Lower extremity powered exoskeletons (LEPEs) allow people with spinal cord injury (SCI) to stand and walk. However, the majority of LEPEs walk slowly and users can become fatigued from overuse of forearm crutches, suggesting LEPE design can be enhanced. Virtual prototyping is a cost-effective way of improving design; therefore, this research developed and validated two models that simulate walking with the Bionik Laboratories' ARKE exoskeleton attached to a human musculoskeletal model. The first model was driven by kinematic data from 30 able-bodied participants walking at realistic slow walki

BACKGROUND: Osseointegration has emerged over the past two decades as a dramatically different approach for the treatment of lower limb amputations, which involves direct attachment of the prosthesis to the skeletal residuum. This approach can address many of the socket-interface issues associated with socket prostheses which represent the current standard of care for amputees. The Osseointegrated Prosthetic Limb (OPL) is an osseointegration implant with a new design and improved features compared to other available implant systems.
OBJECTIVES: To report on the experience and outcomes of usin

The majority of commercially available passive prosthetic feet are not capable of providing joint mechanics that match that of the intact human ankle. Due to their cantilever design, their stiffness characteristics contrast with what has been observed in the biological ankle, namely, an increase in stiffness during the stance phase of walking. In this paper, we introduce the design and control of a pneumatic foot-ankle prosthesis that attempts to provide biomimetic mechanics. The prosthesis is comprised of a pneumatic cylinder in series with a fiberglass leaf spring, and a solenoid valve to co

The paper describes a novel unpowered energy-stored exoskeleton (ES-EXO) for spinal cord injured patients in consideration of patients' characteristics and injured levels. It proposed a method to optimize the energy-stored element to decrease the hip joint moment in walking. EMG patterns, ground reaction force and motion data from one participant with complete spinal cord injury at T10 were recorded. A combined human-ES-EXO model was built and the stored-energy element including locations and stiffness of springs were optimized in AnyBody Modeling System. Significant correlations between exper

PURPOSE: To assess physiological metrics during the use of a commercially available bilateral active ankle exoskeleton during a challenging military-relevant task and if use of the exoskeleton during this task influences: metabolic load, physiological measures or rate of perceived exertion.
METHODS: Nine healthy volunteers (5M, 4F) completed this randomized cross-over design trial, with a baseline visit and two randomized test sessions (with/without the exoskeleton). Variables included impact on time to exhaustion during walking on a treadmill at varying speeds and gradients (0-15%) at 26.7°C

INTRODUCTION: Robotic technologies have been developed for motor rehabilitation and such robots have shown favourable results when compared with equivalent doses of usual clinical therapy. Recently, robotic interventions have been suggested to be applied in orthopaedic rehabilitation with upper extremity disorders, especially those related to hand and wrist. This study aims to determine the effectiveness of combined conventional therapy and HAL-SJ robotic therapy in restoring the wrist functionality following the fractures as compared to the standard conventional therapy solely.

The loss of bilateral hand function is a debilitating challenge for millions of individuals that suffered a motor-complete spinal cord injury (SCI). We have recently demonstrated in eight tetraplegic individuals the presence of highly functional spared spinal motor neurons in the extrinsic muscles of the hand that are still capable of generating proportional flexion and extension signals. In this work, we hypothesized that an artificial intelligence (AI) system could automatically learn the spared electromyographic (EMG) patterns that encode the attempted movements of the paralyzed digits. We

BACKGROUND: Robots are increasingly used in later-life care. However, existing reviews often combine applications such as companionship, telepresence, medication support, monitoring, psychosocial support, traditional AI, and rehabilitation within a single body of evidence. This conceptual overlap makes it difficult to interpret the rehabilitation-specific evidence base.
OBJECTIVE: This study aimed to map empirical studies on embodied robots used in rehabilitation-oriented interventions for older adults with disability or cognitive impairment and to clarify the operational boundary between reh

Neurological disorders often cause severe upper limb motor impairments that restrict independence and quality of life. Robot-assisted rehabilitation enables high-intensity, task-oriented, and quantifiable training. One key feature, gravity compensation (GC), reduces the muscular effort needed to lift the limb and supports voluntary movement by offsetting the weight of the arm. This systematic review aimed to identify the types of GC strategies used in upper limb rehabilitation robots and assess clinical evidence on their effectiveness for improving motor outcomes. Individuals with lower-limb amputation experience higher walking energy cost and discomfort than able-bodied controls when using traditional passive prostheses. Powered prosthetics aim to reduce energy cost and improve comfort, but have not shown empirical metabolic benefits approaching those of comparable exoskeletons or simulation studies. One theorized explanation is that amputees adapt less quickly, and less effectively, to walking assistance than able-bodied participants. Previous work has shown that split-belt walking is an effective test of adaptation to an assistance condition, and tha

Optimizing sensors for physiological input is critical to enhance performance as well as minimize the cost and complexity of assistive devices (e.g. lower-limb exoskeletons). Electromyography (EMG) data can trace muscle activation for gait kinematics prediction. However, identifying optimal muscle groups for electrode placement and the potential variance between users has not yet been established. In this study, we use data-driven channel selection techniques on EMG signals to find muscle group combinations that maximize prediction performance. We apply greedy search (Recursive Feature Elimina

Restoring the ability to walk is a priority for individuals with neurological disorders or neurotraumatic injuries, given its significant impact on independence and quality of life. Multimodal closed-loop strategies that integrate robotic assistance and neuromodulation present promising avenues for personalized and physiological gait recovery. These approaches capitalize on residual motor activity, fostering neuroplasticity and motor relearning. This narrative review emphasizes the importance of mobile brain/body imaging (MoBI) for guiding the development of closed-loop systems that integrate

PURPOSE OF REVIEW: The successful motor rehabilitation of stroke, traumatic brain-injured and spinal cord-injured patients requires an intensive and task-specific therapy approach. Budget constraints limit a hand-to-hand therapy approach, so that intelligent machines may offer a solution to promote motor recovery and obtain a better understanding of motor control. This new field of automated or robot-assisted motor rehabilitation has emerged since the 1990s.
RECENT FINDINGS: This article will present clinically viable devices for upper and lower extremity rehabilitation. The MIT-Manus and the

Stroke has been identified as a major public health concern and one of the leading causes contributing to long-term neurological disability. People suffering from stroke often present with upper limb paralysis impacting their quality of life and ability to work. Motor impairments in the upper limb represent the most prevalent symptoms in stroke sufferers. There is a need to develop novel intervention strategies that can be used as stand-alone techniques or combined with current gold standard post-stroke rehabilitation procedures. There was reported evidence about the utility of rehabilitation

In this paper, we propose an experimental technique for studying the sense of agency (SoA) in joint human-machine actions. This technique is based on the use of an electromechanical finger-lifting device that enables a joint motor action initiated by a participant and completed by the machine. The joint action, later referred to as an "active-passive" action, was implemented as a reaction time task and contrasted with other levels of participant's involvement, including active movement, passive movement, and observation of a dummy's movement. In each trial, a feedback sound signal informed the

Pneumatic artificial muscles (PAMs) are one of the key technologies in soft robotics, and they enable actuation in mobile robots, in wearable devices and exoskeletons for assistive and rehabilitative purposes. While they recently showed relevant improvements, they still present quite low payload, limited bandwidth, and lack of repeatability, controllability and robustness. Vacuum-based actuation has been recently demonstrated as a very promising solution, and many challenges are still open, like generating at the same time a large contraction ratio, and a high blocking force with enhanced axia

BACKGROUND: Lower limb exoskeleton rehabilitation robot is a new technology to improve the lower limb motor function of stroke patients. Recovery of motor function after stroke is closely related to neuroplasticity in the motor cortex and associated motor areas. However, few studies investigate how rehabilitation robots affect the neuroplasticity of stroke patients. This study sought to determine the effects of lower limb exoskeleton robot walking training on neuroplasticity and lower limb motor function in patients with stroke.
METHODS: A total of 25 (50.26 ± 11.42 years, 68.0% male) patients

Lower-limb exoskeletons have the potential to transform the way we move1-14, but current state-of-the-art controllers cannot accommodate the rich set of possible human behaviours that range from cyclic and predictable to transitory and unstructured. We introduce a task-agnostic controller that assists the user on the basis of instantaneous estimates of lower-limb biological joint moments from a deep neural network. By estimating both hip and knee moments in-the-loop, our approach provided multi-joint, coordinated assistance through our autonomous, clothing-integrated exoskeleton. When deployed
