Half-Dose Direct Oral Anticoagulation Versus Standard Antithrombotic Therapy After Left Atrial Appendage Occlusion.

OBJECTIVES: This study evaluated the long-term efficacy of a standard antithrombotic strategy versus half-dose direct oral anticoagulation (DOAC) after Watchman implantation. BACKGROUND: No consensus currently exists on the selection of the most effective antithrombotic strategy to prevent device-related thrombosis (DRT) in patients undergoing endocardial left atrial appendage closure. METHODS: After successful left atrial appendage closure, consecutive patients were prescribed a standard antithrombotic strategy (SAT) or long-term half-dose DOAC (hdDOAC). The primary composite endpoint was D

Clinical use of AcuNav diagnostic ultrasound catheter imaging during left heart radiofrequency ablation and transcatheter closure procedures.

BACKGROUND: AcuNav ultrasound catheter (UC) (10F, 5.5-10 MHz) has unique advantages for left heart imaging with its 4-way tip flexible maneuverability, maximal 16-cm intracardiac imaging depth, and Doppler and color flow imaging capability. METHODS: We assessed the initial use of this UC in 40 consecutive patients (34 men; age 53 +/- 11 years old). All patients were also undergoing transseptal catheterization for percutaneous catheter mapping and ablation of either left atrium (focal initiated atrial arrhythmia/fibrillation, n = 32) or left ventricle (ventricular tachycardia, n = 4), or trans

Feasibility and Safety of Pulsed Field Ablation for Coronary Sinus and Left Atrial Appendage Isolation and Mitral Isthmus Ablation: Acute and Chronic Findings.

BACKGROUND: The safety and efficacy of pulsed field ablation for pulmonary vein and posterior wall isolation in atrial fibrillation ablation are well established; however, evidence regarding its use in extra-pulmonary vein areas remains limited. The aim of this study was to assess the feasibility and durability of pulsed field ablation for coronary sinus (CS) and left atrial appendage (LAA) isolation and mitral isthmus (MI) ablation. METHODS: We analyzed data from consecutive patients who underwent repeat atrial fibrillation ablation with pulsed field ablation between February and October 202

Neurologic Complications of Cardiac Disease and Cardiac Procedures.

OBJECTIVE: This article reviews the neurologic complications of common cardiac conditions and cardiac procedures. LATEST DEVELOPMENTS: During the past two decades, there has been an expansion of pharmacologic and procedural strategies to reduce stroke risk in patients with atrial fibrillation (eg, direct oral anticoagulants, left atrial appendage occlusion, and catheter ablation), patent foramen ovale (ie, percutaneous closure in selected patients), and those undergoing cardiac surgery (eg, embolic protection devices). ESSENTIAL POINTS: Because stroke is the most common neurologic complicati

Surgical considerations for atrial functional regurgitation of the mitral and tricuspid valves based on the etiological mechanism.

Atrial functional mitral regurgitation is a hot research topic in the field of mitral valve disease. Atrial functional mitral regurgitation is distinctly different from ventricular functional mitral regurgitation. The surgical indications for atrial functional mitral regurgitation have not been well established because of the small amount of evidence gathered to date. Mitral annular plication with an artificial ring is an essential surgical procedure because dilatation of the mitral valve annulus is a main factor underlying this pathology. Most of these cases can be treated by mitral annulopla

Renal and Splenic Infarctions: Unmasking Rheumatic Mitral Valve Disease in a Young Adult.

Systemic thromboembolism is an uncommon but clinically significant complication of rheumatic heart disease (RHD), most often seen in association with infective endocarditis. We describe a 39-year-old woman with no known cardiovascular comorbidities who presented with acute left-sided abdominal pain. Imaging revealed concurrent renal and splenic infarctions. Further evaluation uncovered paroxysmal atrial fibrillation, severe mitral regurgitation, and moderate to severe mitral stenosis with marked left atrial enlargement. Transthoracic and transesophageal echocardiography confirmed the diagnosis

Evaluation of Multimodality LAA Leak Closure Methods Following Incomplete Occlusion: The LAA Leak Study.

BACKGROUND: Incomplete left atrial appendage (LAA) closure is an evolving topic of clinical significance and thromboembolic potential, with recent long-term studies suggesting lower cutoffs for relevant leak size. OBJECTIVES: The aim of this prospective observational study was to assess 3 different closure techniques for persistent peridevice leaks after incomplete LAA closure and compare their efficacy and safety outcomes. METHODS: We studied 160 patients (mean age 72 ± 9 years; 71% men) who underwent 1 of the 3 available modalities (detachable embolization coils, vascular plugs or septal o

Real-world outcomes of Left Atrial Appendage Occlusion with the Amplatzer™ Amulet™: a multicentre observational registry from Southeast Asia.

BACKGROUND: Left Atrial Appendage Occlusion (LAAO) is an alternative therapy for atrial fibrillation (AF) patients at high risk of stroke who have contraindications to long term oral anticoagulation (OAC). However, real-world data specifically regarding the Amplatzer™ Amulet™ device remains limited in Southeast Asian (SEA) populations. This study evaluated the procedural characteristics, safety profiles and 1-year clinical outcomes of the Amulet™ device in the multi-country SEA cohort. METHODS: This was a multicentre observational study included four centres across three SEA countries (Malaysi

Unroofed coronary sinus and coronary sinus orifice atresia. Implications for management of complex congenital heart disease.

OBJECTIVES: The aim of this study was to assess the morphology of the coronary sinus, its drainage and associated cardiac malformations when there is either complete unroofing of the coronary sinus or atresia of its connection to the right atrium. BACKGROUND: As more children with complex cardiac anomalies are accepted for primary surgical repair or palliation with cavopulmonary anastomoses, a knowledge of coronary sinus and systemic venous anomalies is important if coronary venous return is to be preserved and residual shunts avoided. METHODS: Twenty-six heart-lung specimens without a coron

Definite Device-Related Thrombus on Cardiac Computed Tomography as a Fabric-Related Thrombus Mimic on Transesophageal Echocardiography.

BACKGROUND: Left atrial appendage closure (LAAC) is an established stroke-prevention option for atrial fibrillation patients unsuitable for long-term oral anticoagulation (OAC), while device-related thrombus (DRT) remains a complication. Cardiac computed tomography (CT) and transesophageal echocardiography (TEE) are modalities for its detection. CASE SUMMARY: A patient after LAAC using WATCHMAN-FLX (Boston Scientific) underwent routine cardiac CT, which demonstrated an irregular, hypoattenuated surface lesion fulfilling criteria for definite DRT. However, the mass had a somewhat atypical conf

Late discovery of left atrial appendage occluder device embolization: a case report.

BACKGROUND: Left atrial appendage (LAA) closure has been well evaluated in the prevention of stroke in patients with atrial fibrillation. Device embolization remains one of the most common complications. To the best of our knowledge, there have been no reports of late discovery of LAA occluder device embolization at 1.5 years after implantation. CASE PRESENTATION: We describe the case of a 77-year-old man who underwent uneventful LAA closure. Echocardiography performed the next day showed the device in place. The patient was discharged but was then lost to follow-up. 1.5 years later, he was a

Fourth-Generation Cryoablation Based Left Atrial Appendage Isolation for the Treatment of Persistent Atrial Fibrillation: First Case Report.

BACKGROUND Pulmonary vein isolation (PVI) is an effective treatment strategy for patients with paroxysmal atrial fibrillation (AF), yet it is associated with limited success rates in patients with persistent AF (PersAF). The left atrial appendage (LAA) was recently identified as a target of catheter ablation especially in PVI non-responders and LAA-isolation (LAAI) by cryoballoon or radiofrequency was shown to be effective. Recently the fourth-generation cryoballoon (CB4) was introduced to clinical practice. Here we are demonstrating the first case report of CB4-based LAAI followed by LAA-clos

Feasibility of the AtriClip Pro Left Atrium Appendage Elimination Device via the Transverse Sinus in Minimally Invasive Mitral Valve Surgery.

BACKGROUND: Achieving external access to and manual occlusion of the left atrial appendage (LAA) during minimally invasive mitral valve surgery (MIMVS) through a small right thoracotomy is difficult. Occlusion of the LAA using an epicardial closure device seems quite useful compared to other surgical techniques. METHODS: Fourteen patients with atrial fibrillation underwent MIMVS with concomitant surgical occlusion of the LAA using double-layered endocardial closure stitches (n=6, endocardial suture group) or the AtriClip Pro closure device (n=8, AtriClip group) at our institution. The primary

Long-term outcomes of one-stop unilateral thoracoscopic hybrid ablation for persistent atrial fibrillation after failed catheter ablation.

BACKGROUND: Hybrid ablation is a treatment option for persistent (PsAF) or long-standing persistent atrial fibrillation (LS-PsAF) after failed catheter ablation (CA), but long-term outcomes (> 5 years) and predictors of recurrence after hybrid ablation in this population have not been well characterized in prior studies. METHODS: We retrospectively analyzed 44 consecutive patients with symptomatic PsAF or LS-PsAF who underwent one-stop unilateral thoracoscopic hybrid ablation with left atrial appendage (LAA) management between May 2015 and April 2021. The procedure consisted of same-day epica

Inverted left atrial appendage mimicking a left atrial mass after surgical repair of an atrial septal defect: a case report.

BACKGROUND: Inverted left atrial appendage (ILAA) is a rare condition following cardiac surgery. Failure to recognize the condition or making misdiagnosis of a tumour, a thrombus or vegetation can lead to unnecessary and potentially adverse events. We present a case of ILAA after surgical repair of an atrial septal defect (ASD) in a young female. CASE SUMMARY: A 3-year-old caucasian female was admitted for surgical repair of an ASD. The intraoperative course was uneventful until the opening of the right atrium (RA) after the commencement of cardiopulmonary bypass (CPB) and vacuum application,

Diagnosis and Management of Inadvertent Iliac Vein Injury During Structural Heart Interventions.

Femoral venous access is increasingly used by structural interventional cardiologists in procedures such as patent foramen ovale, atrial septal defect, and left atrial appendage closure, as well as tricuspid and mitral interventions. Containing less muscular and elastic fibers than arteries, veins are fragile structures that are more easily pierced than arteries. In this case series, we present 2 cases of iliac vein perforation during structural cardiology interventions. Even mild resistance on advancement of a large sheath should prompt suspicion of venous perforation, in which case control a

[Leadless Pacemaker Implantation During Tricuspid Valve Surgery in the Presence of a Permanent Pacemaker with Tricuspid Regurgitation:Report of a Case].

A 74-year-old man was implanted with a permanent pacemaker for sick sinus syndrome ten years earlier and permanent atrial fibrillation( AF). Echocardiography indicated progressive severe tricuspid regurgitation (TR) and right ventricular systolic dysfunction with tricuspid annular dilatation and tricuspid valve tethering. The pacemaker lead passing through the tricuspid valve may have contributed to TR, therefore we decided to perform tricuspid valve surgery, pacemaker lead removal, and leadless pacemaker implantation simultaneously during open heart surgery. Tricuspid annuloplasty was perform

Novel nonpharmacologic approaches for stroke prevention in atrial fibrillation: results from clinical trials.

Atrial fibrillation (AF), the most common cardiac arrhythmia, confers a 5-fold risk of stroke that increases to 17-fold when associated with mitral stenosis. At this time, the most effective long-term solution to protect patients from stroke and thromboembolism is oral anticoagulation, either with vitamin K antagonists (VKAs) or a novel oral anticoagulant (NOAC). Despite the significant benefits they confer, both VKAs and NOACs are underused because of their increased potential for bleeding, and VKAs are underused because of their narrow therapeutic range, need for regular international normal

CT support of cardiac structural interventions.

Due to its high temporal and isotropic spatial resolution, CT has become firmly established for pre-procedural imaging in the context of structural heart disease interventions. CT allows to very exactly measure dimensions of the target structure, CT can provide information regarding the access route and, as a very valuable addition, volumetric CT data sets can be used to identify fluoroscopic projection angulations to optimally visualize the target structure and place devices. This review provides an overview of current methods and applications of pre-interventional CT to support adult cardiac
