Side effects of nasal continuous positive airway pressure in sleep apnea syndrome. Study of 193 patients in two French sleep centers.

Nasal continuous positive airway pressure (N-CPAP) is now the treatment of choice for patients with sleep apnea syndrome (SAS). Side effects and adverse reactions have been described with this device. We have therefore systematically studied side effects of N-CPAP in 193 patients recruited consecutively from two French sleep centers (Lyon and Grenoble). Patients were followed up with repeated polysomnography, clinical assessment, and a formal questionnaire about subjective benefits and side effects of treatment. The patients (mean age, 59 +/- 12 years) were obese (body mass index, 32 +/- 7 kg/

A mixed-methods pilot study of domiciliary nasal high-flow therapy for breathlessness in people with chronic obstructive pulmonary disease who do not qualify for domiciliary long-term oxygen therapy.

BACKGROUND: High-flow nasal oxygen (HFNO) therapy delivers humidified, heated air with flow rates of up to 60 L/min with oxygen entrained. HFNO has advantages over conventional oxygen therapy, including precise and reliable fraction of inspired oxygen delivery, therefore is recommended as first-line treatment for people with acute hypoxaemic respiratory failure. OBJECTIVES: This pilot study aimed to determine the feasibility and acceptability of domiciliary nasal high flow (NHF) without entrained oxygen for people with chronic obstructive pulmonary disease (COPD) and severe breathlessness. Neuroprotective effect of the combination therapy of melatonin and URB447 after neonatal hypoxia-ischemia.

BACKGROUND: Perinatal asphyxia leading to neonatal encephalopathy is one of the major causes of death and childhood disability worldwide. As therapeutic hypothermia showed detrimental effects in medium/low-income countries, new therapeutic approaches are urgently needed to treat non-cooled asphyctic newborns. Melatonin and cannabinoids have emerged as possible neuroprotective strategies because of their multiple-acting mechanisms. The objective of this study was to assess the neuroprotective effects of combining melatonin and the cannabinoid URB447 after moderate-to-severe neonatal hypoxia-isc

Examining the Relationship Between Polystyrene Microplastics and Male Fertility: Insights From an In Vivo Study and In Vitro Sertoli Cell Culture.

BACKGROUND: While polystyrene microplastics (PS-MPs) are emerging as potentially significant health threats, linked to cancer and reproductive dysfunction, their precise effects on human health remain largely unknown. We aimed to investigate the underlying mechanisms promoting microplastic-induced damage in the reproductive system. METHODS: Thirty C57BL/6 male mice were randomly allocated into six equal-sized groups. Mice were exposed to fluorescent PS-MPs (5 µm, < 18%, green) at a dose of 1 and 3 mg/dL via oral gavage for 28 and 56 days, respectively (control, 0 mg/dL). The presence of antib

A laminar flow unit for the care of critically ill newborn infants.

INTRODUCTION: Medical and nursing care of newborns is predicated on the delicate control and balance of several vital parameters. Closed incubators and open radiant warmers are the most widely used devices for the care of neonates in intensive care; however, several well-known limitations of these devises have not been resolved. The use of laminar flow is widely used in many fields of medicine, and may have applications in neonatal care. OBJECTIVE: To describe the neonatal laminar flow unit, a new equipment we designed for care of ill newborns. METHODS: The idea, design, and development of t

The value of cell cultures for the diagnosis of mixed myelodysplastic/myeloproliferative disorders.

BACKGROUND AND OBJECTIVE: Myelodysplastic syndromes (MDS) are a group of disorders characterized by dyshematopolesis in bone marrow (BM) and peripheral blood (PB) cytopenias. In recent years particular attention has been paid to myeloproliferative disorders with dysplastic features or myelodysplastic syndromes that evolve into a myeloproliferative disorder. The present study was designed to analyze patients with MDS but with a normal or increased colony forming capacity, in order to see whether or not cell cultures could contribute to the diagnosis of intermediate MDS-MPD conditions. Heliox for croup in children.

BACKGROUND: Croup is an acute viral respiratory infection with upper airway mucosal inflammation that may cause respiratory distress. Most cases are mild. Moderate to severe croup may require treatment with corticosteroids (the benefits of which are often delayed) and nebulised epinephrine (adrenaline) (the benefits of which may be short-lived and which can cause dose-related adverse effects including tachycardia, arrhythmias, and hypertension). Rarely, croup results in respiratory failure necessitating emergency intubation and ventilation. A mixture of helium and oxygen (heliox) may prevent m

Lung Volume Change Under Apnoeic Oxygenation With Different Flow Rates in Children: A Single-Centre Prospective Randomized Controlled Non-Inferiority Trial.

BACKGROUND: High-flow oxygen in children prolongs the apnea time. The exact mechanism remains unclear. AIMS: This study investigated whether low- and high-flow nasal oxygen are non-inferior to very high-flow oxygen in preventing lung volume loss during apnoea in children under general anesthesia. We also examined whether early onset oxygen using the Optiflow Switch cannula reduces lung volume loss compared to conventional late-onset application. Finally, we assessed the timing and regional distribution of lung volume changes using electrical impedance tomography (EIT). METHODS: We conducted

Safety and efficacy of double plastic wrap in reducing insensible water loss in preterm infants in first week of life - a pilot randomized controlled trial from a low-to-middle-income country.

BACKGROUND: Preterm infants experience excessive insensible water loss in postnatal period. Established practices like cling film might not be sufficient alone in reducing this loss. Being expensive, humidified incubators might not be affordable in developing countries. Hence, we tried to explore double plastic wrap (cling film attached to the bassinet wall and occlusive plastic wrap covering the baby) as a low-cost solution of increased insensible water loss in postnatal period. METHODS: In this pilot trial 63 inborn infants of less than 32 weeks of gestation or birth weight less than 1200 g

Seasonal variations in developmental competence and relative abundance of gene transcripts in buffalo (Bubalus bubalis) oocytes.

Hot season is a major constraint to production and reproduction in buffaloes. The present work aimed to investigate the effect of season on ovarian function, developmental competence, and the relative abundance of gene expression in buffalo oocytes. Three experiments were conducted. In experiment 1, pairs of buffalo ovaries were collected during cold season (CS, autumn and winter) and hot season (HS, spring and summer), and the number of antral follicles was recorded. Cumulus oocyte complexes (COCs) were aspirated and evaluated according to their morphology into four Grades. In experiment 2, G

The response of bovine granulosa cells to different gonadotrophins in culture.

Previous studies with bovine granulosa cells cultured in vitro indicated that follicle-stimulating hormone (FSH) stimulated differentiation and progesterone production of granulosa cells in a dose-dependent manner, this was due mainly to an increase in the number of differentiated cells. The objectives of the present study were to investigate (1) whether the response of bovine granulosa cells in culture to luteinising hormone (LH) and equine chorionic gonadotrophin (eCG) was similar to the response to FSH, and (2) whether granulosa cells derived from different cattle breeds responded similarly

Safety and effectiveness of a portable nasal high-flow device for improving exercise tolerance in patients with chronic obstructive pulmonary disease: a study protocol for an open-label, randomized, crossover pilot trial.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory disease characterized by airflow limitation and exertional dyspnea, leading to reduced exercise tolerance and physical inactivity. Nasal high-flow (NHF) therapy delivers heated and humidified gas at a high flow rate through a nasal cannula, washing out anatomical dead space, providing a small degree of positive airway pressure, improving ventilation efficiency, and reducing the work of breathing. In addition, adequate humidification may enhance ciliary clearance. The AIRVO3TM (Fisher & Paykel Healthcare, Aucklan

Feasibility and safety of oxygen-free nasal high flow therapy in the acute postoperative period: a single-center randomized study.

BACKGROUND: Routine postoperative oxygen administration in the general ward is widely practiced despite limited evidence of benefit in low-risk surgical patients and increasing concerns regarding delayed detection of hypoventilation and unnecessary hyperoxia. Nasal high flow (NHF) therapy can reduce respiratory effort even without supplemental oxygen, but its feasibility and safety in the general ward during the acute postoperative period remain uncertain. OBJECTIVE: This randomized feasibility study aimed to evaluate the feasibility, safety, and protocol adherence of oxygen-free NHF therapy

Fetal cardiovascular changes during open and fetoscopic in-utero spina bifida closure.

OBJECTIVE: Fetoscopic closure of spina bifida using heated and humidified carbon dioxide gas (hhCO2) has been associated with lower maternal morbidity compared with open closure. Fetal cardiovascular changes during these surgical interventions are poorly defined. Our objective was to compare fetal bradycardia (defined as fetal heart rate (FHR) < 110 bpm for 10 min) and changes in umbilical artery (UA) Doppler parameters during open vs fetoscopic closure. METHODS: This was a prospective cohort study of 22 open and 46 fetoscopic consecutive in-utero closures conducted between 2019 and 2023. Bot

Intermittent maternofetal oxygenation during late gestation improved birthweight, neonatal growth, body symmetry, and muscle metabolism in intrauterine growth-restricted lambs.

In humans and animals, intrauterine growth restriction (IUGR) results from fetal programming responses to poor intrauterine conditions. Chronic fetal hypoxemia elevates circulating catecholamines, which reduces skeletal muscle β2 adrenoceptor content and contributes to growth and metabolic pathologies in IUGR-born offspring. Our objective was to determine whether intermittent maternofetal oxygenation during late gestation would improve neonatal growth and glucose metabolism in IUGR-born lambs. Pregnant ewes were housed at 40 °C from the 40th to 95th day of gestational age (dGA) to produce IUGR
