Three-dimensional volumetric analysis of condylar head and glenoid cavity after mandibular advancement.

The aim of this study was to assess condylar resorption, spatial change in glenoid cavity, and its risk factors after mandibular advancement by three-dimensional volumetric analysis. Subjects consisted of 30 condyles of 15 patients diagnosed with mandibular retrognathism who underwent Le Fort I and bilateral sagittal split ramus osteotomy advancement. CBCT images were taken before surgery (T0), immediately after surgery (T1), and postoperatively at 6 months (T2) and 1 year (T3). Condylar resorption was observed in 21 condyles. The posterior was the most affected region, while the anterior was

Effects of fixed functional appliance treatment on the temporomandibular joint.

INTRODUCTION: In this study, we tested the hypothesis that fixed functional appliance treatment in a group of Class II Division 1 patients with mandibular retrusion changes the condyle position in the glenoid fossa. METHODS: Transverse computed tomography images were taken of the temporomandibular joint region in 60 children with Class II Division 1 malocclusion. Thirty randomly selected patients were treated with a fixed functional orthodontic appliance (Forsus nitinol flat-spring, 3M Unitek Corp, Monrovia, Calif) for 7 months; another 30 patients without treatment were used as controls. [Is condylar resorption a contra-indication for surgery?].

INTRODUCTION: Idiopathic condylar resorption, often fortuitously discovered when performing orthodontic-surgical assessment, is a clinical entity in itself. There is a significant risk of postoperative degenerating and worsening. Is this sufficient to contra-indicate orthognathic surgery? The authors suggest an answer after studying a series of patients. MATERIAL AND METHOD: A retrospective study of 10 consecutive patients was made from July 1999 to October 2004. The preoperative assessment was triple: clinical, radiological and photographic. At least 6 months of occlusal, radiological and mo

[Nager syndrome].

In this publication, Nager syndrome was analyzed in the literature and six patients from our clinic were evaluated in relation to symptoms, etiology and pathogenesis. The diseases to be considered when making a differential diagnosis are pointed out. Clarification of the etiology is still pending. Molecular genetic research in these patients is possibly the key for new findings. A case report illustrates the results of interdisciplinary treatment by the surgeon and orthodontist. Possibilities and problems in relation to therapy are demonstrated.

Predicting difficult laryngoscopy via three-dimensional graphical modeling and hybrid artificial intelligence using standard facial photographs in elective surgery patients.

BACKGROUND: Unexpected difficult laryngoscopy remains one of the leading causes of life-threatening complications in anesthetic practice. Conventional airway assessment tests (e.g., Mallampati classification, thyromental distance) are inherently subjective and demonstrate limited diagnostic accuracy. We aimed to develop a hybrid artificial intelligence model that extracts three-dimensional geometric ratios from standardized facial photographs using deep learning and graph-based modeling to predict difficult laryngoscopy. METHODS: In this prospective observational study, consecutive patients s

Finnish-Enriched SLC26A7 Variant in Congenital Hypothyroidism: Clinical Spectrum, Thyroid Histopathology, and Expression Analysis.

BACKGROUND: Defects in thyroid hormone synthesis at birth lead to congenital hypothyroidism (CH). Recently, pathogenic variants in the SLC26A7 gene have been linked to dyshormonogenetic goitrous CH. This anion transporter is highly expressed in the thyroid and is involved in thyroid hormone synthesis; however, its exact function and cellular localization remain unclear. In this study, we investigated SLC26A7 variants in Finnish patients with CH, characterized the phenotypes, and analyzed thyroid-specific gene expression. METHODS: SLC26A7 variants were identified from a clinical CH cohort (n =

Does Cranial Base Angle Make a Difference in the Effectiveness of Functional Orthopedic Treatment? A Retrospective Cohort Study.

Background/Objectives: The literature suggests that the cranial base angle is considered one of the contributing factors to sagittal jaw malpositions when its relationship with the viscerocranium is examined. Our study aims to compare and evaluate the outcomes of fixed functional orthopedic treatment in patients with mandibular retrognathia across different cranial base groups. Methods: Participants were treated at Erciyes University with fixed functional appliances and categorized by CBA into low (<130°), medium (130°-134°), and high (>134°) groups. A total of 39 patients were included: 13 in

Sleep fragmentation and daytime vigilance in patients with OSA treated by surgical maxillomandibular advancement compared to CPAP therapy.

Impaired vigilance is a frequent daytime complaint of patients with obstructive sleep apnoea (OSA). To date, continuous positive airway pressure (CPAP) is a well established therapy for OSA. Nevertheless, in patients with certain craniofacial characteristics, maxillomandibular advancement osteotomy (MMO) is a promising surgical treatment. Twenty-four male patients with OSA (pretreatment respiratory disturbance index (RDI) 59.3 SD +/- 24.1 events/h) participated in this investigation. The mean age was 42.7 +/- 10.7 years and the mean body mass index was 26.7 +/- 2.9 kg/m2. According to cephalom

Assessing the influence of chin prominence on perceived attractiveness in the orthognathic patient, clinician and layperson.

The purpose of this investigation was to undertake an objective and quantitative evaluation of how the degree of chin prominence influences perceived attractiveness. The chin prominence of an idealized profile image was altered in 2 mm increments from -24 to 12 mm, in order to represent retrusion and protrusion of the chin, respectively. These images were rated on a 7-point Likert scale by a pre-selected group of pre-treatment orthognathic patients, clinicians and laypeople. In treatment planning to alter the sagittal prominence of the chin in an individual with an otherwise normal soft tissue

Subjective sensory symptoms associated with axonal and demyelinating nerve injuries after mandibular sagittal split osteotomy.

PURPOSE: The effect of the type of nerve injury on subjective sensory disturbances and recovery has not been addressed in orthognathic surgery. Using neurophysiologic monitoring during 19 bilateral sagittal split osteotomy operations, we were able to classify intraoperative inferior alveolar nerve injuries as either axonal or demyelinating. This study aimed to analyze the quality and extent of the subjective sensations experienced by the patients after these 2 injury types at different time points up to 12 months. MATERIALS AND METHODS: Of the 36 injured nerves, 21 showed signs of demyelinati

Craniofacial surgical management of a patient with systematic juvenile idiopathic arthritis and Crohn's disease.

Juvenile idiopathic arthritis (JIA) is the most common chronic inflammatory disease in early age. It affects one or more joints, lasts more than 3 weeks, and appears in patients younger than 16 years. Juvenile idiopathic arthritis is classified according to the International League of Associations for Rheumatology consensus depending on the number of affected joints in the beginning of the disease. When JIA affects the temporomandibular joint, the development of the mandible is constrained. Patients show a tendency toward retrognathism and a vertical facial development pattern. The purpose of
