Mild hypothermia is associated with improved outcomes in patients undergoing microvascular head and neck reconstruction.

OBJECTIVE: Microvascular free tissue transfer has become the standard for reconstruction for large defects. With long operative times and an increased surface area exposed, transient hypothermia is common, but it is unclear how this impacts surgical outcomes. This study evaluated the impact of core body temperature on free tissue flap outcomes in patients undergoing microvascular reconstruction. STUDY DESIGN: Retrospective data analysis. SETTING: Mount Sinai Hospital; NYC, NY; 2007-2016. SUBJECTS AND METHODS: Demographic information, mean/minimum/maximum body temperatures, and the presence

Thoracoscopic Thoracic Duct Ligation for Congenital and Acquired Disease.

PURPOSE: Congenital and acquired chylothorax presents a unique management challenge in neonates and infants. A failure of conservative therapy requires surgical ligation to prevent continued fluid and protein losses. This article examines a 15-year experience with thoracoscopic ligation of the thoracic duct. PATIENTS AND METHODS: From June 1999 to December 2013, 21 patients presented with chylothorax refractory to conservative management. Sixteen patients presented following cardiac procedures, 1 after tracheoesophageal fistula repair, 1 after extracorporeal membrane oxygenation cannulation,

Chylous ascites after hepatopancreatobiliary surgery.

BACKGROUND: Postoperative chylous ascites following abdominal surgery is uncommon. It potentially induces malnutrition and immunodeficiency, contributing to increased mortality. In the field of hepatopancreatobiliary (HPB) surgery, no large studies have been conducted that focused on postoperative chylous ascites. The aim of this study was to determine the incidence, risk factors and management of chylous ascites following HPB surgery, with particular emphasis on pancreatic resection. METHODS: Consecutive patients who had HPB surgery between 2000 and 2011 at a single institution were reviewed

Safety of donor in adult-to-adult living donor liver transplantation using right lobe graft.

BACKGROUND: The growing gap between the number of patients awaiting liver transplantation and available organs has continued to be the primary issue facing the transplant community. To overcome the waiting list mortality, living donor liver transplantation has become an option, in which the greatest concern is the safety of the donor, especially in adult-to-adult living donor liver transplantation (A-A LDLT) using a right lobe liver graft. OBJECTIVE: We evaluated the safety of donors after right lobe liver donation for A-A LDLT performed in our center. METHODS: From January 2002 to March 200

Safety of modified radical neck dissection for differentiated thyroid carcinoma.

OBJECTIVES/HYPOTHESIS: The management of cervical metastases from differentiated thyroid carcinoma (DTC) remains controversial. Most surgeons perform a neck dissection (ND) for clinically apparent disease. The extent of nodal dissection varies from regional to comprehensive. Morbidity from ND in the setting of DTC remains high, particularly when performed in the setting of a thyroidectomy (TT). To determine complications from ND for DTC, we retrospectively reviewed our surgical experience of modified radical neck dissection for nodal metastases. STUDY DESIGN: Retrospective chart review. Intestinal ultrasound in primary intestinal lymphangiectasia (Waldmann's disease): a PRISMA-ScR scoping review with an illustrative adult case.

Primary intestinal lymphangiectasia (PIL; Waldmann's disease, ORPHA 90362) is a rare protein-losing enteropathy in which dilated intestinal lacteals leak chyle into the gut lumen. Cross-sectional imaging and lymphoscintigraphy have historically dominated assessment, while transabdominal B-mode intestinal ultrasound (IUS)-a cornerstone modality for inflammatory bowel disease-has remained almost unmapped in PIL. We conducted a PRISMA-ScR scoping review (Open Science Framework preregistration gfbc9) of all primary studies reporting transabdominal B-mode IUS in PIL, searching three bibliographic p

Surgical complications following pancreaticoduodenectomy in pancreatic neuroendocrine tumors: A systematic review.

BACKGROUND: The management of pancreatic tumors varies depending on the tissue origin, as the tissue type results in numerous differences in the tumor's behavior. Pancreatic neuroendocrine tumors (pNETs) arise from pancreatic islet cells, distinguishing them from pancreatic adenocarcinomas that arise from glandular tissue. Pancreatic neuroendocrine tumors are frequently resected by pancreaticoduodenectomy. This review examines complications of pancreaticoduodenectomy in patients with pancreatic neuroendocrine tumors. METHODS: PubMed, Cochrane, Virtual Health Library, and Web of Science databa

Case Report: Lymphangiogram and embolization for malignant chylothorax in cancer patients.

Tumor-related lymphatic obstruction can cause malignant chylothorax, which can be debilitating. Conventional management includes dietary modifications, percutaneous drainage, and medical management (octreotide), most of which prove refractory in high-output chylothorax cases. Lymphangiogram and embolization in such cases offers a minimally-invasive alternative; however, its use in non-iatrogenic malignant chylothorax is underreported. We present three cases of malignant chylothorax managed with lymphangiogram followed by therapeutic embolization. Case 1: A 70-year-old female with relapsed angi

Traumatic chylothorax management post-coronary artery bypass grafting - A systematic review.

IntroductionCoronary artery bypass graft (CABG) surgery is performed globally around 400,000 times annually. Despite its benefits, CABG can lead to complications, including chylothorax, a rare condition where chyle accumulates in the pleural cavity due to thoracic duct trauma. Currently, there are no international guidelines for traumatic chylothorax management post-CABG. This is the first systematic review to provide a comprehensive overview of the current state of management for chylothorax post-CABG.MethodsThis systematic review was conducted by searching EMBASE, Cochrane, Ovid and PubMed d

Laparoscopic Bilateral Nerve-Sparing Retroperitoneal Lymph Node Dissection for Testicular Cancer After Chemotherapy.

OBJECTIVES: Nerve-sparing techniques are commonly used in retroperitoneal lymph node dissection (RPLND) for patients with testicular cancer to preserve postoperative ejaculation. Laparoscopic RPLND is applicable to small residual masses; however, nerve-sparing techniques remain challenging. This study aimed to establish a safe, bilateral nerve-sparing laparoscopic RPLND via the transperitoneal approach. The technique was developed through cadaveric surgical training (CST) and applied clinically. METHODS: Thiel-fixed human cadavers were used for CST. In patients with testicular cancer after ch

Successful resolution of a continuous pneumothorax using canine xeno-blood patch pleurodesis in a cat.

CASE SUMMARY: An 11-year-old male neutered cat was referred to The Ohio State University's Veterinary Teaching Hospital after being diagnosed with pleural effusion by a referral veterinarian. After thoracocentesis, analysis of the effusion was consistent with chyle. Echocardiography, radiographs and bloodwork were used to diagnose hypertrophic cardiomyopathy phenotype and left-sided congestive heart failure, suspected to be secondary to uncontrolled hyperthyroidism. While initiating medical therapy, repeated thoracocenteses were required. A severe pneumothorax developed, necessitating placemen

The phrenic nerve; the forgotten nerve in head and neck surgery.

BACKGROUND: Neck dissection is a commonly performed procedure for oncologic control of head and neck malignancy. With contemporary modified radical and selective neck dissections, haematoma, wound infection, tissue necrosis, chyle leak and injury involving the marginal mandibular, hypoglossal, vagus or accessory nerves are commonly described complications. Although the phrenic nerve courses within the surgical planes explored during a neck dissection and has a vital function in innervating the diaphragm, few studies have been performed to investigate the exact incidence of post-operative phren
