Primary Bladder Lymphoma with Extravesical Extension: A Case Report and Literature Review on Prognosis and Clinical Characteristics.

Background: Primary bladder lymphoma is generally regarded as having a favorable prognosis due to the predominance of low-grade lymphomas confined to the bladder. However, our investigation reveals that cases with extravesical extension, predominantly involving diffuse large B-cell lymphoma (DLBCL), exhibit a distinct clinical course with varied prognostic outcomes. Methods: In this report, we present and analyzed the clinical features and outcomes of 47 patients with primary bladder lymphoma with extravesical extension, including the case that we experienced. Results: An 77-year-old man who e

Can we identify the risk factors for SIRS/sepsis after percutaneous nephrolithotomy? A meta‑analysis and literature review.

With the increase in percutaneous interventions such as percutaneous nephrolithotomy (PCNL) for renal lithiasis, infectious complications are becoming more frequent. The present study performed a systematic Medline and Embase databases search, using the following words: 'PCNL' [MeSH Terms] AND ['sepsis' (All Fields) OR 'PCNL' (All Fields)] AND ['septic shock' (All Fields)] AND ['urosepsis' (MeSH Terms) OR 'Systemic inflammatory response syndrome (SIRS)' (All Fields)]. Because of the technological advances in endourology, articles published between 2012 and 2022 were searched. Of the 1,403 resu

Removal of intramural trapped intrauterine device by cystoscopic incision of bladder wall.

A healthy 37 - year - old woman referred to our clinic with one - year history of recurrent urinary tract infection, dysuria and frequency. Her past medical history informed us that an IUD (Copper TCu380A) had been inserted 11 years ago. Eleven months after the IUD insertion she had become pregnant, unexpectedly. At that time, she had undergone gynecological examination and abdominal ultrasound study. However, the IUD had not been found, and the gynecologist had made the diagnosis of spontaneous fall out of the IUD. She had experienced normal pregnancy and caesarian section with no complicatio

Clinical assessment of children with renal abscesses presenting to the pediatric emergency department.

BACKGROUND: Renal abscesses are relatively uncommon in children but may result in prolonged hospital stays and life-threatening events. We undertook this study to analyze the clinical spectrum of renal abscesses in children admitted to the pediatric emergency department (ED) and to find helpful clinical characteristics that can potentially aid emergency physicians for detecting renal abscesses in children earlier. METHODS: From 2004 to 2011, we retrospectively analyzed 17 patients, aged 18 years or younger, with a definite diagnosis of renal abscess admitted to the ED. The following clinical

Once daily isepamicin treatment in complicated urinary tract infections.

Isepamicin is a new aminoglycoside, derived from gentamicin B, which is more stable than other aminoglycosides against inactivating enzymes, and is less nephrotoxic. We evaluated the efficacy and safety of a once daily isepamicin in the treatment of complicated urinary tract infections (UTIs), as compared with amikacin. During the period May, 1997, to January, 1998, a total of 52 patients with similar demographic and baseline characteristics were enrolled into a prospective, randomized, open-label, single-center trial at the Veterans General Hospital-Kaohsiung. Eleven patients were excluded fo

Serological typing of Ureaplasma urealyticum isolates from urethritis patients by an agar growth inhibition method.

An agar growth inhibition method for serotyping Ureaplasma urealyticum is described, and the results of applying this method to serotyping 338 strains of the organism are presented. The serotyped strains consisted of cloned isolates from male patients with primary and recurrent nongonococcal urethritis (NGU), isolates from symptomatic patients with other genitourinary tract infections and disorders, and isolates from asymptomatic carriers of U. urealyticum in the genitourinary tract (controls). Among 122 male patients with NGU, serotype 4 was associated most frequently (52%) with this disease

Single-dose therapy for cystitis in women. A comparison of trimethoprim-sulfamethoxazole, amoxicillin, and cyclacillin.

We evaluated single-dose regimens of trimethoprim-sulfamethoxazole, amoxicillin, and cyclacillin as treatment for acute cystitis in 38 women. The trial was prematurely stopped because of frequent treatment failures. At two days after treatment, all 13 patients given trimethoprim-sulfamethoxazole were cured, while four (31%) of 13 given amoxicillin and four (33%) of 12 given cyclacillin had persistent bacteriuria. At two weeks, 11 (85%) of 13 patients given trimethoprim-sulfamethoxazole, six (50%) of 12 given amoxicillin, and three (30%) of ten given cyclacillin were cured. One patient with pos

One thousand cases of prostatectomy.

A survey of one thousand transvesical prostatectomies was conducted for studying occasional complications. An improved technique is recommended for the removal of adenomas, 30-40 g in weight or above. The number of complications showed a decline which, together with the possibility to reduce the operating and nursing periods, is attributed to some outstanding features of the method such as the use of continuous running haemostatic synthetic sutures for constricting the edges of the prostatic bed, the use of a three-bore catheter with its balloon located inside the bladder, rinsing the bladder

Are RAS inhibitors necessary during intensive immunosuppression for lupus nephritis? Real-world evidence from the STAR cohort.

OBJECTIVE: While guidelines advocate renin-angiotensin system inhibitors (RASi) for renoprotection in lupus nephritis (LN), evidence is largely extrapolated from non-lupus populations. Evidence specifically evaluating RASi efficacy during the intensive induction phase of active LN remains scarce. This study aimed to evaluate the real-world impact of adding RASi to standard induction therapy on renal remission and glucocorticoid tapering in LN. METHODS: This retrospective cohort study used data from the STAR (Treat-SLE-to-Target) registry. Patients receiving ≥3 months of continuous RASi during

Distinct clinical profiles of nongonococcal gram-negative septic arthritis: a retrospective analysis from a tertiary center.

BACKGROUND: Septic arthritis is life-threatening, with gram-positive bacteria as the primary cause. However, gram-negative infections are more prevalent in Asia, often resulting in inadequate empirical therapy and higher mortality. The factors linked to gram-negative septic arthritis remain poorly understood. Our aim was to investigate the prevalence, clinical characteristics, and predictive factors of nongonococcal gram-negative septic arthritis in the northeastern region of Thailand. METHODS: We conducted a retrospective study by analyzing medical records from Srinagarind Hospital, a 1,500-b

The impact of intravesical Double-J stent configuration and dwell time on significant urine culture positivity in pediatric patients.

BACKGROUND: Double-J (DJ) stents are widely used in pediatric urology; however, stent-related urinary tract infection (UTI) remains a significant source of morbidity. Although prolonged indwelling time is a recognized risk factor, the impact of intravesical distal coil configuration on infection risk has not been systematically evaluated in pediatric cohorts. METHODS: This retrospective, single-center cohort study included 144 pediatric patients (<18 years) who underwent unilateral DJ stent placement following pyeloplasty for ureteropelvic junction obstruction (UPJO) or after endourological i

Whole-genome characterisation of Escherichia coli isolates from older women with urinary tract infection and asymptomatic bacteriuria.

OBJECTIVES: Diagnosing urinary tract infections (UTIs) in older women is challenging due to the high prevalence of asymptomatic bacteriuria (ASB). This study aimed to investigate whether pathogen-related factors differ between older women with UTI and those with ASB, and to gain further insight into the persistence of Escherichia coli in ASB. METHODS: In this exploratory study, E. coli isolates from 46 participants were analysed. The study included community-dwelling and long-term care facility women aged ≥ 65 years diagnosed with either UTI (defined as ≥ 2 new-onset lower urinary tract sympt

Identification of Risk Factors in Patients with Recurrent Cystitis May Improve Individualized Management.

Background/Objectives: Management of acute episodes of lower urinary tract infection (LUTI) depends on whether they are sporadic or recurrent. We aimed to define factors that differentiate patients with acute sporadic cystitis (AC) from those with recurrent cystitis (RC) and thereby improve individualized care. Methods: We performed a post hoc analysis of prospectively collected data from the multinational GPIU.COM study. Female patients with an acute LUTI episode completed the Acute Cystitis Symptom Score (ACSS) and underwent a routine clinical and laboratory evaluation, including a physical

Serious bacterial infections in infants younger than 90 days of age with acute bronchiolitis.

UNLABELLED: Although acute bronchiolitis is primarily caused by viral infections, antibiotics are often administered to children with acute bronchiolitis. Due to concerns about serious bacterial infections (SBI), neonates and young infants are particularly prone to antibiotic overuse. This study aimed to identify the clinical characteristics and prevalence of SBI in neonates and infants aged < 90 days with acute bronchiolitis. We retrospectively reviewed the medical records of 651 neonates and infants aged < 90 days hospitalized with acute bronchiolitis between September 2015 and August 2024.

Granulomatous Tubulointerstitial Nephritis as a Rare Cause of Allograft Failure: A Case Report.

Although granulomatous interstitial nephritis (GIN) is a rare histological finding in kidney transplants, the joint occurrence of GIN and focal segmental glomerulosclerosis (FSGS) has not, to our knowledge, been reported in the literature. We report a case of GIN and de novo FSGS in kidney transplant recipients leading to allograft failure. A 69-year-old male with a history of end-stage renal disease (ESRD) of unknown etiology, as well as liver failure from hepatitis B and C co-infection, initially had a living unrelated kidney transplant (LURT) in 2007 and subsequently received both liver and
