48.4%] female and 33 [51.6%] male cadavers aged 15 to 91 years) were measured. The dorsalis pedis artery was dissected from its origin to its termination (i.e., the first dorsal metatarsal artery and the deep plantar perforating art

Characterization of a first thoracic rib ligament: anatomy and possible clinical relevance.

STUDY DESIGN: Cadaver dissection and measurement. OBJECTIVE: To describe a previously undocumented intracostal ligament that limits the potential space through which the T1 ventral ramus passes before joining the C8 ventral ramus. SUMMARY OF BACKGROUND DATA: Preclavicular entrapment of the T1 ventral ramus can lead to radiculopathy, neurogenic thoracic outlet syndrome, or both, the so called "double crush" phenomenon. The usual sites of entrapment include the neural foramen, the interscalene interval, an aberrant cervical rib, the first rib itself, or an apical thoracic mass. METHODS: A tot

Comparison of the flat torso versus the elevated torso shoulder pad removal techniques in a cadaveric cervical spine instability model.

STUDY DESIGN: Controlled laboratory study in a cadaveric model. OBJECTIVE: To determine if removing shoulder pads using the elevated torso technique generated less spinal segment motion than using the flat torso method. SUMMARY OF BACKGROUND DATA: Guidelines for care of the injured football player with a suspected spinal injury recommend initial immobilization with shoulder pads and helmet in place. There is a need to develop a safe protocol, for shoulder pad removal that maintains optimum cervical stability. METHODS: Five lightly embalmed cadavers were studied before and after a globally u

Morphological changes in the cervical intervertebral foramen dimensions with unilateral facet joint dislocation.

BACKGROUND: Many investigators have conducted studies to determine the biomechanics, causes, complications and treatment of unilateral facet joint dislocation in the cervical spine. However, there is no quantitative data available on morphological changes in the intervertebral foramen of the cervical spine following unilateral facet joint dislocation. These data are important to understand the cause of neurological compromise following unilateral facet joint dislocation. METHODS: Eight embalmed human cadaver cervical spine specimens ranging from level C1-T1 were used. The nerve roots of these

Pronated oblique view in assessing proximal scaphoid articular cannulated screw penetration.

PURPOSE: Volar and percutaneous techniques of screw fixation for scaphoid fractures usually do not involve direct visualization of the dorsoproximal scaphoid articular surface. Intra-articular screw penetration may be overlooked if the proper fluoroscopic views are not obtained. The hypothesis of this study is that the pronated oblique fluoroscopic view of the scaphoid is the most sensitive in detecting dorsal articular screw penetration of the scaphoid when compared with other views. METHODS: A 3.6-mm Steinmann pin was inserted along the central axis of the scaphoid using an open dorsal tech

Patterns of the circumflex femoral arteries revisited.

Knowledge of variations of the circumflex femoral arteries is important when undertaking clinical procedures within the femoral region and in hip joint replacement. Since the 19th century, many different patterns have been proposed to classify their origins. This work studied a statistically reliable sample, the lower limbs of 221 embalmed human cadavers (equal right-left and approximately equal sex distributions), and reviewed the previous literature to propose a unified and simple classification that will be useful to clinicians. Statistical comparisons were made using the chi(2) test. The m

Anatomical consequences of "open-book" pelvic ring disruption: a cadaver experimental study.

UNLABELLED: "Open-book" pelvic fractures associate a diastasis and/or a fracture of the pubic rami with a posterior pelvic disruption of the sacro-iliac joint. These uni or bilateral lesions are potentially lethal mainly due to associated injuries and massive pelvic hemorrhage. The most frequently injured arteries are parietal branch of the commune, internal or external arteries because of their proximity to the bone, the sacro-iliac joint and the inferior ligaments of the pelvis. The pelvic bone dislocation and the increase of pelvic volume facilitate blood effusion. The aim of this study was

Effect of fibular plate fixation on rotational stability of simulated distal tibial fractures treated with intramedullary nailing.

BACKGROUND: The effect of an intact fibula on rotational stability after a distal tibial fracture has, to the best of our knowledge, not been clearly defined. We designed a cadaver study to clarify our clinical impression that fixation of the fibula with a plate increases rotational stability of distal tibial fractures fixed with a Russell-Taylor intramedullary nail. METHODS: Seven matched pairs of embalmed human cadaveric legs and sixteen fresh-frozen human cadaveric legs, including one matched pair, were tested. To simulate fractures, 5-mm transverse segmental defects were created at the sa

Laparoscopic pelvic lymphadenectomy in an anatomical model: results of an experimental comparative trial.

OBJECTIVES: The aim of this paper was to compare the accuracy of laparoscopic versus open pelvic lymphadenectomy in an experimental trial. STUDY DESIGN: We performed unilateral laparoscopy pelvic lymphadenectomy (LPL) in 33 non-embalmed cadavers between the external iliac vein, the obliterated umbilical artery and the obturator nerve. Then a laparotomy was performed to inspect the LPL limits, look for laparoscopic complications and finally realize a controlateral lymphadenectomy. The LPL side was randomly decided. A pathologist counted the number of lymph nodes collected with both techniques.

Multiscale analysis of the adult human superior hypogastric plexus and hypogastric nerve.

The superior hypogastric plexus (SHP) and hypogastric nerves (HN) are components of the autonomic nervous system required for sympathetic regulation of the pelvic viscera. Despite their functional importance, the three-dimensional (3D) distribution and immunohistochemical composition of neurons in these structures remain poorly characterized. In this study, we used a multiscale imaging approach to generate a 3D anatomical and immunohistochemical characterization of the SHP and HN in adult humans. Both embalmed (body donor program) and unembalmed paraformaldehyde (PFA)-fixed specimens (organ do

Morphometric analyses of neurovascular triangles in pterional craniotomy: a cadaveric study in Eastern India.

PURPOSE: The pterional approach facilitate access to particular neurovascular structures through the optic-carotid and carotid-oculomotor triangles. Even with broad adoption, quantitative morphometric data remain limited, particularly in South Asian populations. Main objectives were to evaluate and compare the distance of optic-carotid (Triangle I) and carotid-oculomotor (Triangle II) triangles from internal aspect of pterion of both sides in cadaveric specimens using standardized pterional craniotomy techniques. METHODS: A cross-sectional cadaveric study was conducted on seven formalin embal

Topographic and morphometric anatomy of the superficial temporal artery for STA-MCA bypass in Vietnamese adults.

PURPOSE: The superficial temporal artery (STA) is the principal donor vessel for extracranial-intracranial bypass, particularly the STA-middle cerebral artery (MCA) bypass. The present study aimed to characterise the topographic and morphometric anatomy of the STA common trunk and terminal branches, to establish surface mapping data for pre-operative planning, and to evaluate the suitability of terminal branches as bypass conduits in Vietnamese adults. METHODS: Fifty hemi-head specimens from 25 embalmed Vietnamese adult cadavers were dissected. Topographic relationships between the STA, adjac
