The objective of this study is to assess the results of labyrinthine fenestration for fixed stapes in chronic ear disease. Using a prospective database, pre- and postoperative audiometric data from patients undergoing labyrinthine fenestration for fixation of the stapes in chronic ear disease others than otosclerosis between 2002 and 2012 were evaluated. Twenty-three labyrinthine fenestrations in chronic ear disease were performed (17 malleo-stapedotomies, 4 incus-stapedotomies, 1 neo-malleus-stapedotomy, 1 TORP-stapedotomy). Overall, the mean short-term (2 months) and long-term (42 months) po

OBJECTIVE: To present preliminary results in transmeatal exclusive endoscopic ear surgery.
STUDY DESIGN: Retrospective.
INTERVENTION: Rigid endoscopes were used for all procedures. A wide posterior tympanomeatal flap was elevated transmeatally, and the scutum was removed with a bone curette or was drilled until visualization of cholesteatoma extension and the mastoid antrum. The malleus and incus were removed when they were involved in the cholesteatoma or restricted access to it. When present, the stapes was left intact. Endoscopic accessibility was defined by no extension of the cholesteat

A human temporal-bone preparation was used to determine the effects of various degrees of artificial ossicular fixation on the sound-induced velocity at the input-side (the umbo of the malleus) and the output-side (the stapes) of the ossicular chain. Construction of various degrees of attachment between an ossicle and the surrounding temporal bone provided a range of reduction in ossicular mobility or "fixations". The results demonstrate different effects of the fixations on the umbo and stapes velocity: fixations of the stapes or incus produce larger reductions in sound-induced stapes velocit

For the majority of patients with sensorineural hearing loss (SNHL) many available hearing aids often do not achieve satisfactory results. For these patients partially implantable hearing devices have been developed, allowing distortion-free hearing and speech intelligibility that may be superior to conventional hearing aids. The external parts of partial implants, however, may result in a patient's stigmatization. Furthermore, they do not use the acoustic properties of the external auditory canal. Recently, we published the successful development of the first totally implantable hearing devic

Eight chinchillas aged 8.9 to 12.8 years were used to examine the effect of noise on the aging ear. The left malleus/incus complex was removed to produce a 50-dB conductive hearing loss which protected those ears from noise damage. The animals were then exposed for 36 days to an octave band of noise with a center frequency of 0.5 kHz and a sound pressure level of 95 dB. After 1 hour (n = 2) or 1 month (n = 6) of recovery, their cochleas were prepared for microscopic examination. The percentages of missing inner hair cells (IHCs) were 7.4 +/- 6.0% and 7.8 +/- 5.1% for their protected and expose

OBJECTIVES: Chronic otitis media can cause ossicular chain discontinuity (OCD), especially erosion of long process of incus. This study introduces a novel "Hot Dog" technique for OCD reconstruction that combines glass ionomer cement (GIC) with autogenous cartilage dust and platelet-rich fibrin (PRF) to address these limitations. The aim is to evaluate the auditory and anatomical outcomes of the "Hot Dog" technique compared to GIC alone, and to describe the "Hot Dog" reconstruction method in detail.
METHODS: This prospective, randomized controlled trial compared the "Hot Dog" technique to GIC

OBJECTIVES: Stapedotomy is performed to address conductive hearing deficits. While hearing thresholds reliably improve at low frequencies (LF), conductive outcomes at high frequencies (HF) are less reliable and have not been well described. Herein, we evaluate post-operative HF air-bone gap (ABG) changes and measure HF air conduction (AC) thresholds changes as a function of frequency.
METHODS: Retrospective review of patients who underwent primary stapedotomy with incus wire piston prosthesis between January 2016 and May 2020. Pre- and postoperative audiograms were evaluated. LF ABG was calcu

Microtia with aural atresia presents a dual challenge-restoration of external ear anatomy and rehabilitation of conductive hearing loss. Multiple staged procedures are often required, increasing patient burden. This study presents a single-stage surgical approach combining auricular reconstruction using Porous High-Density Polyethylene (pHDPE, Medpor®) with simultaneous implantation of a Vibrant Soundbridge (VSB) and intraoperative audiological evaluation. To assess the surgical feasibility, safety, and postoperative cosmetic and audiological outcomes of a combined one-stage Medpor reconstruct

Air conduction and bone conduction thresholds in audiograms provide clinically useful information. However, whether bone conduction audiograms can help distinguish between different conductive hearing disorders remains unestablished. This retrospective case-control study included patients with hearing loss (surgically-confirmed ossicular sclerosis, ossicular discontinuity or tympanic membrane perforation) and a group of people with normal hearing (Jikei University School of Medicine, Tokyo, Japan). Data were collected between November 30, 2017 and December 30, 2022. Bone conduction threshold p

OBJECTIVES: To better distinguish NOG-related-symphalangism spectrum disorder (NOG-SSD) from chromosomal 17q22 microdeletion syndromes and to inform surgical considerations in stapes surgery for patients with NOG-SSD.
BACKGROUND: Mutations in NOG cause a variety of skeletal syndromes that often include conductive hearing loss. Several microdeletions of chromosome 17q22 lead to severe syndromes with clinical characteristics that overlap NOG-SSD. Isolated deletion of NOG has not been described, and therefore the contribution of NOG deletion in these syndromes is unknown.
