Client attachment to therapist: Relation to client personality and symptomatology, and their contributions to the therapeutic alliance.

This study examined the relation of client attachment to the therapist to diverse facets of the therapeutic alliance, client personality, and psychopathological symptoms, as well as the relative importance of therapeutic attachments, personality, and symptomatology in predicting the alliance. Eighty clients in ongoing therapy completed measures of client attachment to therapist (CATS), personality (6FPQ), psychopathological symptoms (BSI), and therapeutic alliance (WAI-Short, CALPAS, HAQ). Secure and Avoidant-Fearful attachment to the therapist correlated positively and negatively, respectivel

[Attachment Based Short Intervention During Inpatient Treatment of Adolescents].

Attachment Based Short Intervention During Inpatient Treatment of Adolescents In clinical attachment research the established and economic Adult Attachment Projective Picture System (AAP) has been increasingly used in the past few years to assess the attachment representations in adults and recent studies demonstrated sufficient validity for assessing attachment representations in adolescents. Apart from coding attachment classifications, the AAP provides useful and clinically valid information concerning attachment related defences, aspects of capacity to act and synchrony in relationships an

The living with dysarthria group: implementation and feasibility of a group intervention for people with dysarthria following stroke and family members.

BACKGROUND: The broad life implications of acquired dysarthria are recognized, but they have received little attention in stroke management. Reports of group therapy, which may be a suitable approach to intervention, are not available for stroke-related dysarthria. AIMS: To examine the operational feasibility of and response to a new eight-session weekly group intervention programme, Living with Dysarthria, designed for people with chronic dysarthria following stroke and their main communication partners. METHODS & PROCEDURES: The target participation was for programme completion by two grou

Residential cognitive therapy versus residential interpersonal therapy for social phobia: a randomized clinical trial.

Eighty patients meeting DSM-IV criteria for social phobia were randomly assigned to 10 weeks of residential cognitive therapy (RCT) or residential interpersonal therapy (RIPT). Subjects were assessed at pretreatment, midtreatment, posttreatment, and 1 year after end of treatment. The patients reported chronic, highly comorbid social phobia. Most had tried other treatments without success. Existing individual treatment protocols for cognitive therapy and interpersonal therapy were extensively modified for an integrated group, individual, and residential format. The RCT and RIPT patients improve

One-year follow-up study of posttraumatic stress disorder among survivors of Cambodian concentration camps.

One year after receiving a diagnosis of posttraumatic stress disorder, five of 12 Cambodian concentration camp survivors who entered a treatment program no longer met the diagnostic criteria for the disorder, and symptoms in three others had improved. The intrusive symptoms of nightmares, sleep disorders, and startle reactions showed the most consistent improvement. Avoidance behavior, shame, and caring for others improved the least. The authors advocate use of tricyclic antidepressant medication and a consistent, supportive long-term psychotherapeutic commitment in treating this severely trau

Parent-assisted transfer of children's social skills training: effects on children with and without attention-deficit hyperactivity disorder.

OBJECTIVE: Previous research has demonstrated that peer rejection is a significant part of the clinical presentation of many children with attention-deficit hyperactivity disorder (ADHD). Outcome studies of treatment interventions have typically failed to show generalization of treatment gains to the home and classroom. This has been especially true for children who have comorbid oppositional defiant disorder (ODD). The present study was intended to demonstrate generalization of an outpatient social skills training program when parents were trained in skills relevant to their child's social ad

[Cognitive Behavior Therapy for Eating Disorders].

Cognitive dysfunction such as body-image disturbance and undue influence of body weight on self-worth is a conspicuous feature of eating disorders. The cognitive problems are known to be extremely difficult to treat. Why and how, therefore, is cognitive behavioral ther- apy (CBT) recommended, with high quality evidence, in clinical guidelines such as the NICE guidelines in the UK? In reverse direction to the history of eating disorders, namely anorexia nervosa first and then bulimia, CBT was developed for bulimics first and then after its establishment, the skills were applied to anorexia nerv

What is a "good outcome" in psychotherapy? A qualitative exploration of former patients' point of view.

The aim of this study was to explore how former psychotherapy patients characterize "good outcome" after having completed treatment that they described as successful, in the sense that therapy had changed their lives in ways that made a difference. Semistructured qualitative depth interviews were conducted with 10 former psychotherapy patients. A hermeneutical-phenomenological approach was used to analyse interview transcripts. The former patients' descriptions of good outcome clustered around four themes: establishing new ways of relating to others; less symptomatic distress, or changes in be

Special education versus inclusive education: the role of the TEACCH program.

Our study aimed at comparing, over a period of 3 years, the effectiveness of three different educational approaches addressed to children with autism and severe mental retardation. The first one was a treatment and education of autistic and related communication handicapped children (TEACCH) program implemented in a residential center; the second was a TEACCH program implemented at home and at mainstream schools, after a specific parent psychoeducational training; the third approach referred to inclusive education in mainstream schools, in which a nonspecific approach was implemented. Trial of interpersonal counselling after major physical trauma.

OBJECTIVE: The purpose of the present study was to determine if interpersonal counselling (IPC) was effective in reducing psychological morbidity after major physical trauma. METHODS: One hundred and seventeen subjects were recruited from two major trauma centres and randomized to treatment as usual or IPC in the first 3 months following trauma. Measures of depressive, anxiety and post-traumatic symptoms were taken at baseline, 3 months and 6 months. The Structured Clinical Interview for DSM IV diagnoses was conducted at baseline and at 6 months to assess for psychiatric disorder. RESULTS: F

Child and adolescent depression intervention overview: what works, for whom and how well?

The authors review the currently available evidence-based treatments of child and adolescent major depressive disorder. Medication monotherapy, namely with selective serotonin reuptake inhibitors, is supported by large clinical trials in adolescents. For mild to moderate depression, cognitive behavior therapy (CBT) and interpersonal therapy are reasonable options as monotherapies. There is also evidence that the combination of medication and CBT is superior to medication alone for accelerating the pace of treatment response and remission, despite some negative studies. Response, remission, and

Participation-engagement: a philosophically based heuristic for prioritizing clinical interventions in the treatment of comorbid, complex, and chronic psychiatric conditions.

We propose Participation-Engagement (PAR-EN) as a philosophically based heuristic for prioritizing interventions in comorbid, complex, and chronic psychiatric conditions. Drawing from 1) the sociologist Talcott Parsons, 2) the continental-philosophical tradition, and 3) our own previous work (Davidson & Shahar, 2009; Shahar, 2004, 2006), we argue that participation in personally meaningful life goals represents a hallmark of mental health. Symptoms and vulnerabilities that impede such participation should therefore be targeted vigorously, whereas others which do not pose such imminent threats

The use of BRIAC for evaluating therapeutic effectiveness.

Forty-six children in eight different psychotherapeutic settings were evaluated at the beginning and at the end of a 12-month period by means of the Behavior Rating Instrument for Autistic Children (BRIAC). The younger children (3 to 6 years of age) were initially more severely disturbed and improved significantly more than the older children (7 to 9 years of age). The large, bleak, custodial residential center produced significantly less improvement in the children than did the other settings. However, among good programs--i.e., those which were sensitive to the behaviors and needs of the chi
