Endolymphatic hydrops is the hystopathological substrate characteristic of Ménière's disease. Besides the classical treatment with diuretics and/or osmotic drugs for some time, now treatment in a "pressure chamber" (OTI) has also been applied. The oxygen administered in the hyperbaric chamber can reduce the hydrops both by increasing the hydrostatic pressure and by mechanically stimulating the flow of endolymph toward the duct and endolymphatic sac. In addition, an increase is seen in the amount of O2 dissolved in the labyrinthine fluids and this contributes to recovering cell metabolism and r

von Hippel-Lindau disease is a hereditary cancer syndrome characterized by the development of vascular tumors of the central nervous system and retina, clear cell renal carcinomas, pheochromocytomas, pancreatic islet cell tumors, endolymphatic sac tumors, and benign cysts affecting a variety of organs. VHL disease is caused by germline mutations of the von Hippel-Lindau tumor suppressor gene located on chromosome 3p25. Tumor development in this setting is due to inactivation or loss of the remaining wild-type allele in a susceptible cell. The highly vascular nature of VHL-associated neoplasms

Preoperative vestibular function was studied in 56 patients undergoing the endolymphatic subarachnoid shunt procedure for Ménière's disease by electronystagmographic recording of bithermal air caloric stimulation. Fifty-two percent had normal vestibular function while 48% had a reduced vestibular response on the affected side using 20% difference as being significant. Best results of surgery with relief of vertigo were found in the group of patients with normal preoperative vestibular function (82%) while the group with reduced vestibular response preoperatively showed 63% with a good result.

Results are reported for 40 patients with disabling vestibular symptoms, treated with endolymphatic sac surgery (ELS), gentamicin injections in the middle ear, or vestibular neurectomy by the middle fossa approach. If ELS failed, an additional treatment with a second ELS procedure or gentamicin instillations was performed. If the gentamicin treatment failed, the patients were treated with a second series of gentamicin instillations or vestibular neurectomy or both. The results in relieving the patients of their vertigo were found to be almost the same with each of the treatment modalities: app

We have now performed the cochlear endolymphatic shunt operation for Meniere's disease on 102 ears and have follow-up data on ninety cases. In this fourth report of results of the procedure, we show that after follow-up ranging from one month to six years (average 22.3 months), definitive vertigo was relieved in 72% of the cases and that hearing at the time of most recent audiogram (average 12.6 months postoperatively) was worse (AAO criteria) in 45% of the cases.

CONCLUSION: Even after the administration of a standard dose of an intravenous gadolinium-based contrast agent (GBCA), visualization of endolymphatic hydrops (ELH) became possible in patients with Meniere's disease. The next step would be to consistently visualize ELH in the upper part of the cochlea.
OBJECTIVE: To visualize ELH after routine administration of an intravenous GBCA.
METHODS: An intravenous GBCA (gadodiamide; 0.2 ml/kg) was administered to three patients with unilateral Meniere's disease and two healthy volunteers. Three-dimensional fluid attenuated inversion recovery (3D-FLAIR

CONCLUSIONS: TSCP, which can reduce vertiginous symptoms in patients with intractable MD, represents an effective therapy for this disorder.
OBJECTIVE: To explore the long-term efficacy of triple semicircular canal plugging (TSCP) in the treatment of intractable Meniere's disease (MD).
METHODS: Seventy-nine patients diagnosed with unilateral MD referred to a vertigo clinic of the hospital between December 2010 and December 2013 were included in this study for retrospective analysis. TSCP was performed in the affected ear for each patient. Vertigo control and auditory function were measured.

von Hippel-Lindau (VHL) disease is an inheritable condition with an incidence of 1 in 36000 live births. Individuals with VHL develop benign and malignant tumors including retinal and central nervous system hemangioblastomas, clear cell renal cell carcinomas (RCC), pheochromocytomas, pancreatic neuroendocrine tumors and endolymphatic sac tumors (ELSTs). VHL is caused by germline loss of function of the VHL gene on one allele at chromosome 3p25-26. A somatic "second hit" event leads to the loss of the other allele and tumor formation. Loss of VHL function in cells leads to increased expression

INTRODUCTION: Congenital malformations and acquired lesions of the inner ear are characterised by small structural changes in this region. In recent decades, treatment options have improved considerably. At the same time, there has been a great advancement in diagnostic methods, obtaining high-resolution labyrinth images. Currently, we use a 64-multislice computed tomography scanner in spiral mode (Brilliance 64 Phillips, Eindhoven, the Netherlands), with an overlap of 0.66 mm and an interval of 0.33 mm, 120 KV and 300 mA. The magnetic resonance images were taken with Signa HDxt 1.5 and 3.0 T

An enlarged vestibular aqueduct (EVA) is one of the most commonly identified inner ear bony malformations in children with sensorineural hearing loss of unknown cause. Most previous reports have focused on hearing loss, but individuals with EVA may also experience paroxysmal vertigo lasting minutes to hours. We report the clinical vestibular features and vestibular function testing of two children and one adult with EVA who had a history of sensorineural hearing loss and presented to our Neurotology Clinic for the evaluation of episodic vertigo. All the patients had an antecedent history of pr

BACKGROUND: Pendred syndrome is often associated with inner ear malformations, especially enlarged vestibular aqueduct (EVA). Recently, mutations in the Pendred syndrome gene (PDS) have been reported in patients with EVA, in addition to those with classical Pendred syndrome.
OBJECTIVE: The aim of this study was to investigate the genotype-phenotype correlations of PDS.
METHODS: Each of the 21 exons and flanking splice regions of PDS was analysed by direct DNA sequencing in nine patients with EVA; allele-specific amplification was performed to confirm the mutation. Genetic analyses were compa

PURPOSE: It has been reported that the inner ear is capable of responding to antigen challenge. In this study, we have investigated the antibody reactivity to herpes simplex virus 1 (HSV-1) proteins in sera from patients with Ménière's disease.
PATIENTS AND MATERIALS: Serum from 21 patients scheduled to undergo endolymphatic sac decompression for Ménière's disease was obtained. The sera from 21 age- and sex-matched individuals without a history of ear disease served as the control. An enzyme-linked immunosorbent assay (ELISA) was used to detect antibodies to HSV-1, HSV-2 cytomegalovirus (CMV)

Gitelman syndrome (GS) is a rare salt-losing tubulopathy caused by an inactivating mutation in the SLC12A3 gene, encoding the thiazide-sensitive sodium chloride cotransporter (NCC). Patients with GS frequently complain of vertigo, usually attributed to hypovolemia. Because NCC is also located in the endolymphatic sac, we hypothesized that patients with GS might have vestibular dysfunction. Between April 2013 and September 2016, 20 (22%) out of 90 patients followed at the reference center complained of vertigo in the absence of orthostatic hypotension. Sixteen of them were referred to an otolog

Von Hippel-Lindau (VHL) is a hereditary multisystem disorder caused by germline alterations in the VHL gene. VHL patients are at risk for benign as well as malignant lesions in multiple organs including kidney, adrenal, pancreas, the central nervous system, retina, endolymphatic sac of the ear, epididymis, and broad ligament. An estimated 30%-35% of all families with VHL inherit a germline deletion of one, two, or all three exons. In this study, we have extensively characterized germline deletions identified in patients from 71 VHL families managed at the National Cancer Institute, including 5
