Cholelithiasis and aortic reconstruction: the problem of simultaneous surgical therapy. Conclusions from a personal series.

From 1976 to 1983, 682 patients have undergone aortic reconstruction at Parkland Memorial Hospital and the Veterans Administration Hospital in Dallas, Texas. Thirty-five patients (5.1%) had a biliary tract operation performed before, during, or after their aortic procedure. Fourteen percent of patients had bacteria in the bile and 11.4% needed common bile duct exploration. Twelve patients had their aortic reconstruction first. Biliary pancreatitis developed postoperatively in one patient. Two patients who had infected prostheses removed had acalculous cholecystitis after operation and one had

Long-Term Outcomes of Combined Cystic Duct Embolization and Chemical Gallbladder Ablation in High-Risk Patients with Acute Cholecystitis.

PURPOSE: To evaluate long-term effectiveness, safety, and recurrence rates of combined cystic duct embolization (CDE) and chemical gallbladder ablation (CGA) in high-risk patients with acute cholecystitis (AC) unsuitable for surgery. MATERIALS AND METHODS: In this retrospective cohort study (2013-2025), 19 high-risk patients (mean age, 77.7 years [SD ± 19.3]) underwent CDE followed by CGA using 3% Aethoxysklerol or Glubran with ethiodized oil. All patients initially underwent percutaneous cholecystostomy. Technical failure was defined as the inability to catheterize the cystic duct. Clinical

Laparoscopic Subserosal Cholecystectomy: A Safe and Effective Bail-out Strategy for Difficult Acute Cholecystectomy-A Technical Description.

Difficult acute cholecystitis frequently precludes attaining the critical view of safety during minimally invasive cholecystectomy. Established bail-out options-subtotal cholecystectomy (fenestrating/reconstituting), cholecystostomy drainage with delayed cholecystectomy, or conversion to open-remain widely accepted with distinct limitations. We present an operative framework and a technical description of subserosal cholecystectomy (SSC), supported by an operative video. The technique dissects the edematous, avascular subserosal plane between the outer fibrofatty serosal layer and the inner per

Outcomes of Percutaneous Cystic Duct Stent Placement for Acute Cholecystitis.

PURPOSE: To evaluate whether PCDS placement is safe and effective for nonsurgical patients with acute cholecystitis and cystic duct (CD) anatomy predicts technical success. MATERIALS AND METHODS: Retrospective analysis of 118 consecutive patients who underwent PCDS (double pigtail plastic stent) attempts after percutaneous cholecystostomy tube (PCT) placement between September 2018 and September 2024 was performed. The primary endpoint was technical success, defined as successful deployment of a CD stent. Secondary endpoints included clinical success (symptom resolution without recurrence), a

Laparoscopic subtotal cholecystectomy outcomes across a low-and middle-income country metropolitan health service.

BACKGROUND: Laparoscopic subtotal cholecystectomy (LSC) is a safe alternative for difficult cholecystectomies to prevent bile duct injury and open conversion. The primary aim was to detail the use and outcomes on LSCs. METHODS: Retrospective analysis of a prospectively maintained database of laparoscopic cholecystectomy (LC). Relative clinical factors, outcomes, and 30-day follow-up between LSC and LC were compared using univariate and multivariate analyses. RESULTS: Six hundred and twenty four cholecystectomies were performed and 53 (8.5%) required LSC. 81.8% were fenestrating LSC. Male sex

Unveiling of Type B Lactic Acidosis From Systemic Lupus Erythematosus-Associated B-cell Lymphoma: A Fatal Oncology Emergency.

Type B lactic acidosis is a rare oncological emergency usually associated with leukemia and lymphoma but also with solid malignancies. It can often go unrecognized as a possible source of lactic acidosis, leading to a delay in treatment. We review a 56-year-old woman with systemic lupus erythematosus and generalized lymphadenopathy being evaluated for underlying malignancy who presented with dyspnea, fatigue, and hematemesis. The patient was hemodynamically unstable and had severe lactic acidosis, leukocytosis, electrolyte derangements, multiple organ damage, and worsening diffuse lymphadenopa

Ten-Year Trends of Persistent Mortality With Gallstone Disease: A Retrospective Cohort Study in New Jersey.

BACKGROUND AND AIMS: Recent trends in mortality with gallstone disease remain scarce in the United States. Yet multiple changes in clinical management, such as rates of endoscopy, cholecystectomy, and cholecystostomy, and insurance access at the state level, may have occurred. Thus, we evaluated recent secular trends of mortality with gallstone disease in New Jersey. METHODS: We performed a retrospective, cohort study of mortality from 2009 to 2018 using the National Center for Health Statistics, Restricted Mortality Files. The primary outcome was any death with an International Classificatio

Novel advances in surgery for Gallstone Disease.

PURPOSE OF REVIEW: Examine recent advances in the treatment of patients with complex gallstone disease. RECENT FINDINGS: Laparoscopic common bile duct exploration (LCBDE) has been shown to be an effective and safe treatment for choledocholithiasis, resulting in decreased hospital length of stay and costs when compared with ERCP plus laparoscopic cholecystectomy (LC). Novel simulator-based curricula have recently been developed to address the educational gap that has resulted in an underutilization of LCBDE. Patients with cholecystitis who are too ill to safely undergo LC have traditionally be

Setting up a surgical complex gallstone service in a non-HPB unit.

OBJECTIVES: Complex gallstone disease is associated with a higher risk of complication during laparoscopic cholecystectomy than biliary colic and simple cholecystitis. It is traditionally managed in a hepatopancreaticobiliary (HPB) unit where there is expertise for common bile duct exploration and repair. We developed a mentorship scheme for a busy upper gastro-intestinal (UGI) unit, with support from a specialist HPB unit to treat complex gallstone disease, to reduce the burden on the HPB unit and enable local treatment of patients. MATERIAL AND METHODS: Through the creation of a service lev

Robotic Subtotal Cholecystectomy in a Geriatric Acute Care Surgery Patient with Super Obesity.

Introduction. Unique challenges exist with conventional laparoscopic operations in patients with super obesity (BMI > 50). Limited literature is available regarding use of the robotic platform to treat patients with super obesity or acute care surgery patients. This case describes an interval robotic subtotal cholecystectomy in an elderly patient with super obesity and multiple comorbidities. Case Description. A 74-year-old male with a BMI of 59.9 developed acute cholecystitis. He was deemed excessively high risk for operative intervention due to concurrent comorbid conditions and underwent pe

Comparison and Selection of Three Methods of Minimally Invasive External Drainage for Children with Congenital Choledochal Cysts.

Aim: Emergent biliary drainage is necessary due to acute symptoms of choledochal cysts. Percutaneous biliary drainage (PBD), laparoscopic bile duct drainage (LBD), and laparoscopic cholecystostomy (LC) are the three most common drainage treatments. So far, there is no comparative study on these three approaches, which all have been applied in our hospital. This article compares the drainage effects of these three approaches and illustrates their respective merits and demerits, with the aim of providing a reference for clinical application. Materials and Methods: We conducted a retrospective st

Ten-year Audit of Safe Bail-Out Alternatives to the Critical View of Safety in Laparoscopic Cholecystectomy.

BACKGROUND: To prevent vasculobiliary injuries according to the Tokyo Guidelines, Critical View of Safety (CVS) is the recommended method for the identification of the cystic duct and cystic artery. Our aim was to audit laparoscopic cholecystectomies, in order to determine the rate of CVS feasibility and to explore safe bail-out alternatives, when CVS cannot be obtained. METHODS: Patients who underwent either elective or emergent laparoscopic cholecystectomy, between January 2009 and December 2018, in whom the CVS was attempted, were retrospectively identified from the institutional electroni

Risk factors and therapeutic outcomes of acute acalculous cholecystitis.

OBJECTIVE: Acute acalculous cholecystitis (AAC) is traditionally known to occur in critically ill patients and to have a poor prognosis. Although cholecystectomy is usually recommended for treating AAC, nonsurgical management may be a good alternative. The objective of this study was to review the incidence, risk factors, treatment modality, and therapeutic outcomes of AAC compared to acute calculous cholecystitis (ACC). MATERIAL AND METHODS: Data from 69 patients with AAC and 415 patients with ACC between January 2007 and August 2011 were collected. Analysis and comparison of clinicopatholog

Acute acalculous cholecystitis in malaria: a review of seven cases from an adult cohort.

PURPOSE AND METHODS: Acute acalculous cholecystitis (AAC) is an uncommon condition related to serious clinical conditions, such as surgery, trauma, burn injuries and sepsis. The diagnosis of AAC remains challenging to make, since it generally occurs as a secondary event in acutely ill patients with another disease. Imaging evaluation is crucial, and well-known criteria are accepted for the diagnosis. To our knowledge, only case reports of AAC related to 12 malaria adult patients have been published. In this series, seven cases of AAC from a cohort of 42 adult patients with severe imported falc

Gallbladder stent placement for prevention of cholecystitis in patients receiving covered metal stent for malignant obstructive jaundice: a feasibility study.

PURPOSE: Covered self-expanding metal stents (CSEMS) have been used for palliation of malignant distal biliary strictures. Occlusion of the cystic duct by CSEMS may be complicated by cholecystitis. This potentially could be prevented by placement of a transpapillary gallbladder stent (GBS). PATIENTS AND METHODS: Between 11/2006 and 10/2007, a total of 73 patients (50 male) aged 65 +/- 14 years underwent CSEMS placement for palliation of malignant obstructive jaundice. In cases where CSEMS placement caused occlusion of the cystic duct, a 7 French transpapillary pigtail gallbladder stent (GBS)

The role of extracorporeal shock wave lithotripsy combined with endoscopic management of impacted cystic duct stones in patients with high surgical risk.

BACKGROUND/AIMS: Cholecystoscopic lithotripsy can be an alternative procedure for the treatment of impacted cystic duct stones in patients who are high risk for surgery. Conventional methods, including electrohydraulic lithotripsy (EHL), occasionally fail due to the inability to access or capture the impacted stones in the narrow, spirally long cystic duct. Using extracorporeal shock wave lithotripsy (ESWL) may be more effective to disintegrate cystic duct stones. The aim of the study is to evaluate the role of ESWL in the endoscopic treatment of impacted cystic duct stones in patients with hi
