Early lymphocyte trapping in malaria infections: a particulate antigen mediated phenomenon.

During the course of rodent malaria a marked decrease in the numbers of circulating lymphocytes within the peripheral blood occurred 2-4 days post-infection. Monocytes and polymorphs did not show the same degree of decline. For both avirulent Plasmodium yoelii and lethal Plasmodium berghei infections lymphocyte numbers returned to control levels by day 6-8 post-infection. While these levels were maintained until clearance of P. yoelii infection, a sustained and abnormal increase occurred during P. berghei infection. Early lymphocyte depletion was also observed following Babesia microti and Pla

Hemagophagocytic lymphohistiocytosis associated with recurrent Babesiosis with Lyme disease co-infection in an immunocompromised host on anti-CD20 monoclonal antibody therapy: A case report.

In the Northeastern United States, a nationally notifiable emerging infectious disease caused by a protozoan infecting erythrocytes is endemic. Transmitted by a deer tick, Ixodes scapularis, this protozoan, Babesia microti, has a complex life cycle including multiple mammalian hosts with humans affected as a dead end reservoir. Although humans are not necessary for the survival of this protozoan, if human erythrocytes are infected by it, especially in a host that is immunocompromised, devastating clinical illness with a significant risk of mortality occurs. Erythrocytic infection of the human

Detection, education and management of the asplenic or hyposplenic patient.

Fulminant, potentially life-threatening infection is a major long-term risk after splenectomy or in persons who are functionally hyposplenic as a result of various systemic conditions. Most of these infections are caused by encapsulated organisms such as pneumococci, Haemophilus influenzae and meningococci. A splenectomized patient is also more susceptible to infections with intraerythrocytic organisms such as Babesia microti and those that seldom affect healthy people, such as Capnocytophaga canimorsus. Most patients who have lost their spleens because of trauma are aware of their asplenic co

A Two-Phase Approach to the Use of Tafenoquine Was Associated with Cure of Babesiosis in Two Immunocompromised Patients with Relapsing Babesiosis.

Two immunocompromised patients with relapsing babesiosis were cured using a well-tolerated course of tafenoquine (171 and 212 days, respectively), with negative polymerase chain reactivity intentionally exceeding the 120-day lifespan of erythrocytes before discontinuing the tafenoquine. During the first 10 days of tafenoquine both patients also received an atovaquone-containing drug regimen. This was prescribed based on the demonstrated benefit of atovaquone on the efficacy of tafenoquine for completely eradicating B. microti in an animal study.

Precision medicine: retrospective chart review and data analysis of 200 patients on dapsone combination therapy for chronic Lyme disease/post-treatment Lyme disease syndrome: part 1.

PURPOSE: We collected data from an online survey of 200 of our patients, which evaluated the efficacy of dapsone (diaminodiphenyl sulfone, ie, DDS) combined with other antibiotics and agents that disrupt biofilms for the treatment of chronic Lyme disease/post-treatment Lyme disease syndrome (PTLDS). We also collected aggregate data from direct retrospective chart review, including laboratory testing for Lyme, other infections, and associated tick-borne coinfections. This helped us to determine the frequency of exposure to other infections/coinfections among a cohort of chronically ill Lyme pat

Ticks and tick-borne microbes identified through passive and active surveillance in Alaska.

Rapid environmental change in Alaska and other regions of the Arctic and sub-Arctic has raised concerns about increasing human exposure to ticks and the pathogens they carry. We tested a sample of ticks collected through a combination of passive and active surveillance from humans, domestic animals, and wildlife hosts in Alaska for a panel of the most common tick-borne pathogens in the contiguous United States to characterize the diversity of microbes present in this region. We tested 189 pooled tick samples collected in 2019-2020 for Borrelia spp., Anaplasma spp., Ehrlichia spp., and Babesia

Development and validation of a pan-Plasmodium molecular assay as a confirmatory method for blood donor screening.

BACKGROUND: Transfusion-transmitted malaria remains a concern in non-endemic regions due to asymptomatic parasitemia in donors with prior residence in or travel to malaria-endemic areas. Nucleic acid testing (NAT)-based screening has been proposed to mitigate this risk, and supplemental assays are needed to confirm reactive donations. We developed a pan-Plasmodium reverse transcription quantitative PCR (RT-qPCR) assay with species identification capability as a supplemental tool for malaria donor screening. STUDY DESIGN AND METHODS: The assay targets a conserved region of the 18S rRNA shared

Nested qPCR assay to detect Babesia duncani infection in hamsters and humans.

Human babesiosis is caused by Babesia duncani that is transmitted through tick bites, blood transfusions, and transplacental transmission. Despite its health burden, diagnostic assays for this pathogen are either unsuitable for clinical applications or have a low detection efficiency; therefore, it remains undetected during transfusion and utilization of blood and blood-component transfusions. This study used a molecular approach via nested quantitative polymerase chain reaction (qPCR) by designing primers and probes corresponding to the variable regions of B. duncani 18S rRNA gene to specific

A historical snapshot of Ixodes scapularis-borne pathogens in New Jersey ticks reflects a changing disease landscape.

Historical specimens, when available, can provide new insight into the distribution and evolution of pathogens that may not be discernible from more recent samples. We used ticks collected from hunter-killed white-tailed deer in New Jersey in 2002 to examine the prevalence and distribution of four pathogens transmitted by Ixodes scapularis, the blacklegged tick. Infection with Borrelia burgdorferi sensu stricto, Babesia microti, and Anaplasma phagocytophilum (the agents of Lyme disease, human babesiosis, and human granulocytic anaplasmosis, respectively) was highest in the Coastal Plain and lo

[Human babesiosis - a rare but potentially dangerous zoonosis].

Human babesiosis is caused by the intraerythrocytic parasite of the genus Babesia (phylum Apicomplexa). Humans are commonly infected by the bite of Ixodid ticks. Rarely, transmission does occur perinatal or via contaminated blood transfusion. There is only insufficient data available on the clinical relevance in Europe, whereas there are known endemic states in the United States with an increasing importance of the disease in transfusion medicine. The following article gives an overview of the situation in Germany. Human babesiosis is a zoonotic disease with a worldwide increasing importance a

Babesiosis in splenectomized adults. Review of 22 reported cases.

Since 1957, there have been 22 reported cases of human babesiosis in splenectomized persons, representing about one third of all clinical human babesiosis. Splenectomy had been performed one month to 36 years (mean 8.7 years, median 6.0 years) earlier for a variety of reasons. Four of the seven European cases were from Babesia divergens whereas 12 of the 15 United States cases were from B. microti. Most of the 22 patients had moderate to severe clinical disease including hemolytic anemia, yet all but six recovered. Three patients had transfusion-acquired babesiosis. Treatments employed include

Infection with agents of human granulocytic ehrlichiosis, lyme disease, and babesiosis in wild white-footed mice (Peromyscus leucopus) in Connecticut.

White-footed mice, Peromyscus leucopus, were captured in southern Connecticut during 1997 and 1998 to determine the prevalence of infections caused by granulocytic Ehrlichia sp., Borrelia burgdorferi, and Babesia microti. Of the 50 mice captured and recaptured, 25 of 47 (53.2%) and 23 of 48 (47.9%) contained antibodies to the BDS or NCH-1 Ehrlichia strains, respectively, as determined by indirect fluorescent antibody (IFA) staining methods. The majority (83.3%) of 48 mice also contained antibodies to B. burgdorferi, as determined by enzyme-linked immunosorbent assay. Moreover, 20 of 26 (76.9%)

Development of a capture sequencing assay for enhanced detection and genotyping of tick-borne pathogens.

Inadequate sensitivity has been the primary limitation for implementing high-throughput sequencing for studies of tick-borne agents. Here we describe the development of TBDCapSeq, a sequencing assay that uses hybridization capture probes that cover the complete genomes of the eleven most common tick-borne agents found in the United States. The probes are used for solution-based capture and enrichment of pathogen nucleic acid followed by high-throughput sequencing. We evaluated the performance of TBDCapSeq to surveil samples that included human whole blood, mouse tissues, and field-collected ti

Using citizen science to describe the prevalence and distribution of tick bite and exposure to tick-borne diseases in the United States.

Tick-borne pathogens are increasing their range and incidence in North America as a consequence of numerous factors including improvements in diagnostics and diagnosis, range expansion of primary vectors, changes in human behavior, and an increasing understanding of the diversity of species of pathogens that cause human disease. Public health agencies have access to human incidence data on notifiable diseases e.g., Borrelia burgdorferi, the causative agent of Lyme disease, and often local pathogen prevalence in vector populations. However, data on exposure to vectors and pathogens can be diffi

Human babesiosis is caused predominantly by B. microti and B. divergens, a protozooan parasites of red blood cells. Both are transmitted by Ixodes ricinus ticks, also the primary vector of Lyme disease. Clinical manifestation varied widely from asymptomatic infection to a serve rapidly fatal disease. The diagnosis of babesiosis include examination of stained blood smers, serological evaluation indirect antibody tests and PCR. With the evolution PCR--based techniques, the diagnosis and monitoring of babesial infections became more sensitive and reliable.
