[Neurogenic bladder in children: basic principles in diagnosis and treatment].

The diagnosis of neurogenic bladder can be easy in myelomeningocele and much more difficult in occult dysraphia or medical etiologies. Careful clinical examinations and urodynamic investigations are mandatory for the diagnosis and the follow up of affected patients. Clinico-anatomical correlations are poor. If urinary leak is the first apparent symptom, preservation of the upper urinary tract is the main goal of the surgeon. If natural history of the neurogenic bladder is destruction of the detrusor and paralysis of the trigona, obstructive uropathy is the main physiological concern. Urinary l

The surgical techniques of transperineal anastomotic urethroplasty for complex posterior urethral stenosis in boys and the long-term follow up outcomes.

OBJECTIVE: To explored the curative effects of various surgical methods used to treat complicated posterior urethral strictures in boys and the long-term complication. METHODS: We retrospectively studied 28 boys under 14 years of age with complicated posterior urethral strictures treated at our hospital from January 2015 to December 2020. Urethral angiography revealed posterior urethral strictures. Twelve had previously failed urethral surgery; four had urethral fistulae. All underwent end-to-end urethral anastomoses via a transperineal, inferior pubic approach. We freed the distal end of the

A Novel Modification of Transurethral Enucleation and Resection of the Prostate in Patients With Prostate Glands Larger than 80 mL: Surgical Procedures and Clinical Outcomes.

OBJECTIVE: To evaluate the efficacy and safety of the modified transurethral enucleation and resection of the prostate (M-TUERP) vs the conventional bipolar transurethral resection of the prostate (B-TURP) for the treatment of prostates larger than 80 mL. METHODS: From April 2012 to May 2014, 86 patients with a prostate volume of >80 mL were divided into 2 groups to undergo M-TUERP and B-TURP. In the M-TUERP group, we proposed combining the 12-mm trocar suprapubic cystostomy and using the techniques of "umbrella-shaped resection," "point resection," and "segmental enucleation" to modify the t

Considerations before repair of acquired rectourethral and urethrovaginal fistulas in children.

BACKGROUND: Acquired rectourethral or urethrovaginal fistula between the rectum or vaginal and lower urinary tract is an uncommon entity, which occurs as a consequence of pelvic disorder, including trauma, iatrogenic injury, inflammatory bowel disease, pelvic neoplasm or infection. But when is it appropriate to repair the fistula and what methods to be chosen? There has been no consensus on them. This study was undertaken to determine the timing of the procedure and the repair of rectourethral and urethrovaginal fistula. METHODS: From 1998 to 2006, we treated 19 children with rectourethral or

Percutaneous cystolithotomy of large urinary-diversion calculi using a combination of laparoscopic and endourologic techniques.

BACKGROUND AND PURPOSE: Patients with urinary diversions are at higher risk for bladder urolithiasis. Often, the stone burden is large, necessitating open stone removal. We describe our technique for removing such stones using a combination of laparoscopic and endourologic instrumentation. PATIENTS AND METHODS: With the patient in the dorsal lithotomy position or supine, cystoscopy is performed via the native urethra or catheterizable stoma, respectively. With the urinary reservoir distended with normal saline, percutaneous access is obtained under direct vision with a 10-mm trocar introduced

Laparoscopic Lich - Gregoir ureteroneocystostomy versus open surgery: Are there benefits from the minimally invasive procedure?

BACKGROUND: The optimal treatment of vesicoureteral reflux is controversial. When patients need surgery, the gold standard is open vesicoureteral reimplantation. Despite the more frequent use of laparoscopy when managing vesicoureteral reflux, we have found scarce comparative studies comparing the open versus the laparoscopic Lich-Gregoir reimplantation. OBJECTIVE: The aim of this study is to compare the open Lich-Gregoir reimplantation technique versus the laparoscopic approach, in patients with primary vesicoureteral reflux in terms of complications and the procedure´s success, to document

Clinical experience of the VQZ plasty for catheterizable urinary stomas.

OBJECTIVE: To evaluate the effectiveness of VQZ plasty, we analyzed our experience with continent catheterizable urinary stomas using VQZ plasty and V-flap techniques. PATIENTS AND METHODS: We retrospectively reviewed the records of 25 patients who underwent Mitrofanoff procedures in 2000-2009. All stomas were created in the lower quadrant. There were 10 patients who underwent VQZ plasty and 15 in whom the V-flap technique was used. The stomal complications of these two groups were compared. RESULTS: Two patients (20%) with VQZ plasty and five (33.3%) with V-flap required surgical revision.

Pathogenesis of renal injury in the megabladder mouse: a genetic model of congenital obstructive nephropathy.

Congenital obstructive nephropathy (CON) is the most common cause of chronic renal failure in children often leading to end-stage renal disease. The megabladder (mgb) mouse exhibits signs of urinary tract obstruction in utero resulting in the development of hydroureteronephrosis and progressive renal failure after birth. This study examined the development of progressive renal injury in homozygous mgb mice (mgb-/-). Renal ultrasound was used to stratify the disease state of mgb-/- mice, whereas surgical rescue was performed using vesicostomy. The progression of renal injury was characterized u

A simplified technique for laparoscopic ureteroneocystostomy without ureteral nipple or submucosal tunneling.

PURPOSE: To present our preliminary experience with laparoscopic ureter reimplantation for distal ureteral stricture without an everted ureteral nipple or submucosal tunneling, and to evaluate its clinical feasibility, safety, and validity. PATIENTS AND METHODS: Six patients with distal ureteral stricture underwent transperitoneal laparoscopic ureteral reimplantation. The ureter was reimplanted into the bladder without everting the ureter or tunneling construction. The seromuscular wall of the ureter was anastomosed circumferentially to the detrusor muscle using continuous absorbable sutures.

Incidence, Detection, and Management of Urological Injuries in Obstetric and Gynecologic Surgeries: A Five-Year Observational Study.

INTRODUCTION: Urological injuries are rare but significant complications of obstetric and gynecologic procedures, associated with morbidity, prolonged hospital stay, and medicolegal implications. Bladder injuries are typically identified intraoperatively, while ureteric injuries are often delayed presentation. Early recognition and prompt management are critical to reduce complications. The study aimed to evaluate the incidence, type, timing of detection, management, and outcomes of urological injuries in patients undergoing obstetric and gynecologic surgeries. MATERIALS AND METHODS: This ret

The Subcutaneous Ureteral Bypass 3.0 device shows improved short-term outcomes compared to the 2.0 device for treatment of benign ureteral obstructions in cats.

OBJECTIVE: The Subcutaneous Ureteral Bypass (SUB) 3.0 device was designed to treat ureteral obstructions to circumvent kinking found with prior versions where the nephrostomy and cystostomy catheters traversed the body wall. The authors hypothesized that new adaptations would reduce procedure times and kinking without negatively impacting other short-term complication rates. METHODS: The medical records of cats with SUBs for benign ureteral obstructions flushed routinely with tetrasodium EDTA were reviewed. Cats with a SUB 2.0 (group 1), SUB 3.0 (group 2), and unilateral SUB 2.0/unilateral SU

A systematic review and meta-analysis of surgical approaches in pelvic fracture-associated urethral injury: primary endoscopic realignment versus delayed urethroplasty.

BACKGROUND: Pelvic fracture urethral injury (PFUI) can produce considerable morbidity, including urethral stricture, urinary incontinence, and erectile dysfunction. Optimal management of PFUI is unclear, however, the current gold standard is placement of a suprapubic cystostomy with delayed urethroplasty (SCDU) performed several months later. Another option is primary realignment (PR) with urethral catheter. Through a systematic review and meta-analysis, we aimed to compare the outcomes of PR and SCDU in patients with PFUI. METHODS: A systematic search across databases (PubMed, ScienceDirect,

Cystoscopically Placed Pancreatic Duct Stent for Management of Bladder-Drained Pancreas Transplant with Pancreatic Duct Leak.

Background: Historically, exocrine pancreas secretions during pancreas transplant were commonly managed by bladder drainage. Although this technique has fallen out of favor because of significant rates of urologic complications, urologists must still be prepared to assist when they arise. We describe the first reported case of a cystoscopically placed pancreatic duct stent for management of a pancreas transplant duodenocystostomy leak in the setting of normal bladder function. Case Presentation: A 63-year-old male with a history of type 1 diabetes mellitus complicated by end-stage renal diseas
