Clinical Role of the "Venous" Ultrasound to Identify Lower Extremity Pathology.

BACKGROUND: Accreditation in peripheral venous testing can be obtained based on femoropopliteal (FP) duplex ultrasound evaluation, and many laboratories limit their examination to this segment only. This simplified protocol detects acute FP deep venous thrombosis (DVT) but misses calf vein DVT, superficial venous thrombosis, chronic DVT, venous reflux, and other nonvenous findings potentially responsible for the patients' presenting conditions. A protocol limited to the FP segment results in additional unnecessary testing and can create patient dissatisfaction. We evaluated the differences in

[Ultrasonography of the hip, knee and ankle].

Hip ultrasonography (US), besides visualization of anatomic structures, provides precise puncture and aspiration of the joint content. The most often US findings in inflammatory rheumatic diseases are intra-articular effusion (pathologic finding>2 mm), hypertrophy ofsynovial membrane, cartilage damage and bone erosions. US characteristics ofosteoarthritis (OA) are small amount of effusion, lost of cartilage, osteophytes on the articular edges and possible mild hypertrophy of synovial membrane. Recently, US has been used in diagnostics of many knee disorders, including lesions of articular cart

Systematic review of MRI, ultrasound, and scintigraphy as outcome measures for structural pathology in interventional therapeutic studies of knee arthritis: focus on responsiveness.

OBJECTIVE: validated imaging outcome tools to assess response to therapies in a single joint are required. Our aim was to review the published literature to ascertain the responsiveness of novel imaging techniques as outcome measures in interventional therapeutic studies of knee arthritis. METHODS: an Ovid Medline search was performed for original articles in English that used imaging techniques to assess response at the knee joint to therapy in osteoarthritis, rheumatoid arthritis, and psoriatic arthritis. Changes in response to therapy were assessed with regard to both internal and external

Correlation between clinical and ultrasound assessment of the knee in children with mono-articular or pauci-articular juvenile rheumatoid arthritis.

BACKGROUND: Ultrasonography of the knee is a non-invasive, readily available and low-cost tool for demonstrating peri-articular tissues. OBJECTIVE: To correlate clinical features with US findings in the detection, quantification and follow-up of inflammatory signs of the knee in children with pauci-articular juvenile rheumatoid arthritis (JRA). MATERIALS AND METHODS: US of both knees was performed in 49 patients on the same day as the clinical examination. All joints were classified into two groups by clinical criteria: group A (active disease) or group B (quiescent disease). Thirteen patien

Pyogenic Baker cyst in a patient with psoriatic arthritis during etanercept therapy.

Patients with psoriatic arthritis (PsA) are at increased risk of infection due to disease course or treatment-related immunodeficiency. We describe a patient with a 10-year history of PsA, with arthritis of the right knee and pain and edema in the right calf, treated with the TNFa inhibitor etanercept for 6 months. Ultrasound showed accumulation of hypoechoic fluid, which was aspirated and was positive for staphylococcus and aspergillus. The patient recovered after surgical drainage and four weeks of antibiotic and antifungal therapy.

Different approaches in the treatment of cystic adventitial disease of the popliteal artery.

Three cases of cystic adventitial disease (CDA) of the popliteal artery and the results of a literature review are described in order to identify the treatment of first choice of this rare clinical entity. Three male patients (mean age 56,3 years) presented at our Institution due to a sudden abrupt of disabling claudication of the lower limb. The eco-color-Doppler examination showed multiple hypoechoic and anechoic masses arising in the wall of the artery consistent with CAD. The magnetic resonance imaging (MRI) confirmed this finding. In 2 cases a venous interposition grafting was performed a

Epstein-Barr (EB) monoarthritis leading to ruptured Baker's cyst.

We report a case of Epstein-Barr (EB) infection that presented as an acute monoarthritis of the knee. This formed a Baker's cyst which ruptured into the gastrocnemius. The peripheral and synovial white blood counts were dominated by neutrophils. After repeated aspirations, spontaneous resolution occurred.

Ultrasound of the knee.

Ultrasound is emerging as a viable imaging modality in the diagnosis and assessment of the musculoskeletal system. Advantages of ultrasound include its easy availability and multiplanar capability, as well as economic advantages. Unlike magnetic resonance imaging, ultrasound demonstrates the fibrillar microanatomy of tendons, ligaments and muscles, enhancing its diagnostic capability. The ability to compress. dynamically assess structures and compare easily with the contralateral side is advantageous. The patient's exact point of clinical tenderness can be correlated with underlying anatomical

Compressive neuropathy of the tibial nerve and peroneal nerve by a Baker's cyst: case report.

We report a case of Baker's cyst that induced compression of both the tibial and common peroneal nerves. The patient presented with calf atrophy and foot drop over a 6-month period. These signs and symptoms could have been mistaken for those of spinal origin. Based on an electrodiagnostic study and magnetic resonance imaging, compression of nerves by an asymptomatic Baker's cyst measuring 6x4 cm was confirmed. This cyst communicated with the articular joint which was also associated with a medial meniscal lesion. We treated the patient arthroscopically by performing partial medial meniscectomy

Posterior dislocation of meniscal bearing insert in mobile bearing unicompartmental knee arthroplasty mimicking a Baker cyst.

Unicompartmental knee arthroplasty (UKA) is a well-established procedure for isolated medial compartment osteoarthritis. Meniscal dislocation is a well-recognized complication in mobile bearing UKA especially when performed for the lateral compartment. Dislocation in medial UKA is relatively uncommon and is usually anterior when encountered. We would like to report a case of posterior dislocation of the bearing insert in a mobile bearing medial UKA presenting as a Baker cyst, which was subsequently revised to total knee arthroplasty.

[Results of diagnostic sonography in injuries of the knee joint].

In an experimental study we examined three knee specimens with regard to the value of ultrasound for imaging soft tissue structures. Afterwards we documented artificial lesions of the menisci and ligaments. With this experience we started a prospective study of 101 patients suffering from acute knee injuries or chronic knee complaints. We compared the results of preoperative ultrasound investigation done by two different examiners with the final diagnosis after arthroscopy. For ultrasound evaluation we used a 5 MHz linear and a 7.5 MHz sector scanner. In both the experimental and the clinical

Ultrasonography findings in knee osteoarthritis: a prospective observational cross-sectional study of 100 patients.

Worldwide, knee osteoarthritis (KOA) accounts for 2.2% of total years lived with disability. There is a low correlation between joint tissue damage and pain intensity. Periarticular structures may be involved and cannot be identified in X-rays. To describe the main ultrasonography (USG) changes in symptomatic patients with primary KOA; to correlate the number of USG findings with KOA severity assessed by Kellgren and Lawrence (K&L) radiological scores, with pain intensity measured by a visual analogue scale (VAS) and with functioning scores assessed with the Timed up and go test (TUG) and West

[Villonodular synovitis of the knee: about 20 cases].

Pigmented villonodular synovitis (PVNS) is a rare benign proliferation of synovial joints, serous bursa, tendinous sheaths of unknown etiopathogeny. We here report 20 cases of PVNS of the knee recorded at the Avicenne Military Hospital, Marrakech over a period of 9 years, from January 2000 to December 2009. This study aimed to identify the specific features of this lesion and to examine its anatomoclinic and prognostic aspects. Annual incidence was 2.2 cases per year: 15 men and 5 women. The average age was 32.5 years. It occurred in the right-hand in 55%, 18 patients had monoarticular present
