Applied anatomy of the latissimus dorsi free flap for refinement in one-stage facial reanimation.

BACKGROUND: The face can be reanimated after long-term paralysis by free microneurovascular tissue transfer. Flaps from gracilis and pectoralis minor usually require a two-stage procedure with a cross-face nerve graft. Latissimus dorsi has a much longer muscular nerve, the thoracodorsal nerve, which could avoid the need for a second cross-face nerve graft. Our hypothesis is that the neurovascular pedicles of small segments of latissimus dorsi would be long enough to reach the opposite side of the face and to provide a reliable blood and nerve supply to the flaps. METHOD: To test this hypothes

Changes in shape and length of the collateral and accessory collateral ligaments of the metacarpophalangeal joint during flexion.

BACKGROUND: Although the collateral and accessory collateral ligaments of the metacarpophalangeal joint contribute to the stability of this joint, the functional role of the various portions of these ligaments during flexion is unclear. We investigated changes in the three-dimensional shape and length of the collateral and accessory collateral ligaments during flexion to determine how each portion stabilized the metacarpophalangeal joint. METHODS: Twelve fingers from three embalmed cadavers were examined. The origin and the insertion point of the dorsal, middle, and volar portions of the radi

Image-guided anatomical and morphometric study of supraorbital and transorbital minicraniotomies to the sellar and perisellar regions: comparison with standard techniques.

OBJECT: Minimally invasive approaches have been proposed for the treatment of anterior cranial base pathology. Whereas earlier studies have quantified surgical exposure by referring to the opening on the surface, this cadaveric morphometric study redefines the concept of working area by examining the deep exposures afforded by several different approaches. Specifically, the authors systematically quantify and compare the operative exposure afforded by the pterional, supraorbital, and transorbital keyhole approaches to the sellar, suprasellar, and perisellar regions, including the anterior comm

Implications of lumbar plexus anatomy for removal of total disc replacements through a posterior approach.

STUDY DESIGN: An anatomic study in which the lumbar plexuses of 14 embalmed cadavers were dissected bilaterally and measured using a posterior approach. OBJECTIVE: To determine the cephalocaudal (root-to-root) distances and the mediolateral (root-to-tether) distances within the lumbar plexus and determine the feasibility for removal of a lumbar total disc replacement (TDR) through these anatomic spaces using a posterior approach. SUMMARY OF BACKGROUND DATA: Currently, lumbar TDRs are implanted primarily through an anterior retroperitoneal or transperitoneal approach. However, revision surger

Three-dimensional fluoroscopy for evaluation of articular reduction and screw placement in calcaneal fractures.

BACKGROUND: Anatomic reconstruction of the posterior calcaneal facet after intra-articular fracture is one of the critical factors in achieving a good functional result. Intraoperative evaluation of fracture reduction and implant placement relies on direct view by standard fluoroscopy. We hypothesized that three-dimensional (3D) fluoroscopy is more accurate than conventional fluoroscopy, and equivalent to CT for determining fracture reduction and screw position in calcaneal fractures. METHODS: A Sanders type IIB fracture pattern was created in eight embalmed lower extremity cadaver specimens.

Development, validation, and biomechanical evaluation of a high-fidelity finite element model of the forearm musculoskeletal system.

BACKGROUND: The human forearm, a dynamically coordinated system of bones, muscles, and connective tissues, is central to upper limb function but poses significant challenges for computational modeling due to its anatomical complexity. Existing finite element (FE) models of the forearm often oversimplify soft tissues or lack rigorous validation, limiting their clinical utility. This study aimed to develop and validate a high-fidelity 3D FE model of the forearm musculoskeletal system to replicate in vivo stress distribution under axial loading. METHODS: The model was constructed using CT and MRI

The use of serratus anterior artery as the pedicle for complex humerus reconstruction with a vascularized fibular graft: Anatomical study and clinical application in a case series.

INTRODUCTION: Free vascularized fibular graft is an optimal option for the reconstruction of long, complex bone defects in the humerus. Previous surgeries, infection, and trauma, particularly with brachial artery revascularization, may limit the use of conventional recipient vessels. In this study, we review the use of the serratus anterior artery as a recipient vessel, the anatomical validation of its feasibility and its clinical application in a series of cases. METHODS: The anatomical study consisted of 14 dissections on 7 Thiel-embalmed cadavers. The distance from the origin of the serrat

Emergence Point of the Infratrochlear Nerve With Reference to the Nasion and Its Clinical Implications in Craniofacial Surgery.

BACKGROUND: Infratrochlear nerve (ITN) block is widely used in craniofacial surgery to provide anesthesia and reduce perioperative discomfort. Accurate localization of the emergence point of the ITN (EP-ITN) using external landmarks may improve the precision of the block. PURPOSE: This study aimed to identify the anatomical location of the EP-ITN in the medial orbital margin with reference to the nasion, and to evaluate whether the nasion may serve as a consistent external landmark for ITN block procedure. STUDY DESIGN, SETTING, SAMPLE: This was a cross-sectional anatomic study conducted at

Suprapubic Breast Augmentation (SUBA): A Cadaveric Study For a New Anatomic Approach.

BACKGROUND: Breast augmentation is a common aesthetic procedure, often performed alongside abdominal wall repairs such as rectus diastasis or umbilical hernias. Traditional incisions include inframammary, periareolar, axillary, and transumbilical approaches, each with benefits and drawbacks. Inspired by laparoscopic techniques, the suprapubic approach offers a novel anatomical approach that may reduce visible scarring while facilitating concurrent abdominal procedures. METHODS: A cadaveric feasibility study was conducted on two embalmed female specimens. A 3 cm suprapubic incision, similar to

Acromioclavicular Joint Depth and Angle in the Thai Population: Implications for Clavicle Hook Plate Design.

BACKGROUD: Subacromial erosion and impingement syndrome are frequently reported complications following clavicle hook plate (CHP) fixation in Thai orthopedic practice. These complications may result from CHP designs that do not adequately accommodate the acromioclavicular joint (ACJ) anatomy of the Thai population. Currently, only 2 CHP sizes (12 mm and 15 mm in depth) with a 0° angle are commercially available, potentially increasing the risk of osteolysis due to direct point contact with the acromion. This study aimed to determine the average depth and angle of the ACJ in the Thai population

Stress-strain relationship of individual hamstring muscles: A human cadaver study.

The incidence of hamstring muscle strain varies among muscles, suggesting that the mechanical stresses associated with elongation may differ among muscles. However, the passive mechanical properties of whole human muscles have rarely been directly measured and clarified. This study aimed to clarify the stress-strain relationship of the hamstring muscles using a soft-embalmed Thiel cadaver. The long heads of the biceps femoris (BFlh), semimembranosus (SM), and semitendinosus (ST) muscles were dissected from eight cadavers. The proximal and distal hamstring tendons were affixed to the mechanical

Lateral Ulnar Collateral Ligament of the Elbow Joint: Reconsideration of Anatomy in Terms of Connection with Surrounding Fibrous Structures.

BACKGROUND: To improve the clinical results of lateral ulnar collateral ligament (LUCL) reconstruction of the elbow joint, better understanding of the anatomy of the aponeuroses and joint capsule could be relevant. This study considers the previously described anatomy of the LUCL in relation to the related aponeuroses and joint capsule rather than as a discrete ligament. We hypothesized that the deep aponeuroses of the superficial extensor muscles and supinator form a relevant portion of the joint capsule previously defined as the LUCL. METHODS: Twenty-four elbows (12 right) from 21 embalmed

Ultrasound as a Diagnostic Tool in the Assessment of Lisfranc Joint Injuries.

OBJECTIVES: Ligamentous Lisfranc injuries are frequently overlooked because of subtle clinical presentations and diagnostic difficulties. The dorsal Lisfranc ligament (DLL) is easily visualized with ultrasound (US), which can provide quick, cost-effective diagnoses of disorders but is not considered standard clinical practice. This study sought to compare DLL measurement accuracy between US and cadaveric dissection. METHODS: Ultrasound images of 22 embalmed cadaveric feet were obtained with an M-Turbo US machine and a 6-13-MHz linear array (FUJIFILM SonoSite, Inc, Bothell, WA). Images were me
