Asymptomatic cor triatrium in an elderly patient--observation by biplanar transesophageal Doppler echocardiography.

We present a case of asymptomatic cor triatrium in a 78-year-old man, in whom the condition was diagnosed and the severity of the lesion was accurately assessed by biplanar transesophageal echocardiography. To our knowledge, our patient is the oldest living person with cor triatrium.

Pulsed Doppler echocardiographic detection of total anomalous pulmonary venous return: resolution of left atrial line.

During precatheterization M mode echocardiographic examination, 33 infants were identified as having findings including right ventricular enlargement and presence of a line within the left atrial portion of the M mode tracing suggesting the possibility of cor triatiatum or total anomalous pulmonary venous return. Pulsed Doppler echocardiography was used to sample blood characteristics on either side of the left atrial line to determine which line was artifactual and which was indicative of an important structure. Five patients whose blood flow characteristics were different on either side of t

Incidental discovery of an unusual right atrial membrane in an adult patient.

We describe presence of an unusual right atrial membrane in a 30-year old female with end stage renal disease, hypertension and peripheral vascular disease. The patient was scheduled for midline sternotomy and pericardiotomy and removal of a migrated vascular stent in the right pulmonary artery. An intraoperative transesophageal echocardiogram (TEE) revealed an unusual membranous structure with fenestrations that stretched across the right atrium with attachments superiorly at the free wall and inferiorly at the inter-atrial septum. There was no evidence of flow obstruction across the tricuspi

[The pathological remnants of the right sinus venosus valve. A presentation of 6 hearts of the cor tritriatum dexter type].

Six hearts specimens of cor triatriatum dextrum, eight with the Chiari's network, and 3 with a membranous remnant of the crista terminalis are studied. These anomalies are interpreted as varying degrees of persistence of the right valve of the sinus venous which reflect different stages of their morphogenesis. The most frequent congenital heart defects associated with these sinus remnants are the absence of a right atrioventricular connexion and pulmonary atresia with intact ventricular septum. There may be some haemodynamic factors in the right atrium such as partial of complete obstruction b

[Subtotal cor triatrium with left partial anomalous pulmonary venous connection--successful correction in an infant].

This report concerns a 10-month-old infant with subtotal cor triatrium associated with left sided partial anomalous pulmonary venous connection to the innominate vein. In the operation, we found that the fossa ovalis existed between the right atrium and the accessory atrial chamber. We performed a radical operation which consisted of a resection of the intra-left-atrial diaphragma and a anastomosis of the vertical vein to the left atrium. His postoperative course was uneventful, and the result was satisfactory. We have presented and reviewed our case, and then discussed the embryogenesis and t

[Echo-angiocardiographic correlations in congenital abnormalities of the mitral valve (author's transl)].

Echocardiographic findings satisfactorily compared with angiocardiographic data in six infants affected by congenital abnormalities of the mitral valve. Peculiar echocardiographic features were as follows: a) mitral stenosis: reduced E-F slope of the anterior mitral leaflet, paradoxical diastolic movement of the posterior leaflet, enlargement of the left atrium; b) "parachute mitral valve": small, dysmorphic, stiff mitral valve, with an extra echo projecting between those of the two leaflets; c) "cor triatriatum": enlarged left atrium whose cavity is divided in two portions by an extra echo; d

Treatment of caudal cavoatrial junction obstruction in a dog with a balloon-expandable biliary stent.

A 2-year-old intact female mixed breed dog was presented for ascites. Echocardiography demonstrated severe obstruction at the level of the caudal right atrium. Initially, a variant of cor triatriatum dexter was diagnosed, and balloon catheter dilation was performed. However, ascites recurred within a week. Further imaging revealed an obstruction at the entrance of the caudal vena cava into the right atrium rather than a dividing membrane in the right atrium. The diagnosis was revised to suprahepatic obstruction of the caudal vena cava because of remnant Eustachian valve tissue. Deployment of a

Klippel-Feil syndrome and levo-looped transposition of the great arteries.

Klippel-Feil syndrome is a skeletal disorder characterised by low hairline and a short neck due to abnormal fusion of two or more cervical vertebrae. Although congenital heart and lung defects are infrequent, some abnormalities such as cor triatriatum, coarctation of the aorta, total anomalous pulmonary venous connection, or lung agenesis have been reported. The challenge of recognising Klippel-Feil syndrome lies in the fact that there is an association of this syndrome with other significant conditions such as skeletal, genitourinary, neurological, ear, and some cardiac defects. We report a K

Perioperative Management of Sickle Cell Disease in Complex Congenital Cardiac Surgery: A Compilation of Two Cases.

Preoperative exchange transfusion is frequently recommended in patients with homozygous sickle cell anemia (homozygous SS) who undergo cardiopulmonary bypass to reduce the concentration of circulated sickle hemoglobin. The information regarding the ideal level of sickle hemoglobin for sickle cell disease (SCD) patients who require surgery is still divergent in the literature. We present the successfully managed cases of two children aged 11 months and three years with homozygous SS who underwent cardiopulmonary bypass for double-outlet right ventricle and cor-triatriatum sinistrum, respectivel

Persistent left superior vena cava: a case report and review of literature.

Persistent left superior vena cava is rare but important congenital vascular anomaly. It results when the left superior cardinal vein caudal to the innominate vein fails to regress. It is most commonly observed in isolation but can be associated with other cardiovascular abnormalities including atrial septal defect, bicuspid aortic valve, coarctation of aorta, coronary sinus ostial atresia, and cor triatriatum. The presence of PLSVC can render access to the right side of heart challenging via the left subclavian approach, which is a common site of access utilized when placing pacemakers and Sw

Inability to cannulate the coronary sinus in patients with supraventricular arrhythmias: congenital and acquired coronary sinus atresia.

We report 4 cases of congenital and acquired coronary sinus ostial atresia incidentally found during electrophysiologic assessment for supraventricular arrhythmias. Congenital variants consisted of coronary sinus drainage predominantly via persistent left superior vena cavae and partial coronary sinus unroofing into the left atrium. The acquired variant was inadvertently produced during surgery for cor triatriatum. A variety of electrophysiologic substrates including right and left-sided accessory pathways and both typical and atypical AV nodal reciprocating tachycardia were identified. Approa

Congenital cardiac malformation with three-chambered right atrium and a persistent left cranial vena cava in a dog.

This report describes an unusual congenital abnormality in a dog in which multiple distinct membranes were observed within the right atrium, creating obstruction to venous return from both the cranial vena cava and the caudal vena cava. A persistent left cranial vena cava was also identified. In addition to a membrane in the typical location for cor triatriatum dexter, the dog also had a perforated membrane separating the main right atrial body and tricuspid valve from a more cranial right atrial chamber and the right cranial vena cava. Balloon dilation was performed successfully to alleviate

Multimodality imaging of an asymptomatic giant right atrial appendage aneurysm.

Right atrial appendage (RAA) aneurysm is a rare congenital cardiac abnormality thought to be caused by dysplasia of the muscular wall of the RA and RAA. Structural, contractile, or electrical atrial changes are associated with the onset and progression of atrial fibrillation. The RAA aneurysm can have various symptoms, such as thromboembolic complications. Diagnosis of RAA aneurysm greatly depends on imaging modalities, among which echocardiography is the most widely used one. Echocardiography is very effective in discovering the aneurysm, distinguishing it from other abnormalities such as per

Remnant of the common pulmonary vein mistaken for a left atrial mass: clarification by transoesophageal echocardiography.

Faulty incorporation of the common pulmonary vein leaves it as a distinct structure posteriorly, into which the pulmonary veins empty. This "chamber" is separated from the anterior "fetal" left atrium (containing the left atrial appendage and communicating with the mitral valve) by a diaphragm, and is known as cor triatriatum, one of the rarest of cardiac malformations. Less pronounced but still incomplete regression of this vein would result in the persistence of a portion of the common pulmonary vein appearing as a mass along the lateral wall of the left atrium at the junction of the left at

Supravalvular stenosing ring of the left atrium.

Two patients with supravalvular stenosing ring of the left atrium are described. In 1 patient with an associated ventricular septal defect and Wolff-Parkinson-White syndrome, the diagnosis of supravalvular stenosing ring was only suspected. This patient underwent correction but died 34 days after the operation because of pulmonary embolism. In the second patient a preoperative diagnosis was not made, and this contributed to his death following correction of tetralogy of Fallot. The association of these two anomalies is very rare. Differential diagnosis from other congenital anomalies producing

Beware the pregnant woman with breathlessness.

BACKGROUND: Randomised controlled trials often rely on questionnaires for follow-up. OBJECTIVE: To compare response rates to an online and postal 12-month follow-up questionnaire on sexual health in female students who took part in a chlamydia screening trial. METHODS: 1329 sexually active female students aged 16-27 were recruited from 12 universities and further education (FE) colleges. The 299 participants recruited between September 2004 and February 2005 were sent a postal questionnaire after 12 months. The 1030 participants recruited between March and December 2005 were contacted by ema

Divided left atrium associated with supravalvar mitral ring.

Reported is a case with a rare association of divided left atrium, supramitral stenosing ring of the left atrium, connection of the left superior caval vein to the roof of the left atrium, unroofed coronary sinus with an interatrial communication at the mouth of the unroofed sinus and ventricular septal defect. The need for a complete echocardiographic examination in the presence of pulmonary venous obstruction is emphasized. Surgery was successful in spite of significant preoperative pulmonary hypertension.
