Left atrial strain predicts the rhythm outcome in patients with persistent atrial fibrillation undergoing left atrial cryoablation during minimally invasive mitral valve repair.

OBJECTIVES: Patients with atrial fibrillation (AF) have lower left atrial (LA) strain, which is a predictor for LA function. Here, we evaluated the prognostic value of LA strain to predict the rhythm outcome in patients with persistent AF undergoing LA cryoablation concomitant to minimally invasive mitral valve repair. METHODS: Between 01/2016 and 12/2020, 72 patients with persistent AF underwent LA cryoablation during minimally invasive mitral valve surgery. All patients received a complete LA lesion set and left atrial appendage (LAA) closure with a clip. All patients received preoperative

A new procedure for elimination of atrial fibrillation associated with mitral valve disease: a proof-of-concept study.

BACKGROUND: Left atrial enlargement and fibrosis have been linked to the pathogenesis of atrial fibrillation (AF). The authors aimed to introduce a novel concept and develop a new procedure for AF treatment based on these characteristics. METHODS: The study included three stages. The first stage was a descriptive study to clarify the characteristics of the left atrial enlargement and fibrosis' distribution in patients with mitral valve disease and long-standing persistent AF. Based on these characteristics, the authors introduced a novel concept for AF treatment, and then translated it into a

Incidence, persistence, and clinical relevance of iatrogenic atrial septal defects after percutaneous left atrial appendage occlusion.

OBJECTIVE: To investigate the rate and clinical impact of a persisting iatrogenic atrial septal defect (iASD) after percutaneous left atrial appendage occlusion (LAAO). BACKGROUND: Percutaneous LAAO is an alternative to oral anticoagulation (OAC) for the prevention of ischemic stroke and systemic embolism in patients with atrial fibrillation (AF). Data regarding incidence and persistence of iASD after LAAO procedures and its clinical relevance is scarce. METHODS: We retrospectively analyzed 144 patients that underwent LAAO at our center between 2009 and 2020 who had at least one follow-up in

Left atrial appendage occlusion in a mirror-image dextrocardia: A case report and review of literature.

BACKGROUND: In mirror-image dextrocardia, the anterior-posterior position of the cardiac chambers and great vessels is maintained, but the left-right orientation of the abdominal organs is reversed. The abnormal anatomy of the heart poses surgical challenges and problems in dealing with surgical risk and monitoring complications. There are few reports on closure of the left atrial appendage (LAA) in dextrocardia and no reports on the application of enhanced recovery after surgery (ERAS) following LAA occlusion (LAAO) procedures. CASE SUMMARY: The objective for this case was to ensure perioper

Mechanisms, predictors, and evolution of severe peri-device leaks with two different left atrial appendage occluders.

AIMS: Incomplete left atrial appendage occlusion (LAAO) due to peri-device leak (PDL) is a limitation of the therapy. The Amulet IDE trial is the largest randomized head-to-head trial comparing the Amulet and Watchman 2.5 LAAO devices with fundamentally different designs. The predictors and mechanistic factors impacting differences in PDLs within the Amulet IDE trial are assessed in the current analysis. METHODS AND RESULTS: An independent core lab analysed all images for the presence or absence of severe PDL (>5 mm). The incidence, mechanistic factors, predictors using propensity score-match

Efficacy and Safety of a Dedicated Left Atrial Appendage Occlusion Protocol.

OBJECTIVES: The aim of this study is to present a dedicated left atrial appendage closure protocol, which could be of great interest in the approach of frail patients. BACKGROUND: Left atrial appendage (LAA) occlusion emerged as a promising therapeutic tool for stroke prevention, as most of the atrial fibrillation (AF) related strokes result from LAA thromboembolism, with an important residual risk even for anticoagulated patients. As an internationally recognized and scientifically-based protocol for the patients with higher comorbidities has not yet been defined for this procedure in terms

Anesthesia Management Involving Remimazolam in a Patient With Pulmonary Hypertension and Right Ventricular Failure: A Case Report.

In the anesthetic management of patients with pulmonary hypertension (PH), it is essential to minimize right ventricular (RV) load and prevent abrupt changes in pulmonary vascular resistance (PVR). We report a case in which safe anesthetic management was achieved without circulatory collapse by using remimazolam in a patient with PH. An 84-year-old woman was scheduled to undergo percutaneous left atrial appendage closure for non-valvular atrial fibrillation. She had developed secondary PH and RV failure secondary to atrial fibrillation and systemic sclerosis, necessitating careful general anes

Double-outlet right ventricle with left malposition of the great arteries and total anomalous pulmonary venous connection.

The patient was diagnosed with double-outlet right ventricle with left malposition of the great arteries, total anomalous pulmonary venous connection, and pulmonary stenosis at 2 months of age. Because the progression of pulmonary venous obstruction was suspected at 2 years of age, the patient was catheterized and underwent a definitive repair. The patient had a right aortic arch and left juxtaposition of the atrial appendages; the intraventricular anatomy exhibits malalignment between the conal septum and the muscular intraventricular septum, and the conal septum is well developed and dextrop

Cardiac Computed Tomography in Structural Heart Interventions: From Preprocedural Planning to Procedural Strategy.

Cardiac computed tomography (CT) has become an essential imaging modality in structural cardiac interventions, providing high-resolution anatomical and functional assessments. Moreover, the role of cardiac CT spans pre-procedural planning, intra-procedural guidance, and post-procedural follow-up in interventions such as transcatheter aortic valve implantation (TAVI), mitral, tricuspid, and pulmonary valve interventions, left atrial appendage occlusion (LAAO), atrial septal defect (ASD), and paravalvular leak (PVL) closures. Furthermore, compared to traditional imaging techniques, cardiac CT of

Anti-Thrombotic Therapy Following Transcatheter Structural Heart Intervention.

Transcatheter structural heart interventions, including aortic, mitral and tricuspid valve replacement or repair, and patent foramen ovale, atrial septal defect, and left atrial appendage closure, have dramatically expanded over the past two decades, providing substantial improvements in both clinical outcomes and quality of life. These interventions are performed in a high-risk patient population, which is at risk for both thrombotic and bleeding complications. The introduction of prosthetic devices into the arterial or venous circulation under heterogeneous hemodynamic conditions inevitably

[Intracerebral hemorrhage under platelet inhibition and oral anticoagulation in patients with cerebral amyloid angiopathy].

Oral anticoagulation in patients with cerebral amyloid angiopathy is a therapeutic challenge. The association of cerebral amyloid angiopathy with intracerebral hemorrhage, a high mortality of intracerebral hemorrhage especially under oral anticoagulation and the high risk of recurrent bleeding require a multidisciplinary approach and a thorough risk-benefit analysis. Vitamin K antagonists increase the risk of intracerebral bleeding and the accompanying mortality by 60% and should be avoided if possible or reserved for special clinical situations (e.g. mechanical aortic valve replacement). Outcomes of repair of left partial anomalous pulmonary venous connection in children.

Herein, we report a case series of patients who underwent repair of left partial anomalous pulmonary venous connection (L-PAPVC) via anastomosing the anomalous pulmonary vein (PV) to the left atrial appendage. Fifteen children underwent repair of L-PAPVC between 1980 and 2014. The median age at surgery was 3.6 years (range: 5 days to 17.2 years). Concomitant anomalies were present in 87% (13/15). There were no early deaths. There was 1 late death occurring 63 days following surgical repair due to pneumococcal septicaemia in a patient with prior atrial septal defect closure and Ehlers-Danlos sy

Feasibility of closed-chest ligation of the left atrial appendage in humans.

BACKGROUND: Atrial fibrillation is associated with an increased risk of embolic events. The left atrial appendage (LAA) is believed to be an incubator for thrombus formation. LAA exclusion has been advocated to potentially reduce embolic events arising from the LAA. OBJECTIVE: The aim of the study was to determine the feasibility of a closed-chest surgical suture ligation of the LAA in man. METHODS: Thirteen patients undergoing either mitral valve surgery (n = 2) or electrophysiological study and radiofrequency catheter ablation for atrial fibrillation (n = 11) underwent ligation of the LAA

Management strategies and outcomes of thromboembolism prevention in atrial fibrillation co-existing with immune thrombocytopenia: A review of evidence.

This review aimed to assess bleeding risks and explore management options in atrial fibrillation (AF) patients with immune thrombocytopenia (ITP), aiming to formulate an optimal therapeutic approach for improved patient prognosis. Employing MeSH terms, a comprehensive search strategy identified articles on bleeding risks and management guidelines in AF combined with ITP. Original research papers were included, while animal studies, reviews, and non-English articles were excluded. From four databases, 1891 articles were initially retrieved, resulting in 10 relevant full-text articles. Eight stu

Sequential multidisciplinary minimally invasive therapeutic strategy for heart failure caused by four diseases: A case report.

BACKGROUND: The coexistence with patent ductus arteriosus (PDA), mitral valve prolapse (MVP), atrial fibrillation (AF) and hyperthyroidism is extremely rare and complex. The optimal therapeutic strategy is difficult to develop. CASE SUMMARY: A 27-year-old female with PDA, MVP, AF and hyperthyroidism presented with severe dyspnea. Given that a one-stage operation for PDA, MVP and AF is high risk, we preferred a sequential multidisciplinary minimally invasive therapeutic strategy. First, PDA transcatheter closure was performed. Hyperthyroidism and heart failure were simultaneously controlled vi

Thoracoscopic Left Atrial Appendage Clipping: A Multicenter Cohort Analysis.

OBJECTIVES: This study sought to document the closure rate, safety, and stroke rate after thoracoscopic left atrial appendage (LAA) clipping. BACKGROUND: The LAA is the main source of stroke in patients with atrial fibrillation, and thoracoscopic clipping may provide a durable and safe closure technique. METHODS: The investigators studied consecutive patients undergoing clipping as part of a thoracoscopic maze procedure in 4 referral centers (the Netherlands and the United States) from 2012 to 2016. Completeness of LAA closure was assessed by either computed tomography (n = 100) or transesop
