Nucleus 21-channel auditory brainstem implant in patients with previous tumour removal.

Three patients with neurofibromatosis type 2 who had undergone previous cochlear nerve tumour removal were implanted with the Nucleus 21-channel auditory brainstem implant (ABI). The time intervals between tumour removal and implantation were 4, 5, and 7 years, respectively. Total bilateral deafness was confirmed before implantation. One patient was also blind after acute intracranial hypertension. The translabyrinthine approach was used in all cases. The choice of side for implantation depended on pre-operative magnetic resonance imaging study, the facial nerve function, the presence of recur

Correlation of Histomorphometric Changes with Diffusion Tensor Imaging for Evaluation of Blast-Induced Auditory Neurodegeneration in Chinchilla.

In the present study, we have evaluated the blast-induced auditory neurodegeneration in chinchilla by correlating the histomorphometric changes with diffusion tensor imaging. The chinchillas were exposed to single unilateral blast-overpressure (BOP) at ∼172dB peak sound pressure level (SPL) and the pathological changes were compared at 1 week and 1 month after BOP. The functional integrity of the auditory system was assessed by auditory brainstem response (ABR) and distortion product otoacoustic emissions (DPOAE). The axonal integrity was assessed using diffusion tensor imaging at regions of i

Acoustically Evoked Compound Action Potentials Recorded From Cochlear Implant Users With Preserved Acoustic Hearing.

OBJECTIVES: Less traumatic intracochlear electrode design and the introduction of the soft surgery technique allow for the preservation of low-frequency acoustic hearing in many cochlear implant (CI) users. Recently, new electrophysiologic methods have also been developed that allow acoustically evoked peripheral responses to be measured in vivo from an intracochlear electrode. These recordings provide clues to the status of peripheral auditory structures. Unfortunately, responses generated from the auditory nerve (auditory nerve neurophonic [ANN]) are somewhat difficult to record because they

Intraoperative auditory monitoring in vestibular schwannoma surgery: Diagnostic accuracy and interventional effectiveness - a systematic review.

Hearing preservation is a key goal of vestibular schwannoma (VS) microsurgery. Intraoperative auditory monitoring spans far-field auditory brainstem responses (ABR)/brainstem auditory evoked potentials (BAEP) and near-field cochlear nerve action potentials (CNAP)/dorsal cochlear nucleus action potentials (DNAP). Prior reports often conflate diagnostic accuracy of thresholds with interventional effectiveness. We separated these domains to deliver decision-relevant synthesis. PRISMA-guided systematic review of PubMed, ScienceDirect, and Embase. Eligible VS studies used ABR/BAEP, CNAP, or DNAP an

A device-independent approach to cochlear-implant telehealth.

OBJECTIVE: Commercial cochlear-implant (CI) telehealth is manufacturer-specific and generally does not replicate standard sound-field tests. This study developed and tested a manufacturer-independent CI telehealth system designed to replace a standard clinic visit. DESIGN: The system comprised a laptop, a sound card, headphones (placed over the CI sound processor), manufacturer's programming software and cables, and custom software for aided speech-recognition and warble-tone detection-threshold assessment. Phase 1 compared telehealth-system aided assessment to standard sound-field testing. I

Forward masking patterns by low and high-rate stimulation in cochlear implant users: Differences in masking effectiveness and spread of neural excitation.

The goal of the present study was to compare forward masking patterns by stimulation of low and high rates in cochlear implant users. Postlingually deafened Cochlear Nucleus® device users participated in the study. In experiment 1, two maskers of different rates (250 and 1000 pulses per second) were set at levels that produced equal masking for a probe presented at the same electrode as the maskers. This aligned the two masking functions at the on-site probe location. Then their forward masking patterns for the far probes were compared. Results showed that slope of the masked probe-threshold d

A proposed electroacoustic test protocol for personal FM receivers coupled to cochlear implant sound processors.

BACKGROUND: Current fitting guidelines from the American Academy of Audiology (Academy) support the use of objective electroacoustic measures and behavioral testing when fitting frequency modulation (FM) systems to hearing aids. However, only behavioral testing is recommended when fitting FM systems to individuals with cochlear implants (CIs) because a protocol for conducting electroacoustic measures has yet to be developed for this population. PURPOSE: The purpose of this study was to propose and examine the validity of a newly developed, objective, electroacoustic test protocol for fitting

Contralateral noise has possible asymmetric frequency-sensitive effect on the 2F1-F2 otoacoustic emission in humans.

Contralateral acoustic stimulation has been shown to produce a suppressive effect on the 2F1-F2 distortion-product otoacoustic emission (DPOAE). According to reports by other studies, there is a frequency-specific suppressive region between 1 and 2kHz when the contralateral broadband or narrowband noise stimulation contains energy close to or near the F2 frequencies in that region. In general the broader the bandwidth of the noise stimulation, the greater is the suppressive effect. A frequency-sensitive region between 1 and 2kHz has also been suggested as narrowband noises with center frequenc

The multichannel auditory brainstem implant: how many electrodes make sense?

OBJECT: Development of multichannel auditory brainstem implant (ABI) systems has been based in part on the assumption that audiological outcome can be optimized by increasing the number of available electrodes. In this paper the authors critically analyze this assumption on the basis of a retrospective clinical study performed using the Nucleus 22 ABI surface electrode array. METHODS: The perceptual performances of 61 patients with neurofibromatosis Type 2 were tested approximately 6 weeks after an eight-electrode ABI had been implanted. Of eight implanted electrodes 5.57 +/- 2.57 (mean +/- s

Changes in calcium-binding protein expression in the auditory brainstem nuclei of the jaundiced Gunn rat.

Sensorineural hearing loss and auditory dysfunction are major sequelae of neonatal hyperbilirubinemia. The sites and cellular effects of bilirubin toxicity in the auditory brainstem pathway are not easily detected. Since altered intracellular calcium homeostasis may play a role in neuronal cell death, we hypothesized that the expression of calcium-binding proteins may be altered in the classic animal model of bilirubin neurotoxicity. The expression of the calcium-binding proteins, calbindin-D28k and parvalbumin, in the brainstem auditory pathway of homozygous recessive jaundiced (jj) Gunn rats

Improving Outcomes of Single-Sided Deaf Cochlear Implant Users by Reducing Interaural Frequency and Loudness Mismatches through Device Programming.

The study aimed to improve outcomes in Nucleus cochlear implant (CI) recipients with single-sided deafness (SSD) by reducing interaural frequency and loudness mismatches through device programming. In Experiment 1a, a modified frequency allocation table (FAT) was created to better match the tonotopicity of the contralateral ear and reduce interaural frequency mismatch. Twenty experienced SSD-CI users completed localization and speech recognition tests with their everyday FAT. Tests were repeated after 6 weeks' use of the modified FAT. Participants compared both FATs for 2 weeks before being te

Pathophysiology of Facial Nerve Stimulation and Its Implications for Electrical Stimulation in Cochlear Implants.

OBJECTIVES: A small number of cochlear implant (CI) users experience facial nerve stimulation (FNS), which can manifest as facial twitching. In some patients, this can be resolved by adjusting the electrical stimulation parameters. However, for others, facial stimulation can significantly impair CI outcomes or even prevent its use. The exact mechanisms underlying FNS are unclear and may vary among patients. DESIGN: Transimpedance measurements were used to assess lateral and longitudinal spread of current within 15 cochlea of nucleus CI recipients with FNS (13 unilateral recipients and 1 bilat

Deletion of BDNF in Pax2 Lineage-Derived Interneuron Precursors in the Hindbrain Hampers the Proportion of Excitation/Inhibition, Learning, and Behavior.

Numerous studies indicate that deficits in the proper integration or migration of specific GABAergic precursor cells from the subpallium to the cortex can lead to severe cognitive dysfunctions and neurodevelopmental pathogenesis linked to intellectual disabilities. A different set of GABAergic precursors cells that express Pax2 migrate to hindbrain regions, targeting, for example auditory or somatosensory brainstem regions. We demonstrate that the absence of BDNF in Pax2-lineage descendants of Bdnf Pax2 KOs causes severe cognitive disabilities. In Bdnf Pax2 KOs, a normal number of parvalbumin-

Resolution of long standing tinnitus following radiofrequency ablation of C2-C3 medial branches--a case report.

Tinnitus is described as an auditory phantom perception analogous to central neuropathic pain. Despite the high prevalence of this debilitating symptom, no intervention is recognized that reliably eliminates tinnitus symptoms; a cause has yet to be determined. A 65-year-old healthy man presented with a 3 year history of left-sided tinnitus. Full workup performed by the primary care physician including blood tests for electrolyte imbalance, consultations by 2 independent otholaryngologists, and imaging did not reveal abnormalities to provide etiology of the tinnitus. No other complaints were no
