Alpha-fetoprotein levels in amniotic fluids from spontaneous abortions.

Alpha-fetoprotein (A.F.P.) levels in the amniotic fluid were determined in 54 cases of spontaneous abortion in which the amniotic sac remained intact. These levels were correlated with the morphological and cytogenetic status of the fetus. Of the 29 fetuses with no apparent abnormality 22 had A.F.P. levels below 50 mug/ml, while 10 of the 11 fetuses with severe neural tube defects had raised levels (50-305 mu/ml). Seventeen fetuses had chromosome anomalies of various types. Three out of four which were 45, X had considerably raised A.F.P. levels (78-210 mug/ml) but fetuses with other chromosom

[Prenatal diagnosis. Review, personal and prospective studies].

1. In a review of methods developed for the identification of fetal malformations, the technique, risks and results of amniocentesis are presented. 2. Large series already published have demonstrated the relative simplicity and feasibility of the procedure as well as current indications for its utilization. These include the detection of chromosomal anomalies, the determination of sex (in certain sex-linked disorders), documentation of enzymatic and metabolic deficiencies, and the demonstration of open lesions of the neural tube by appropriate techniques. 3. Experience with over 500 cases pers

Prenatal ultrasound diagnosis of fetal upper gastrointestinal bleeding in a monochorionic twin pregnancy: a case report.

BACKGROUND: Fetal upper gastrointestinal bleeding (UGIB) is an exceptionally rare condition. We report a case of antenatally detected UGIB in a monochorionic diamniotic twin pregnancy, emphasizing key ultrasound features that enabled timely delivery and a favorable neonatal outcome. CASE PRESENTATION: A 23-year-old woman with a monochorionic diamniotic twin pregnancy complicated by Stage II twin-to-twin transfusion syndrome (TTTS) underwent fetoscopic laser ablation at 23 + 1 weeks. At 33 + 5 weeks’ gestation, routine ultrasound revealed marked gastric and intestinal dilatation with echogenic

Evaluation of Inclusion Cysts and Granulation Tissue After Prenatal and Postnatal Myelomeningocele Repair.

AIM: To evaluate the long-term outcomes of nine patients who underwent myelomeningocele repair via fetoscopic surgery, open fetal surgery, and postnatal surgery. MATERIAL AND METHODS: The presence of inclusion cysts and the thickness of granulation tissues at the surgical site were analyzed using spinal magnetic resonance imaging (MRI) at a 7-year follow-up to determine their impact on clinical outcomes. RESULTS: The spinal defect levels ranged from L2 to S2. Granulation tissue at the surgical site was thicker in the prenatal open and postnatal repair groups when compared to the fetoscopic r

Prenatal evolution of hindbrain herniation following fetal open neural tube defect repair.

BACKGROUND: Prenatal open neural tube defect (ONTD) repair improves motor function and reduces hydrocephalus treatment by ensuring watertight closure of the defect. Hindbrain herniation (HBH) reversal is often observed postoperatively, but its association with postnatal outcomes remains unclear. OBJECTIVE: To assess HBH evolution via fetal MRI following prenatal ONTD repair, and its association with reduced hydrocephalus treatment and absence of cerebrospinal fluid (CSF) leak. MATERIALS AND METHODS: This retrospective study included fetuses who underwent prenatal ONTD repair via laparotomy-a

Society for Maternal-Fetal Medicine Consult Series #72: Twin-twin transfusion syndrome and twin anemia-polycythemia sequence.

Thirty percent of spontaneously occurring twins are monozygotic, of which two-thirds are monochorionic, possessing a single placenta. A common placental mass with shared intertwin placental circulation is key to the development and management of complications unique to monochorionic gestations. In this Consult, we review general considerations and a contemporary approach to twin-twin transfusion syndrome and twin anemia-polycythemia sequence, providing management recommendations based on the available evidence. The following are the Society for Maternal-Fetal Medicine recommendations: (1) we r

Impact of the volume of the myelomeningocele sac on imaging, prenatal neurosurgery and motor outcomes: a retrospective cohort study.

To investigate the association of the myelomeningocele (MMC) volume with prenatal and postnatal motor function (MF) in cases who underwent a prenatal repair. Retrospective cohort study (11/2011 to 03/2019) of 63 patients who underwent a prenatal MMC repair (37 fetoscopic, 26 open-hysterotomy). At referral, measurements of the volume of MMC was performed based on ultrasound scans. A large MMC was defined as greater than the optimal volume threshold (ROC analysis) for the prediction of intact MF at referral (2.7 cc). Prenatal or postnatal intact motor function (S1) was defined as the observation

Fetal aortic valvuloplasty may rescue fetuses with critical aortic stenosis and hydrops.

OBJECTIVE: Critical aortic stenosis (CAS) with a restrictive interatrial septum may lead to fetal congestive heart failure and hydrops, usually culminating in fetal demise if left untreated. The aim of this study was to assess the effects of fetal aortic valvuloplasty (FAV) on hemodynamics and outcome in these patients. METHODS: This was a retrospective review of fetuses with CAS and signs of hydrops that underwent FAV in our center between 2000 and 2020. Echocardiograms and patients' charts were analyzed for ventricular and valvular dimensions and for outcome. RESULTS: Hydrops was present a

Management of large congenital chylous ascites in a preterm infant: fetal and neonatal interventions.

Congenital chylous ascites is a rare cause of ascites in newborn infants. Its aetiology varies from localised leaky lymphatic duct to genetic syndromes. Most of these cases have transient ascites resolving over time with conservative management but some may progress needing medical as well as surgical treatment. We describe a case of antenatally detected large fetal ascites necessitating abdominal paracentesis and amnioreduction. Marked respiratory distress at birth required urgent abdominal paracentesis to relieve symptoms. The infant initially showed a good response to medium chain triglycer

Comparison of umbilical cord occlusion methods: Radiofrequency ablation versus laser photocoagulation.

OBJECTIVE: To compare outcomes between 2 umbilical cord occlusion (UCO) methods: radiofrequency ablation (RFA) versus fetoscopic-guided laser photocoagulation. METHODS: Retrospective study of all monochorionic diamniotic multiple gestations that underwent UCO with RFA (preferred after 2014) or laser (preferred before 2014). In bivariate analysis, patients treated with RFA were compared with laser. Risk factors for intrauterine fetal demise (IUFD) and 30-day survival of the co-twin were identified, and multiple logistic models were fitted to examine whether the UCO method was associated with t

Fetoscopic laser photocoagulation for twin-twin transfusion syndrome.

The aim of this study was to review fetoscopic laser photocoagulation (FLP), which ablates placental vascular anastomoses to treat twin-twin transfusion syndrome (TTTS). A review of studies reporting on the procedures, outcomes, complications and nonconventional applications of FLP for TTTS was conducted. FLP has been established as the primary treatment for monochorionic twin pregnancy associated with TTTS at 16-26 weeks. FLP is the only therapy that directly addresses the underlying pathophysiology. The recent technique modification of FLP, referred to as the 'Solomon technique', induces sel

Lower urinary tract obstruction: fetal intervention based on prenatal staging.

The authors present an overview of lower urinary tract obstruction (LUTO) in the fetus with a particular focus on the insult to the developing renal system. Diagnostic criteria along with the challenges in estimating long-term prognosis are reviewed. A proposed prenatal LUTO disease severity classification to guide management decisions with fetal intervention to maintain or salvage in utero and neonatal pulmonary and renal function is also discussed. Stage I LUTO (mild form) is characterized by normal amniotic fluid index after 18 weeks, normal kidney echogenicity, no renal cortical cysts, no

Fetoscopic laser treatment of twin-to-twin transfusion syndrome followed by severe twin anemia-polycythemia sequence with spontaneous resolution.

We present a case of twin anemia-polycythemia sequence after laser surgery for twin-to-twin transfusion syndrome which resolved spontaneously. No residual anastomoses were identified after placental injection with colored dye. We discuss the possible pathogenetic mechanisms that may have led to this remarkable clinical course and discuss the various management options.

Partial amniotic carbon dioxide insufflation (PACI) facilitates fetoscopic interventions in complicated monochorionic twin pregnancies.

BACKGROUND: Adequate visualization of the placenta or umbilical cord during fetoscopic procedures in complicated monochorionic twin pregnancies may be difficult because of placental position and spatial constraints, as well as stained amniotic fluid. Partial amniotic carbon dioxide insufflation (PACI) has made it possible to overcome these obstacles in other fetoscopic procedures, but its value has not yet been reported in monochorionic twins. METHODS: Partial amniotic carbon dioxide insufflation was carried out in five expectant women with complicated monochorionic twin pregnancies between 1

Test amnioinfusion to determine suitability for serial therapeutic amnioinfusion in midtrimester premature rupture of membranes.

OBJECTIVE: To evaluate whether a test amnioinfusion procedure is useful in selecting cases of midtrimester preterm premature rupture of membranes (PPROM) which may benefit from serial amnioinfusions if the initial fluid is retained. STUDY DESIGN: The Centre for Fetal Care database between 1992 and 2000 was reviewed for women with PPROM <26 weeks who had undergone amnioinfusion. Amniotic fluid index (AFI) was assessed before and after a test amnioinfusion procedure. Those who retained fluid > or =48 h underwent serial AFI assessment with a view to serial amnioinfusion when oligohydramnios recu
