Does the use of soft or hard splints affect the short-term outcome of temporomandibular joint arthrocentesis?

Arthrocentesis is an effective treatment modality for temporomandibular joint (TMJ) disc displacement without reduction (DDw/oR), especially in patients who do not respond to non-surgical methods, and is nowadays regarded as a first-line treatment. Soft and hard stabilizing splints have also been used to treat TMJ disorders, but no data are available regarding the use of splints following arthrocentesis. The aim of this study was to compare the effect of the use of soft or hard stabilizing splints versus no appliance following TMJ arthrocentesis on the prognosis of the treatment. Forty-five pa

Otologic symptom improvement through TMD therapy.

OBJECTIVES: Some patients with a temporomandibular disorder (TMD) and coexisting otologic symptoms desire to know the probability of TMD therapy improving their otologic symptoms. The aim of this study was to determine a clinically valid method for identifying which otologic symptoms have a high probability of improving as a result of satisfactory TMD symptom improvement. METHOD AND MATERIALS: Two hundred TMD patients with coexisting tinnitus, otalgia, dizziness, and/or vertigo were asked about their otologic symptom characteristics and associations and were given clinical tests, which were s

Anterior mandibular repositioning in a patient with temporomandibular disorders: a clinical and tomographic follow-up case report.

A 42 year old female with temporomandibular disorders (TMD) was treated by anterior mandibular repositioning which was followed-up clinically and tomographically. The authors tomographically reconfirmed the mandibular repositioning and discovered a type of condyle remodeling which they had not seen previously. Although clinical signs and symptoms of TMD were removed and the condyle was centered tomographically by the anterior repositioning, the MRI image indicated the disk was displaced anteriorly and laterally. The results suggest that image analysis of temporomandibular joint (TMJ) is benefi

Initial TMJ disk recapture with anterior repositioning appliances and relation to dental history.

Fifty-eight consecutive patients in a referral based practice seeking treatment for complex chronic painful temporomandibular joint (TMJ) disease were enrolled in a prospective study to assess the recapture of displaced disks by anterior repositioning appliances (ARA) and the improvement in disk position in those disks that did not fully recapture. After standard clinical workup, including assessment of pain, maxillary and mandibular ARAs were constructed which repositioned condyles to the Gelb 4/7 position as determined by cephalometrically-corrected linear tomograms. Maintenance of condyle-proximal segment position in orthognathic surgery.

Twenty patients underwent bilateral sagittal ramus osteotomy for the correction of mandibular retrognathia. A condylar positioning device (CPD) was used intraoperatively in 10 patients to maintain preoperative condyle-proximal segment position, while the CPD was not used in the other 10 patients. Postoperatively, the condyle-proximal segment positions in both groups were compared and evaluated for vertical, horizontal, and rotational changes. A significant improvement (P less than .05) was observed in the vertical and horizontal condylar position in the group in which the CPD was used. However

Three-dimensional changes in the condyle during development of an asymmetrical mandible in a rat: a microcomputed tomography study.

A rapidly growing postnatal animal model was used to study changes in the calcified tissue of the mandibular condyle during altered muscle function. A maxillary occlusal splint was designed to shift the mandible laterally (left) during closure. Groups of 5 Wistar rats were killed at 5, 9, 15, 21, 30, and 40 weeks (n = 30), with an equal number of controls. The experimental animals developed shorter, asymmetrical mandibles compared with the control animals. The left condyle became larger and thicker than the right condyle. Microcomputed tomography assessment of the left and right condylar trabe

Efficacy of a digitally designed anatomical repositioning splint for adolescent temporomandibular joint anterior disc displacement with reduction: a randomized controlled trial.

BACKGROUND: Anterior disc displacement with reduction (ADDwR) is a common temporomandibular joint disorder in adolescents. Conventional anterior repositioning splints (ARS) often lack anatomical adaptation, which may limit their therapeutic precision and patient comfort. This study aimed to compare the efficacy of a digitally designed anatomical repositioning splint (D-ARS) with that of a conventional ARS in adolescent ADDwR patients. METHODS: In this randomized controlled trial, 82 adolescents (aged 12-16 years) with ADDwR were randomly allocated to receive either a D-ARS (n = 41) or an ARS

Dose-dependent effects of mandibular advancement on optimal positive airway pressure requirements in obstructive sleep apnoea.

BACKGROUND: Implementation of mandibular advancement splint (MAS) therapy as first-line treatment for obstructive sleep apnoea (OSA) is hindered by inter-individual variability of treatment outcomes and lack of robust patient selection methods. Optimal continuous positive airway pressure (CPAP) requirement provides an estimate of airway collapsibility severity, and high CPAP requirements predict MAS therapy failure in retrospective studies. Thus, understanding the effects of mandibular advancement on optimal CPAP requirements may enhance optimisation of patient selection for MAS therapy. OBJE

Efficacy of stabilisation splint therapy combined with non-splint multimodal therapy for treating RDC/TMD axis I patients: a randomised controlled trial.

Stabilisation splint therapy has long been thought to be effective for the management of temporomandibular disorders (TMD). However, the superiority of stabilisation splint therapy compared to other TMD treatments remains controversial. The aim of this study was to determine the efficacy of stabilisation splint therapy combined with non-splint multimodal therapy for TMD. A total of 181 TMD participants were randomly allocated to a non-splint multimodal therapy (NS) group (n = 85) or a non-splint multimodal therapy plus stabilisation splint (NS+S) group (n = 96). Therapy of the osteoarthritis of the temporomandibular joint.

Osteoarthritis is a common condition of the mandible. The treatment of osteoarthritis primarily consists of rest therapy (restricting jaw movements), the use of pharmaceuticals (analgesics, antiinflammatories), splint therapy, thermotherapy or mini-invasive therapy. Our study investigated the effectiveness of the various therapeutic options in the treatment of osteoarthritis. We compared the effectiveness of rest therapy (restricting mouth opening, analgesic therapy), splints, arthrocentesis of the upper joint space, and arthrocentesis in combination with splint therapy. We looked at 80 patien

Resilient appliance therapy of temporomandibular disorders. Subdiagnoses, sense of coherence and treatment outcome.

Temporomandibular disorders (TMD) with orofacial pain with or without reduced jaw function, are frequent conditions in the general population. Different factors such as tooth clenching and grinding, sometimes due to enhanced psychosocial stress, and trauma to the jaws may be important as etiologic factors. Signs and symptoms of TMD are a common cause for general practitioners to use different intraoral appliances as pain and bite-force reducing devices and for improvement of a reduced jaw function. Intraoral appliances are often used parallel to other treatment modalities. Before treatment sta
