Histologic and radiographic assessment of caries-like lesions localized at the crown margin.

STATEMENT OF PROBLEMS: Little is known about the impact of crown margins on the demineralization and remineralization process or on the interpretation of radiographs for the detection of early secondary caries. PURPOSE: The aim of this study was to correlate the findings from 2 different radiographic techniques with the results of a histologic assessment of carieslike lesions at the crown margin. MATERIAL AND METHODS: Ten fully impacted third molars were extracted and restored with complete metal crowns with chamfer preparations. For each tooth, the prepared margin ended in enamel on one sid

Case series and review of glandular odontogenic cyst with emphasis on treatment modalities.

Glandular odontogenic cyst is a newly determined jaw entity with aggressive behavior and a high rate of recurrence. There is histopathologic resemblance to other lesions of the jaw such as intraosseous mucoepidermoid carcinoma. Although enucleation and curettage are not the treatment of choice for this cystic lesion, they comprise the most common method. On the other hand, filling the defect is a controversial matter, especially in lesions with large size. We introduce 4 cases of GOC, of which 2 are cases of recurrence. We applied bone material substitutes in 3 of these cases with success.

Sinus grafting with mineralized allograft and staged implant placement.

Advanced atrophy of the partially edentulous maxilla necessitates bone augmentation as a prerequisite for successful implant osseointegration. This case shows that sinus grafting at the left first molar (free-end gap) by means of particulated mineralized solvent-preserved cancellous bone and sealing of the recipient site with a bioabsorbable xenogenic collagenous membrane provides optimum conditions for a staged implant placement. Four months after sinus membrane elevation, a trephine biopsy was recovered from the augmented floor and a microtextured implant placed into the augmented area. The

Clinical, radiographic, and histologic evaluation of localized ridge augmentation using a mandibular bone block.

This clinical case report presents a new technique for reconstructing the vertical and horizontal dimensions of the alveolar ridge using a mandibular bone block at sites planned for single implants. Augmentation was eased with the use of a mandibular bone block, which was shaped after placement with a round bur to passively fit the dimensions of the defect, at the recipient site. After treatment, approximately 7 mm of horizontal and 3 mm of vertical augmentation were gained. Histologic observation showed that the grafted bone was well integrated with the original bone. The volume of tissue gai

Treatment of gingival recession using a collagen membrane with or without the use of demineralized freeze-dried bone allograft for space maintenance.

BACKGROUND: Studies utilizing collagen membranes for guided tissue regeneration (GTR)-based root coverage procedures have reported promising results. However, creating and maintaining space underneath the membrane remains a challenge. Therefore, the purpose of this clinical trial was to determine whether the addition of bone graft (i.e., demineralized freeze-dried bone allograft [DFDBA]) significantly affects the outcome of collagen membrane GTR-based root coverage procedures. METHODS: Twenty patients participated. One Miller's Class I or II recession defect per patient was treated with a col

The implant-supported milled-bar mandibular overdenture.

Osseointegrated dental implants have been proven successful in the treatment of edentulism. The predictability of the implant-supported prosthesis has also been established. Several techniques have been described for the successful restoration of the edentulous mandible: fixed-detachable prostheses with either the original Brånemark hybrid prosthesis design or conventional implant-supported fixed partial dentures, implant-retained overdentures, and implant-supported overdentures. However, in cases of advanced ridge resorption in which facial tissue support is needed from the flanges of the pro

Root coverage employing an envelope technique or guided tissue regeneration with a bioabsorbable membrane.

BACKGROUND: Different treatment modalities have been described for root coverage in cases of gingival recession. The aim of the present study was to evaluate the postsurgical outcome of 2 modes of surgical root coverage of predominantly shallow, Class I or II, gingival recessions. METHODS: Fourteen buccal recession sites in 13 patients were treated with free connective tissue grafts employing a modified envelope technique; 14 sites in 9 patients were subjected to a coronally repositioned flap in combination with a bioabsorbable membrane. Immediately before surgery as well as after 6 months, g

A double blind randomized clinical trial comparing lingualized and fully bilateral balanced posterior occlusion for conventional complete dentures.

PURPOSE: A lingualized occlusion (LO) for complete dentures reduces lateral inferences and occlusal force contacts and direction; thus, LO is theorized to be more suitable for patients with compromised ridges than fully bilateral balanced articulation (FBBA). However, no studies have yet provided evidence to support LO in edentate patients with compromised alveolar ridges. The purpose of this study was to compare LO and FBBA in edentulous individuals with compromised ridges. METHODS: Sixty edentulous individuals were randomly allocated into groups and received dentures with either LO or FBBA.

Periodontal repair in dogs: rhBMP-2 significantly enhances bone formation under provisions for guided tissue regeneration.

The cystic fibrosis transmembrane regulator (CFTR) should no longer be viewed primarily as a 'chloride channel' but recognized as a channel that also controls the efflux of other physiologically important anions, such as glutathione (GSH) and bicarbonate. More effective approaches to cystic fibrosis treatment may result from this reconceptualization of the CFTR by researchers and clinicians. For example, oxidant damage in cystic fibrosis has been assumed to be a significant part of the pathophysiology of the disease. Generally speaking, antioxidant status in cystic fibrosis is compromised. How

Recombination hot spot in a 3.2-kb region of the Charcot-Marie-Tooth type 1A repeat sequences: new tools for molecular diagnosis of hereditary neuropathy with liability to pressure palsies and of Charcot-Marie-Tooth type 1A. French CMT Collaborative Research Group.

Charcot-Marie-Tooth type 1A (CMT1A) disease and hereditary neuropathy with liability to pressure palsies (HNPP) are autosomal dominant neuropathies, associated, respectively, with duplications and deletions of the same 1.5-Mb region on 17p11.2-p12. These two rearrangements are the reciprocal products of an unequal meiotic crossover between the two chromosome 17 homologues, caused by the misalignment of the CMT1A repeat sequences (CMT1A-REPs), the homologous sequences flanking the 1.5-Mb CMT1A/HNPP monomer unit. In order to map recombination breakpoints within the CMT1A-REPs, a 12.9-kb restrict

The efficacy of full-arch immediately restored implant-supported reconstructions in extraction and healed sites: a 36-month retrospective evaluation.

PURPOSE: Implants placed in fresh extraction sites and healed sites were restored simultaneously by cross-arch provisional fixed prostheses. Clinical and radiographic parameters were recorded for up to 36 months. MATERIALS AND METHODS: Treatment with a full-arch implant prosthesis, either screw-retained or cemented, was assigned to 54 patients. A total of 676 implants were placed in either immediate extraction sites (n = 367) or in healed alveoli (n = 309), followed by placement of a one-piece provisional prosthesis. The definitive restoration was placed 3 to 6 months after implant placement.

Guided tissue regeneration: comparison of bioabsorbable and non-bioabsorbable membranes. Histologic and histometric study in dogs.

This study examined histologic and histometric responses to 2 bioabsorbable membranes made from a synthetic copolymer of glycolide and lactide. They were tested for their biocompatibility, resorption characteristics, and ability to support periodontal regeneration. Expanded polytetrafluoroethylene (ePTFE) was used as control. Nine fox hound dogs with no periodontal disease were used. They were sedated and their teeth thoroughly scaled and root planed. Plaque control was maintained. Two weeks later, each dog was anesthetized using gas anesthesia. Buccal and lingual mucoperiosteal flaps were ref

Site development in the posterior maxilla using osteocompression and apical alveolar displacement.

Implant placement in the posterior maxilla is often compromised by reduced bone quality and limited bone height beneath the sinus floor. Techniques have been developed using osteotomes to improve localized bone density through osteocompression and provide for additional implant length through apical alveolar displacement. The clinical indications and proposed limitations for these procedures are presented along with the biologic rationale based on relevant literature. The author cites his personal experience with osteotome procedures and presents procedural modifications to aid in simplifying

Surgical treatment of intrabony periodontal defects using expanded polytetrafluoroethylene barrier membranes: influence of defect configuration on healing response.

Twenty-six proximal, intrabony periodontal defects with probing depths equal to or exceeding 6 mm in 23 patients were treated with gingival flap surgery supported by an expanded polytetrafluoroethylene barrier membrane. The material included 1, 2, and 3-wall defects with crestal involvement relative to the root circumference ranging between 90 degrees and 270 degrees. Healing was evaluated clinically 12 months after surgery. Deep defects exhibited greater probing depth reduction and attachment gain than shallower defects. Probing depth reduction, attachment gain, and bone fill were positively

Guided tissue regeneration using biodegradable membranes of polylactic acid or polyurethane.

The purpose of the present investigation was to evaluate the use of biodegradable membranes of polylactic acid or polyurethane for obtaining regeneration following treatment of circumferential periodontal defects in monkeys with the "guided tissue regeneration" procedure. Orthodontic elastic bands were placed around selected test and control teeth in 4 monkeys in order to induce breakdown of the periodontal tissues. When the destruction of the supporting tissues had progressed to a level corresponding to approximately half the length of the roots, the elastic bands were removed. After 1 to 8 m

Physicochemical and remineralisation properties of calcium silicate and glass ionomer cements applied in simulated caries-affected dentine.

Calcium silicate-based and glass ionomer cements are often used in caries-affected dentine. This study investigated the remineralising and physicochemical properties of two calcium silicates and a glass ionomer cement. Human molars with Class I cavities were subjected to pH cycling to simulate caries lesions. These were restored using two different calcium silicate cements (CTR-MTA or MTA-VOCO) or a glass ionomer cement (GIC). After storage in artificial saliva, Knoop microhardness (KHN) was measured within a 300-µm zone from the interface. Solubility, pH, calcium ion release, scanning electro
