Systematic review and meta-analysis of the open abdomen and temporary abdominal closure techniques in non-trauma patients.

BACKGROUND: Several challenging clinical situations in patients with peritonitis can result in an open abdomen (OA) and subsequent temporary abdominal closure (TAC). Indications and treatment choices differ among surgeons. The risk of fistula development and the possibility to achieve delayed fascial closure differ between techniques. The aim of this study was to review the literature on the OA and TAC in peritonitis patients, to analyze indications and to assess delayed fascial closure, enteroatmospheric fistula and mortality rate, overall and per TAC technique. METHODS: Electronic databases

Outcome of small intestinal submucosa graft for repair of anterior urethral strictures.

OBJECTIVES: To investigate the feasibility of small intestinal submucosa graft for the repair of selected anterior urethral strictures. METHODS: From June 2009 to May 2011, 28 men (mean age 39 years) with anterior urethral strictures underwent urethroplasty using a four-layer small intestinal submucosa patch graft in an onlay or inlay fashion. The stricture was localized to the bulbar urethra in eight patients, the bulbopenile area in nine patients and the distal penile urethra in 10 patients. Failed hypospadias was observed in one patient. The mean stricture length was 4.6 cm (range 3.5-7.0

Two-sided bulbar urethroplasty using dorsal plus ventral oral graft: urinary and sexual outcomes of a new technique.

PURPOSE: Repair of bulbar strictures using anastomotic techniques may cause sexual complications, while 1-side graft urethroplasties may not be sufficient to provide an adequate lumen in narrow strictures. We evaluated the urinary and sexual results of a 2-sided dorsal plus ventral graft urethroplasty by preserving the narrow urethral plate in tight strictures. MATERIALS AND METHODS: Between 2002 and 2010, 105 men with bulbar strictures underwent dorsal plus ventral graft urethroplasty. The results are reported in a homogeneous group of 73 of 105 cases in which buccal mucosa was used as a gra

Long-term results of arthrodesis for osteoarthritis of the ankle.

A bone graft in the shape of a triangular prism was taken from the anterior surface of the tibia and used in the arthrodesis in 43 joints of 42 patients with primary and secondary osteoarthritis of the ankle. The modified anterior sliding inlay graft method was used. The average period of external immobilization was 5.8 weeks (range, 27-84 days), and followup ranged from 2 years 4 months to 14 years 11 months (average, 7 years 2 months). Nonunion was detected in three (7%) patients: one patient returned to work without additional treatment, and the other two patients underwent followup surgery

Conversion of Patello-femoral Arthroplasty to Total Knee Arthroplasty: Survivorship, Function, and Predictors of Patellar Revision.

BACKGROUND: Patello-femoral arthroplasty (PFA) is used to treat isolated patello-femoral osteoarthritis, but progression of tibio-femoral disease or implant failure may require conversion to total knee arthroplasty (TKA). Data describing outcomes after conversion and factors associated with patellar component management at conversion are limited. We evaluated survivorship, radiographic outcomes, and clinical outcomes after conversion of PFA to TKA and assessed whether preconversion patellar alignment was associated with patellar component revision at conversion. METHODS: We retrospectively id

Endonasal Endoscopic Management of Cerebrospinal Fluid Rhinorrhea: A Study of 263 Patients.

BACKGROUND: Cerebrospinal fluid (CSF) rhinorrhea is a sign of a breach in the bony skull base. It should be verified, localized, and repaired surgically to alleviate the risk of intracranial infection. Endonasal endoscopic surgery is the standard technique for skull base reconstruction in these patients. The current study was undertaken to evaluate a large case series of patients with CSF rhinorrhea who underwent surgery, focusing on symptoms, etiology, specifics of skull base defects, reconstruction techniques, outcomes, and complications. METHODS: All patients with CSF rhinorrhea who were t

Multilevel Analysis of the Neovascularization and Integration Process of a Nonvascularized Rectus Fascia Transplantation.

BACKGROUND: Failure to close the abdominal wall after intestinal transplantation (ITx) or multivisceral Tx remains a surgical challenge. An attractive method is the use of nonvascularized rectus fascia (NVRF) in which both layers of the donor abdominal rectus fascia are used as an inlay patch without vascular anastomosis. How this graft integrates over time remains unknown. The study aims to provide a multilevel analysis of the neovascularization and integration process of the NVRF. METHODS: Three NVRF-Tx were performed after ITx. Clinical, radiological, histological, and immunological data w

Similar migration for medial congruent and cruciate-retaining tibial components in an anatomic TKA system: a randomized controlled trial of 60 patients followed with RSA for 2 years.

Background and purpose - There is an ongoing debate regarding the appropriate level of constraint in primary TKA. To provide increased stability and better kinematics, more resembling a natural knee, a medial congruent (MC) tibial insert has been introduced. To investigate potential impact on implant migration, we evaluated the cemented Persona TKA, randomizing between MC and cruciate-retaining (CR) tibial designs. Patients and methods - 60 patients with primary osteoarthritis were randomized to either the CR or MC tibial component. Radiostereometric analysis (RSA) examinations, evaluating tra

Double-face Augmentation Urethroplasty for Bulbar Urethral Strictures: Technical Implications and Short-term Outcomes for a Dorsal Versus Ventral Approach.

UNLABELLED: This study describes technical implications and compares short-term outcomes after a dorsal versus ventral approach for double-face augmentation urethroplasty (DFAU) for treating a near-obliterated bulbar urethral stricture (BUS). This was a retrospective evaluation of a prospectively collected database of patients with BUS (<2 cm) who underwent DFAU. The choice between the approaches depended on (1) landmark identification (the relation between the bulbospongiosus muscle and the distal end of the stricture) and (2) corpus spongiosum width. In DFAU, inlay augmentation was at the le

Surgical management of infected abdominal wall mesh: an analysis using the American Hernia Society Quality Collaborative.

INTRODUCTION: Several management strategies exist for the treatment of infected abdominal mesh. Using the American Hernia Society Quality Collaborative, we examined management patterns and 30-day outcomes of infected mesh removal with concomitant incisional hernia repair. METHODS: All patients undergoing incisional hernia repair with removal of infected mesh were identified. A complete repair (CR) was defined as fascial closure with mesh; a partial repair (PR) was defined as fascial closure without mesh or no fascial closure with mesh. A two-tailed p value less than or equal to 0.05 was consi

The Plane of Mesh Placement Does Not Impact Abdominal Donor Site Complications in Microsurgical Breast Reconstruction.

BACKGROUND: Reinforcement of the abdominal wall with synthetic mesh in autologous breast reconstruction using abdominal free tissue transfer decreases the risk of bulging and herniation. However, the impact of the plane of mesh placement on donor site complications has not yet been investigated. METHODS: We performed a retrospective analysis of 312 patients who had undergone autologous breast reconstruction with muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) flaps or deep inferior epigastric perforator (DIEP) flaps as well as polypropylene mesh implantation at the donor sit

[Revision TKA due to instability: diagnostics, treatment options and outcomes].

BACKGROUND: Instability after primary TKA is a frequent reason for revision surgery. Other mechanisms of failure must be ruled out before an in-depth analysis of instability. DIAGNOSTICS: Diagnostic tools for instability consist of medical history, clinical examination, and imaging. The clinical examination must focus primarily on the extent of the instability, the location of the instability and the levels of instability. Varus and valgus stress radiographs in the mediolateral plane in extension and flexion, as well as anteroposterior stress images (drawer) are mandatory. In addition, the un
